Job's not done: BCG-itis as the first manifestation of hyper-IgE syndrome. A case report and review of the literature.

The Bacille Calmette-Guérin (BCG) vaccine is widely used to prevent tuberculosis (TB), especially in newborns and children in high TB prevalence areas. Though generally safe, it can cause adverse reactions like BCG-itis, a localized inflammatory response, and BCG-osis, a systemic infection. This case report details the clinical journey of a male toddler from a non-consanguineous family who presented with regional BCG-itis and later developed a disseminated mycobacterial infection. The patient had a history of recurrent suppurative otitis media and chronic granulomatous inflammation, with mycob

Paroxysmal sympathetic hyperactivity in tubercular meningitis: An underacknowledged association.

INTRODUCTION: Paroxysmal sympathetic hyperactivity (PSH) is most commonly associated with traumatic brain injury. PSH in CNS infections has been quoted variably due to extremely low numbers of its occurrences. With this article, we document a case report of a patient admitted with PSH secondary to tubercular meningitis (TBM), with an attempt to highlight the burden of disease. CLINICAL DISCUSSION: The pathophysiology of PSH is yet to be clearly understood and clinical presentation is ambiguous leading to a delay in identification and, therefore, a delay in management. Despite using the diagno

Imaging Contribution to the Diagnosis of Neuro-Meningeal Tuberculosis and Tuberculous Hypophysitis: A Case Report.

Neuro-meningeal tuberculosis is a severe form of tuberculosis affecting the central nervous system. Tuberculous hypophysitis is a granulomatous inflammation of the pituitary gland, a rare but potentially severe pathology in the context of disseminated tuberculosis. In children, this condition presents significant diagnostic challenges. This article reviews the commonly used imaging modalities, typical radiological signs, and their role in the diagnosis and management of pediatric neuro-meningeal tuberculosis and its complications.

Trial of High-Dose Oral Rifampin in Adults with Tuberculous Meningitis.

BACKGROUND: Tuberculous meningitis is often lethal, and many survivors have disabilities despite antimicrobial treatment and adjunctive glucocorticoid therapy. Standard-dose rifampin has limited central nervous system penetration. Whether high-dose rifampin could improve survival outcomes is unknown. METHODS: We performed a double-blind, randomized, placebo-controlled clinical trial involving adults with tuberculous meningitis in Indonesia, South Africa, and Uganda. We assigned persons with and those without human immunodeficiency virus (HIV) coinfection to receive standard daily isoniazid, r

Brain abscess caused by Mycobacterium avium complex in an adolescent with an inborn error of immunity.

Mycobacterium avium complex (MAC) infections of the central nervous system are rare and represent a diagnostic challenge, especially in immunocompromised patients. Comprehensive management requires early diagnosis, prolonged antimicrobial treatment, and, in some cases, neurosurgical interventions. We present the case of a 16-year-old patient with an inborn error of immunity caused by a variant in the NFKB1A gene, who consulted for paresis of the left upper limb. A diagnosis of brain abscess was made using central nervous system (CNS) imaging, and the etiological agent was confirmed by neurosur

Persistent Undocumented Meningoencephalitis due to Histoplasmosis in an HIV-Infected Patient in Brazil.

The diagnosis of central nervous system histoplasmosis is challenging because histoplasmosis can mimic tuberculosis and the performance of mycological diagnostic methods in the cerebrospinal fluid is questionable. A 50-year-old woman with HIV infection, living in Amapa (northern Brazil), on suboptimal antiretroviral therapy for 8 years, presented to our unit with a 3-year history of recurrent undocumented meningoencephalitis, with several unsuccessful empirical antituberculosis and partially successful anticryptococcal regimens, and presented progressive motor weakness in the lower limbs, diff

Neurocognitive Outcomes and Their Correlation with Brain-Based Biomarkers in Tuberculous Meningitis: A Prospective Study.

Tuberculous meningitis (TBM) is a serious type of infection that impacts the central nervous system, and it is frequently associated with high mortality and long-term neurological sequelae. Although cerebrospinal fluid (CSF) biomarkers are involved in inflammatory processes in TBM, their relationship with cognitive outcomes has not been explored. We aimed to evaluate the cognitive profile of patients with grades 1 and 2 TBM and its correlation with the various immunological markers as well as brain-based biomarkers. Sixty patients (ages 16 years old and older) with definite and probable TBM (g

Cognitive impairment in tuberculous meningitis: Systematic review & meta-analysis.

Tuberculous Meningitis (TBM) is one of the severe forms of tuberculosis affecting the central nervous system, often leading to long-term cognitive impairment. Understanding these deficits is essential for improving treatment outcomes and guiding rehabilitation strategies. A systematic review and meta-analysis following PRISMA guidelines examined cognitive dysfunction in TBM, identifying affected domains and the need for standardized assessment. The study assessed the extent, pattern, and long-term impact of cognitive impairment in TBM survivors, providing recommendations for optimal assessment

Tuberculous meningitis presenting as MERS Type II syndrome: A case report.

The mild encephalitis/encephalopathy with reversible splenial lesions of the corpus callosum is defined as a clinical-radiological entity that includes the presentation of encephalitis or encephalopathy associated with the presence of lesions visualized on MRI, typically described in the splenium of the corpus callosum. This entity has been associated with multiple etiologies, especially infectious ones; however, little literature discusses its association with tuberculosis. This case report describes a patient who initially presented with symptoms of mild encephalitis/encephalopathy with reve

[Mycobacterium tuberculosis cerebral vasculitis: a pediatric case report].

Tuberculosis of the cerebral nervous system is a diagnostic and therapeutic emergency that can lead to death and sometimes disabling sequelae. It can be complicated by stroke in the context of vasculitis. We report a pediatric case. We report the case of a 13-year-old child who presented with a sudden onset of altered consciousness in the context of fever. He had been experiencing unexamined cervical lymphadenopathy and a cough for a year. On day 4 of hospitalisation, he developed a right pyramidal syndrome. Cerebral magnetic resonance imaging revealed a deep middle cerebral artery infarction.

Universal driving restrictions beyond 4 weeks appear unnecessary following CAR-T therapy in multiple myeloma.

INTRODUCTION: Tuberculosis (TB) is a major global cause of morbidity and mortality. TB meningitis (TBM) is an implication of systemic dissemination of a primary TB infection that indicates a poorer disease prognosis with various long-term neurological sequelae. Consequently, it is crucial to understand the different clinical presentations and manifestations of such condition. In TBM, vision loss, which is one of the most devastating complications, may result from optic nerve inflammation and atrophy. Although unilateral optic neuritis as a primary presenting symptom of TBM has been broadly rep

Pontine tuberculoma presenting as a true-localising abducens palsy.

An immunocompetent woman in her early 30s presented with a 3-day history of nausea, vomiting and horizontal diplopia. Examination revealed left-sided abducens paresis, with normal visual acuity, pupillary reactions and fundus. There was no papilloedema, neck rigidity or positive neurological signs. Magnetic resonance imaging (MRI) showed a central pontine T2 hypointense ring-enhancing lesion causing compression of the fourth ventricle. Magnetic resonance spectroscopy showed a lipid lactate peak leading to a diagnosis of pontine tuberculoma. The diagnosis was supported by a positive Mantoux and

A Case of Tuberculous Meningitis With Concomitant Spinal Co-infection With Tuberculosis and Aspergillosis.

Co-infection with Mycobacterium tuberculosis and Aspergillus in an immunocompetent host is rare but can occur. In this case, we present a patient with central nervous system tuberculosis (TB) and biopsy-proven spinal co-infections with TB and Aspergillosis. We highlight the complexities of treating a TB-Aspergillosis co-infection given drug-drug interactions between standard therapy for both conditions. Using susceptibilities for the Aspergillus and carefully monitoring drug levels of the antifungal agents, we were able to optimally treat the co-infection. This patient was ultimately discharge

Tuberculous Meningitis Complicated by Communicating Hydrocephalus and Lacunar Infarcts: A Case Report.

Tuberculous meningitis (TBM) is a life-threatening form of central nervous system tuberculosis (CNS-TB) that often presents with diagnostic and therapeutic challenges, especially in the absence of early microbiological confirmation, and is often associated with complications. We report the case of a previously healthy 32-year-old female who presented with a short history of fever, headache, and altered mental status. Initial cerebrospinal fluid (CSF) analysis revealed a profile consistent with TBM, though microbiological studies were negative. MRI findings showed meningeal enhancement and vasc

Paradoxical Reactions of Central Nervous System Tuberculosis: Report of Three Immunocompetent Cases.

Introduction: Paradoxical reactions during tuberculosis (TBC) therapy are characterized by clinical or radiological worsening of preexisting tuberculous lesions or the appearance of new manifestations following appropriate TBC treatment. Identifying this phenomenon is crucial, since it can be mistaken with treatment failure or relapse. Although widely described in HIV patients following immune reconstitution inflammatory syndrome, the literature on HIV-negative patients is scarce. Case Series: We present three cases of immunocompetent patients with central nervous system tuberculosis (CNS-TBC)

Unveiling the Uncommon Ventriculitis in Tubercular Meningitis.

Tubercular (TB) meningitis is one of the manifestations of central nervous system tuberculosis, a form of extrapulmonary tuberculosis. Despite the high incidence of TB meningitis in developing countries, there are hardly any reports of associated ventriculitis, making it one of the rare complications. Ventriculitis complicating TB meningitis is devastating not only to the immunocompromised but also to the immunocompetent population. The diagnosis of TB meningitis is indeed challenging, owing to the clinical similarities with other types of meningitis and laboratory techniques that are rather i
