A 62-Year-Old-Man With Severe Daytime Sleepiness After Primary Central Nervous System Lymphoma.

A 62-year-old man with a history of primary central nervous system lymphoma first presented with diplopia, diagnosed 2 years ago. A brain MRI showed multiple solid enhancing mass lesions located at the midline bilateral dorsal medulla to both cervicomedullary junction and right flocculus with edema of the ventrolateral medulla. The brain biopsy confirmed relapsed diffuse large B cell lymphoma. Remission was achieved after a course of high-dose methotrexate, rituximab, and whole-brain radiotherapy. He was referred to the sleep clinic because of snoring, witnessed apnea, and severe daytime sleep

Development of Mycobacterium tuberculosis post in vitro fertilization and embryo transfer: A case series derived from a multi-omics analysis and literature review.

The extensive progress in assisted reproductive technology has facilitated successful pregnancies through in vitro fertilization and embryo transfer (IVF-ET) for patients facing infertility. Pregnancy induces substantial endocrine and immune alterations that may diminish immune function, consequently heightening vulnerability to Mycobacterium tuberculosis (M.tb) infection or reactivation. Hematogenous disseminated pulmonary tuberculosis (TB), tuberculous meningitis, and potential congenital TB are serious complications that may arise after IVF-ET, posing significant risks to both maternal and

Role of Low-Dose Infliximab for Inflammatory Complications of Central Nervous System Tuberculosis: A Retrospective Cohort Study.

We evaluated low-dose infliximab (5 mg/kg) as adjunctive therapy in 20 patients with severe central nervous system tuberculosis. At 3 months, 60% achieved disability-free survival (modified Rankin Scale ≤ 2), and 75% showed clinically meaningful improvement. Our outcomes matched reports of high-dose infliximab. Randomized trials to optimize dosing are needed.

Pathogenic characteristics of central nervous system infections in AIDS individuals: a retrospective cohort study based on immune status.

BACKGROUND: Human immunodeficiency virus (HIV) infection frequently leads to central nervous system (CNS) complications, especially as immunity declines. While antiretroviral therapy (ART) has transformed HIV into a manageable condition, in clinical practice, a significant subset of individuals still present with advanced immunosuppression (CD4 < 200 cells/μL) due to delayed diagnosis, treatment failure, or poor adherence, leading to AIDS-defining conditions. This study investigated how immune status affects CNS infection characteristics and outcomes specifically within this immunocompromised

Right Temporal Lobe Tuberculoma Presenting as Seizures in a Patient With Pulmonary Tuberculosis.

BACKGROUND Central nervous system (CNS) manifestations of tuberculosis (TB) account for approximately 1-2% of all TB cases worldwide. Tuberculomas, which are granulomatous lesions of the CNS diagnostically confirmed through biopsy, can mimic more common intracranial pathologies, including abscesses and neoplasms. This report describes a patient with pulmonary TB who presented with seizure and was ultimately diagnosed with a right temporal lobe TB mimicking a brain abscess. CASE REPORT A 30-year-old woman with alcohol use disorder, polysubstance use, and pulmonary TB previously treated with rif

Prevalence of Cervical Lymphadenopathy in Acute CNS Infections - Testing Glymphatics in Humans.

BACKGROUND: Glymphatics in the brain have recently been shown to drain cervical lymph nodes in animal models. Diagnosis of acute central nervous system (CNS) infections is limited by pauci-microbial nature of the cerebrospinal fluid (CSF). Evaluation of cervical lymphadenopathy in acute CNS infections may aid in diagnosis and provide evidence of glymphatics. OBJECTIVE: To study the prevalence of cervical lymphadenopathy in acute CNS infections and to assess diagnostic yield by using cervical lymphadenopathy biopsy/FNAC. DESIGN, SETTING AND PARTICIPANTS: A cross-sectional study was conducted

Clinical characteristics of central nervous system talaromycosis: A comparative analysis with cryptococcal meningitis and tuberculous meningitis in patients with AIDS.

BACKGROUND: The incidence of central nervous system (CNS) talaromycosis, an emerging complication in patients with acquired immunodeficiency syndrome (AIDS), is increasing, accompanied by a high mortality rate. Limited understanding of whether its clinical manifestations differ from those of common AIDS-associated CNS infections, such as cryptococcal meningitis (CM) and tuberculous meningitis (TBM), makes diagnosis challenging. Therefore, we aimed to systematically elucidate the key differences in clinical manifestations, diagnostic approaches, and treatment strategies among CNS talaromycosis,

Clinical and hematological factors associated with neurological involvement in spinal tuberculosis: a retrospective study.

BACKGROUND: The association between peripheral blood immune-inflammatory markers and neurological involvement in patients with spinal tuberculosis, and to develop a predictive model for early risk assessment. Spinal tuberculosis with neurological impairment is a major cause of disability, yet simple, cost-effective early warning indicators remain lacking. Peripheral blood immune-inflammatory markers are valuable in infectious disease prognostication, but their predictive role in spinal tuberculosis neurological involvement remains unclear. METHODS: A retrospective study was conducted on 294 p

Ring-enhancing demyelinating lesions in the brain and optic neuritis: atypical presentation of paediatric neuropsychiatric SLE.

Paediatric-onset systemic lupus erythematosus (SLE) is a rare but clinically significant condition, often presenting with central nervous system (CNS) involvement. The most common CNS manifestations include headaches, seizures and psychosis. We present two cases of neuropsychiatric SLE with unusual clinical and imaging features. The first case involves an adolescent girl who presented with unilateral vision loss and elevated intracranial pressure. MRI revealed large well-defined ring-enhancing lesions in the bilateral subcortical white matter. She underwent extensive evaluation for infectious,

Brainstem Tuberculous Abscess in a 63-Year-Old Adult With Unrepaired Tetralogy of Fallot.

BACKGROUND: Tetralogy of Fallot (TOF) is a congenital heart anomaly usually corrected surgically in childhood. Reports of adults with unrepaired TOF indicate that they are at high risk of endocardial infections and sequelae such as distant abscesses. CASE PRESENTATION: A 63-year-old man born and raised in Vietnam presented with unrepaired TOF and 5 days of dizziness, dysarthria, and left-sided weakness. Magnetic resonance imaging (MRI) of the brain demonstrated a 2.9-cm rim-enhancing pontine lesion with associated edema and a punctate acute infarct in the right cerebellum. Echocardiogram of t

Concurrent Spinal Dural Arteriovenous Fistula and Varicella-Zoster Virus Meningoencephalitis Unmasked by Corticosteroid-Associated Deterioration: A Case Report on the Diagnostic Value of Serial mNGS.

BACKGROUND: Concurrent spinal dural arteriovenous fistula (SDAVF) and varicella-zoster virus (VZV) meningoencephalitis are exceptionally rare, and overlapping features can delay diagnosis. This case adds to the literature by illustrating how corticosteroid exposure before exclusion of vascular and infectious mimics may be followed by neurological deterioration, and by emphasizing the diagnostic value of serial metagenomic nextgeneration sequencing (mNGS). CASE PRESENTATION: A 48-year-old man developed insidious bilateral lower-limb weakness that progressed to numbness, sphincter dysfunction,

Cerebral tuberculoma in pregnancy (Jan 1975-May 2025): a systematic review and descriptive analysis of 33 published cases.

BACKGROUND: Cerebral tuberculoma is an infrequent but life-threatening manifestation of tuberculosis whose diagnosis and treatment are uniquely complicated by pregnancy-related physiological changes. Evidence to guide clinicians remains fragmented and largely anecdotal. METHODS: We systematically searched MEDLINE, LILACS and grey literature to May 2025 for reports of cerebral tuberculoma in pregnant or postpartum women. Twenty-seven studies (24 case reports, 2 descriptive studies, 1 case series) from 20 countries met inclusion criteria, yielding individual-level data on 33 patients. Demograph

Co-expression of MARCKS and GSDMD pathway genes in tuberculous meningitis: a multi-omics analysis of blood-brain barrier disruption.

INTRODUCTION: Tuberculous meningitis (TBM), the most severe form of Mycobacterium tuberculosis infection, is characterized by high mortality and neurological sequelae, largely attributed to blood-brain barrier (BBB) disruption. While recent studies identified GSDMD-mediated endothelial pyroptosis as a key mechanism of inflammatory BBB damage, the full molecular landscape in TBM remains unclear. METHODS: This study employed an integrated multi-omics approach, combining bulk and single-cell RNA sequencing of clinical and murine datasets with experimental validation, to identify central mediator

Disseminated Tuberculosis With Cavitary Lung Disease in a Very Preterm Infant.

Tuberculosis (TB) is an important cause of poor health outcomes and mortality for pregnant women and their infants globally. Screening during pregnancy helps diagnose TB at a critical period to avert morbidity and mortality but may be overlooked. TB incidence in the United States is low but has been increasing since 2020, alongside a global resurgence. The Centers for Disease Control and Prevention (CDC) recommends TB screening for all individuals with epidemiologic risk factors, including birth, residence, or prolonged travel in medium- or high-prevalence settings; immunosuppression; and clos

The Clinicoradiological Profile of Ring-enhancing Lesions in the Brain among Children 1 Month-18 Years of Age in a Tertiary Care Hospital.

INTRODUCTION: Ring-enhancing lesions (RELs) in the pediatric brain represent a complex and heterogeneous group of pathological conditions that pose significant diagnostic and therapeutic challenges for clinicians and radiologists. Neuroimaging techniques, particularly magnetic resonance imaging (MRI) with contrast enhancement, play a pivotal role in characterizing these lesions. However, radiological findings alone are often insufficient, necessitating comprehensive clinical correlation and, in some cases, histopathological confirmation. This research aimed to provide a comprehensive analysis
