Primary thyroid tuberculosis is a rare form of extra-pulmonary tuberculosis, with only a few cases recorded in the literature, and none of which had thyroid dysfunction. A 6-year-old child came to us with complains of fever and neck swelling for one month. A neck examination indicated a widespread enlargement in the thyroid region, which was more noticeable on the left side. Thyroid function tests indicated elevated TSH (thyroid stimulating hormone) and low T3, T4, indicating hypothyroidism. Contrast-enhanced computed CT revealed enlarged thyroid glands with a collection inside the left lobe.

Calcaneal tuberculosis (TB) is an uncommon manifestation of osteoarticular TB, accounting for <0.1% of skeletal cases. Its nonspecific features such as heel pain and swelling without systemic signs frequently mimic chronic osteomyelitis or bone tumors, often resulting in delayed diagnosis and risk of long-term disability. We report a case of a 20-year-old male presenting with progressive heel pain and swelling. Diagnostic evaluation included plain radiographs, which demonstrated a lytic lesion in the posterior-inferior calcaneus, and magnetic resonance imaging, which revealed marrow involvemen

Tuberculosis (TB) of the sacroiliac (SI) joint is an uncommon manifestation of osteoarticular TB, typically presenting with vague, long-standing back pain that resembles mechanical or inflammatory disorders. Because of its deep anatomical location, paucibacillary load, and lack of awareness, diagnosis is often delayed. We describe an 18-year-old female from a TB-endemic area with a 3-year history of persistent low back pain radiating to the left gluteal region. Despite repeated symptomatic treatment, symptoms persisted. Examination showed SI joint tenderness and positive FABER and Gaenslen's t

BACKGROUND Spinal tuberculosis is difficult to diagnose in patients undergoing maintenance hemodialysis (MHD) because of immunosuppression, atypical clinical manifestations, and the limited sensitivity of conventional microbiological assays. Rapid and accurate pathogen identification is essential to distinguish spinal tuberculosis from other causes of vertebral destruction, including metastatic malignancy and bacterial spondylitis. This report aims to illustrate the diagnostic value of capture-based targeted sequencing for detecting Mycobacterium tuberculosis in extrapulmonary infection when r

BACKGROUND: Lumbar tuberculosis (LTB) is a significant global health concern, often requiring surgical intervention when medical treatment is insufficient. Retroperitoneoscopic debridement offers a minimally invasive approach to manage LTB, potentially reducing complications and recovery time compared to traditional open surgery. However, its efficacy and safety remain understudied. This retrospective cohort study aims to evaluate the clinical outcomes, complication rates, and long-term effectiveness of retroperitoneoscopic debridement in patients with LTB.
METHODS: This retrospective cohort

PURPOSE OF REVIEW: Tuberculous meningitis (TBM) is a severe manifestation of Mycobacterium tuberculosis infection, associated with high mortality and long-term neurological disability. Cerebral ischaemia and infarction are major contributors to poor outcomes, yet the underlying mechanisms remain incompletely understood. This review summarises current understanding of the pathophysiology, predictors, and emerging therapies for TBM-associated ischaemia, highlighting critical research priorities.
RECENT FINDINGS: Stroke in TBM reflects a complex interplay of neuroinflammation, immunothrombosis,

BACKGROUND: Differentiating between spinal tuberculosis, pyogenic (bacterial) spondylitis and spinal metastasis remains a major diagnostic challenge because their radiological features often overlap. Delayed or incorrect diagnosis may lead to inappropriate treatment, permanent disability or death.
OBJECTIVE: To develop and evaluate deep learning models for automated classification of spinal tuberculosis, pyogenic infection, and spinal metastasis using magnetic resonance imaging (MRI).
METHODS: T2-weighted sagittal MRI scans from 120 patients (40 per disease group) with pathologically or micr

BACKGROUND: Spinal tuberculosis (Pott's disease) is a rare form of extra-pulmonary tuberculosis, often presenting with nonspecific symptoms and posing diagnostic challenges, especially in resource-limited settings. While microbiological confirmation is the gold standard, it is often unavailable in district hospitals.
CASE PRESENTATION: I report the case of a previously healthy 28-year-old Black female individual who presented with a 3-week history of progressive lower back pain, fatigue, and functional impairment but without pulmonary symptoms or known tuberculosis contacts. Examination revea

Cryptococcus neoformans is an environmental pathogenic yeast which causes both life-threatening infection and mild, localized infection. Cutaneous cryptococcosis is an uncommon event that usually results from haematogenous dissemination after a primary infection in the pulmonary or other organs, although primary cutaneous infection is possible. The lesions of cutaneous cryptococcosis are not pathognomonic, varying from nodules to ulcers and plaques, and thus can mimic other skin disorders. This case highlights that clinicians need to observe the skin lesions carefully and improve the relevant

OBJECTIVE: Postoperative poor wound healing (PWH) is a significant complication following posterior surgery for thoracolumbar tuberculosis, leading to prolonged recovery and increased healthcare costs. Traditional prognostic markers often lack precision. This study aimed to develop and validate machine learning (ML)-based models to predict PWH and compare their performance with conventional logistic regression and established clinical scoring systems.
METHOD: A retrospective analysis of 188 patients undergoing posterior debridement and internal fixation for thoracolumbar tuberculosis was cond

BACKGROUND: Human immunodeficiency virus (HIV-1)-associated tuberculous meningitis (TBM) is the most lethal form of extrapulmonary tuberculosis, with a high mortality rate in sub-Saharan Africa. Due to the limited diagnostic performance of current testing, empiric treatment is frequently required as untreated TBM is almost universally fatal. Xpert MTB/RIF Ultra (Xpert Ultra) is the World Health Organization recommended first-line diagnostic test for TBM but is costly relative to the standard of care (SOC). We performed a cost-effectiveness analysis of Xpert Ultra for HIV-associated TBM in Zamb

BACKGROUND: Drug-induced liver injury (DILI) management during tuberculous meningitis treatment may result in premature rifampicin and isoniazid withdrawal and contribute to poor outcomes.
METHODS: Adults with tuberculous meningitis, enroled into the ACT HIV (N=520) and LAST ACT (N=720) corticosteroid trials (NCT03092817;NCT03100786), who developed DILI were randomised to strategy-1: continue all drugs, unless ALT ≥10x upper limit of normal [ULN], bilirubin ≥43µmol/L, or symptoms worsen; or strategy-2: stop pyrazinamide, unless ALT ≥5x ULN by day 6, bilirubin ≥43µmol/L, or symptoms worsen; or

PURPOSE: Thoracic spinal tuberculosis (TSTB) can lead to vertebral destruction, spinal instability, and neurological deficits. Paraplegia is one of its most severe manifestations, yet a concise and clinically interpretable framework for assessing neurological severity at presentation remains lacking.
METHODS: We retrospectively reviewed 148 patients with thoracic spinal tuberculosis who underwent surgery at a tertiary referral centre between 2019 and 2024. Clinical, laboratory, and imaging variables associated with paraplegia at presentation were evaluated using logistic regression analysis.

OBJECTIVES: Neurological impairment in thoracic spinal tuberculosis is often related to complex inflammatory and compressive lesions, while the reversibility of neurological deficits remains difficult to predict preoperatively.
METHODS: We retrospectively analyzed 148 patients with thoracic spinal tuberculosis who underwent surgical treatment between 2019 and 2025. Neurological recovery was defined as improvement in American Spinal Injury Association (ASIA) grade at follow-up. Preoperative MRI findings were evaluated according to the anatomical relationship between the compressive lesion, dur

The diagnosis of neuroinfectious diseases, including Tuberculous Meningitis (TBM), viral encephalitis, Human Immunodeficiency Virus (HIV), and post COVID-19 neurological syndromes, remains challenging due to limited surveillance, non-specific symptoms, inadequate laboratory sensitivity, and restricted availability of high-quality CSF or tissue specimens. The critical need for early and precise diagnosis and the development of reliable, validated biomarkers highlight the importance of biospecimen-driven approaches that leverage CSF and other biobanked materials, as evidenced by the emerging lit

Tuberculous meningitis (TBM) is a severe presentation of the Mycobacterium tuberculosis that can cause sensorineural hearing loss (SNHL) similarly to other etiologies of meningitis. The pathophysiology for this group remains unclear. We report a series with 3 temporal bones of infants who died from TBM, showing acute damage to the inner ear, such as fibrosis of the cochlea, organ of Corti atrophy, neuronal loss and internal auditory canal with abundant pus cells, predominantly mononuclear. The cochlear aqueduct, endolymphatic sac/duct and Rosenthal's canal appear to be routes of dissemination
