Atypical Presentation of Disseminated Tuberculosis With Third Cranial Nerve Palsy and Aortic Aneurysm: A Case Report.

Tuberculosis (TB) remains a leading cause of mortality among infectious diseases globally, posing significant diagnostic and therapeutic challenges despite being curable. This case highlights a delayed diagnosis of miliary TB with rare and severe complications in a patient with an atypical presentation. A 44-year-old female of African origin presented with non-specific symptoms, including fever, malaise, and deranged liver function tests, after travel to Nairobi. After nearly two months of intermittent symptoms, she developed neurological signs, including a third cranial nerve palsy, eye pain,

Updates in the clinical management of tuberculous meningitis.

INTRODUCTION: Tuberculous meningitis is frequently associated with significant mortality and persistent neurological sequelae. Diagnosis is often delayed due to nonspecific symptoms and the low sensitivity of cerebrospinal fluid (CSF) tests. Standard pulmonary tuberculosis regimens are suboptimal for central nervous system involvement due to poor drug penetration and prolonged treatment. Emerging resistance, particularly to rifampicin and isoniazid, further complicates the management. AREAS COVERED: This review synthesizes recent data particularly on diagnostic tools, drug treatment, host-dir

Tuberculosis Following In Vitro Fertilization: A Systematic Review of Maternal and Newborn Outcomes.

Tuberculosis (TB) in pregnancies conceived via in vitro fertilization (IVF) presents unique diagnostic and management challenges, especially in TB-endemic regions. This systematic review synthesizes evidence on maternal and neonatal TB outcomes after IVF. We conducted a systematic review of case reports, case series, and cohort studies, following PRISMA 2020 guidelines. Databases searched included PubMed, Scopus, Embase, and Google Scholar. Quality assessment was performed using Murad's framework and the Newcastle-Ottawa Scale. Seventy-three IVF pregnancies complicated by maternal TB were anal

Levofloxacin-induced focal seizure in a patient with multidrug-resistant tuberculosis and chronic kidney disease.

Levofloxacin, a fluoroquinolone antibiotic, is essential in the treatment protocols for multidrug-resistant tuberculosis (MDR-TB). Generally well tolerated, it infrequently induces central nervous system adverse effects, especially in patients with renal impairment. We present a woman in her early 50s with chronic renal disease and hypothyroidism who experienced focal seizures after 48 hours of commencing levofloxacin as part of her multidrug-resistant tuberculosis treatment regimen. The seizures exhibited unilateral aberrant movements with initially intact consciousness, thereafter followed b

Neurosarcoidosis in Critical Care, Internal, and Pulmonary Medicine: A Practical Approach.

Neurosarcoidosis is a rare but clinically significant manifestation of sarcoidosis, often presenting with diverse neurologic symptoms that can lead to permanent disability if left untreated. This review aims to provide internists, pulmonologists, nonneurologist clinicians, and critical care specialists with a structured, pragmatic approach to the evaluation, diagnosis, and management of neurosarcoidosis in two distinct patient groups: those with a known diagnosis of systemic sarcoidosis and those with no prior history of sarcoidosis. We emphasize the recognition of key acute clinical syndromes

Challenges and advancements in the diagnosis of central nervous system tuberculosis.

Central nervous system (CNS) tuberculosis is one of the most severe forms of extrapulmonary TB, often leading to substantial morbidity and mortality owing to its complex clinical presentation and frequent diagnostic delays. Despite global TB control efforts, CNS TB continues to present unique difficulties owing to its nonspecific clinical manifestations, the paucibacillary nature of cerebrospinal fluid (CSF), and the limitations of conventional diagnostic methods. The diagnostic dilemma is further compounded in resource-limited settings where advanced molecular tools and neuroimaging may not b

Acute ischemic stroke in tuberculous meningitis.

BACKGROUND: The underlying mechanism for stroke in patients with tuberculous meningitis (TBM) remains unclear. This study aimed to investigate the predictors of acute ischemic stroke (AIS) in TBM and whether AIS mediates the relationship between inflammation markers and functional disability. METHODS: TBM patients admitted to five hospitals between January 2011 and December 2021 were consecutively observed. Generalized linear mixed model and subgroup analyses were performed to investigate predictors of AIS in patients with and without vascular risk factors (VAFs). Mediation analyses were perf

Migration of the anal distal end due to ventriculoperitoneal shunt placement: an atypical case report of a 9-month-old infant with tuberculous meningitis and review of the literature.

BACKGROUND: Ventriculoperitoneal shunt (VPS) represents one of the most classic and widely used treatments for hydrocephalus in pediatric patients. Migration and externalization of the distal end of the catheter through the rectum are extremely rare complications of intestinal perforation with devastating consequences such as meningitis or peritonitis due to enteric bacteria that are significantly life-threatening. Besides, one of the biggest topics with that is that it can happen without producing symptoms, like the patient we present in this case report, which further masks the condition and

FUNCTIONAL AND RADIOLOGICAL OUTCOME FOLLOWING EXTENDED POSTERIOR CIRCUMFERENTIAL DECOMPRESSION IN THE TUBERCULOSIS OF DORSAL SPINE.

Spinal Tuberculosis ranks as one of the most common extrapulmonary varieties of tuberculosis. The study outlines the Extended Posterior Circumferential Decompression (EPCD) procedure for managing tuberculous spondylitis, a prevalent extrapulmonary form of tuberculosis. EPCD involves 360-degree dural decompression, anterior column debridement, and reconstruction following posterior instrumentation. This technique addresses both the infection and associated complications, particularly beneficial in cases with or without paraplegia. EPCD aims to improve outcomes by effectively tackling the pathol

Imaging Assessment of Nontraumatic Pathologic Conditions at the Craniovertebral Junction: A Comprehensive Review.

Nontraumatic pathologic conditions of the craniovertebral junction encompass a range of conditions affecting the complex anatomy of this region without direct physical injury. These conditions include congenital syndromes that predispose individuals to ligamentous laxity, potentially leading to instability. Additionally, rare but noteworthy cases such as Grisel syndrome, a cause of pediatric torticollis, may arise without a traumatic trigger. Inflammatory diseases, including rheumatoid arthritis, ankylosing spondylitis, and crystal deposition, can lead to cervical instability and spinal cord c
