Planned complex suicides that combine multiple mechanisms pose substantial challenges for medicolegal reconstruction. We report a rare case of a 27-year-old man who simultaneously employed four methods: plastic-bag asphyxia with a continuous natural-gas supply (stove hose taped to bag tubing), environmental sealing of door seams with adhesive tape, rodenticide ingestion, and superficial incised injuries to the neck and wrist. Scene reconstruction, full autopsy, and comprehensive toxicology established asphyxia from natural-gas inhalation via the plastic-bag apparatus as the cause of death; ant

OBJECTIVE: Limited research has explored the impact of predominant back pain (pBP) versus predominant leg pain (pLP) in patients undergoing unilateral biportal endoscopic lumbar decompression (UBE-LD). This study aimed to evaluate and compare the perioperative and postoperative clinical outcomes, using patient-reported outcome measures (PROMs) and minimal clinically important difference (MCID), between patients with pBP and pLP who underwent UBE-LD.
METHODS: Patients who underwent primary UBE-LD were divided into either the pBP or pLP cohort. Exclusion criteria included patients with a diagno

BACKGROUND: Evidence shows functional electrical stimulation (FES) and intermittent theta-burst stimulation (iTBS) improve motor function in spinal cord injury (SCI). It is unclear whether high-definition cathodal transcranial direct current stimulation (HD C-tDCS) preconditioning could enhance the neuromodulatory aftereffect of iTBS to produce greater improvements in walking function.
OBJECTIVE: This study examined the synergistic effects of combining HD C-tDCS preconditioning with iTBS and FES therapy on walking ability in individuals with SCI.
METHODS: Sixty-four participants with incompl

INTRODUCTION: The distal femoral physis makes an important contribution to limb length and is an uncommon bone to fracture in children. Complications associated with the fracture, such as vessel injury in the short term and angular deformities and limb length discrepancies in the long term, can be detrimental.
CASE REPORT: A 7-year-old child presented with a distal femoral physeal injury, Salter-Harris type 2, without any associated neurovascular injury, following a road traffic accident. He was operated on the same day with closed reduction and fixation with multiple K wires. Recovery was un

BACKGROUND: Simultaneous rupture of multiple major tendons is exceedingly rare and is typically associated with high-energy trauma or underlying systemic disease. This report describes the management and outcome of an unusual low-energy, three-tendon rupture involving the quadriceps, patellar, and triceps tendons in a professional bodybuilder.
CASE: A 36-year-old male professional bodybuilder presented after slipping on ice with acute loss of bilateral knee extension and right elbow extension. Physical examination and imaging confirmed complete rupture of the left quadriceps tendon, right pat

RATIONALE: Tibiotalocalcaneal arthrodesis (TTCA) is a radical orthopedic procedure performed in patients with severe ankle and subtalar joint deformities, loss of limb support function, and chronic pain. TTCA in obese patients with multiple comorbidities carries a particularly high risk of postoperative complications. Reporting such cases helps highlight potential challenges and guide clinical management.
PATIENT CONCERNS: We present a case of a 64-year-old morbidly obese patient (BMI 49.3 kg/m²) with a history of bariatric surgery who underwent TTCA.
DIAGNOSES: Surgical site infection is a

Introduction Chronic low back pain (CLBP) is defined as pain in the lower back that persists for more than three months. It is a common musculoskeletal condition and a major contributor to functional impairment and social limitations among young adults. Hamstring flexibility refers to the ability of the hamstring muscles to stretch to their full length. Since both the hamstrings and lumbar extensor muscles originate from the pelvis, a biomechanical relationship may exist between them. Decreased flexibility or weakness in these muscles may contribute to the development of low back pain. Tight h

Motorboat propeller injuries are a rare, but highly fatal type of trauma. In this paper, a case of fatal motorboat propeller injuries is reported. The autopsy findings, including the retrieval of pieces of foreign material in the fractured skull compatible with the propellers of a seized vessel, supported the hypothesis of hemorrhagic shock resulting from severe injuries (both visceral and non-visceral) as the cause of death and a three-dimensional reconstruction and comparison between the body of the victim and the seized vessel showed a high grade of compatibility. A scientific literature re

Cardiopulmonary resuscitation is a critical intervention for cardiac arrest, but its forceful chest compressions can lead to skeletal injuries, including rare vertebral fractures CASE PRESENTATION: This case report details a 78-year-old woman with a history of cardiac surgery and obesity who suffered a fatal cardiac arrest due to a DeBakey type II aortic dissection. Despite 30 minutes of advanced manual CPR, she succumbed to cardiogenic shock. Autopsy revealed an incidental transvertebral fracture of the 8th thoracic vertebra alongside multiple bilateral rib fractures, with no evidence of pre-

Concealing drugs inside the body to transport them covertly, known as 'body packing', can be fatal. Intestinal occlusion and poisoning are common; fatal gastric perforation is unusual. We present a rare case involving a 54-year-old female 'drug mule' (body packer) who died from peritonitis resulting from gastric perforation. The deceased (from Sweden) was discovered dead in her hotel room. A forensic autopsy revealed diffuse peritonitis due to a 2 cm hole in the superior part of the gastric lesser curvature. There were 70 rigid plastic capsules, about 6 cm long and weighing between 3 and 4 g e

INTRODUCTION: Parkinson's disease (PD) patients have multiple co-morbidities, exposures, and medications. Our objective is to evaluate the complexity of features associated with PD in a study of 933 cases and 291 controls in the Harvard Biomarkers Study.
METHODS: The primary analysis evaluated 64 health features for associations with PD using logistic regression adjusting for age and sex. We adjusted for multiple testing using the false discovery rate (FDR) with ≤ 0.05 indicating statistical significance. Exploratory analyses examined feature correlation clusters.
RESULTS: Depression (OR = 3

BACKGROUND: Interpersonal violence represents a major public health and medico-legal concern worldwide. However, data from forensic medicine services in North Africa remain limited. This study aimed to describe injury patterns, temporary functional incapacity, documented physical findings, and psychosocial variables among victims of interpersonal violence examined in a forensic medicine unit in Southern Algeria, and to explore factors associated with documented physical findings and self-reported social life impact.
METHODS: A single-centre prospective observational cross-sectional study was

This case report presents a 25-year-old man who sustained a crush injury to the right lower limb at the workplace due to compression by a car wheel hub. Initial examination revealed marked swelling of the lower leg and midfoot with preserved sensation and peripheral perfusion. Radiographs demonstrated a bimalleolar ankle fracture and multiple foot fractures. Approximately 5 hours after admission, the patient developed rapidly increasing pain, pallor and cooling of the foot, absence of the dorsalis pedis pulse, decreased toe sensation, and oxygen saturation of 80%. CT angiography showed absent

This study describes 16 deaths from Alberta, Canada, caused by ligature strangulation using self-locking zip tie(s), also known as cable ties. All decedents were male, and all ligatures were horizontally oriented. The manner of death was classified as suicide in 15 cases and undetermined in one. Notably, one quarter of cases lacked petechiae, explained by the highly constrictive nature of zip ties and their ratchet locking mechanism. In half of the cases, neck compression resulted from a single ligature formed by one heavy-duty zip tie (6 cases) or several smaller zip ties interconnected to cr

Wound healing is a major medical challenge as acute injuries affect millions annually, while chronic wounds such as diabetic foot and venous leg ulcers persist in 1-2% of patients, driving substantial morbidity and ~4% of global health-care expenditure. In response to an injury, restoration of skin barrier is critical process and regulated at multiple levels involving coordinated communications and interaction among keratinocytes, immune cells, and fibroblasts. In vivo mouse models are essential for dissecting the cellular and molecular regulators of normal and pathological repair, especially

BACKGROUND: Hard-to-heal wounds remain a major healthcare challenge due to persistent inflammation, extracellular matrix (ECM) degradation, impaired angiogenesis and failure to progress through normal healing pathways. Standard wound care alone often fails to achieve timely wound closure, necessitating biologically active therapies that restore the wound microenvironment. High-purity type I collagen (HPTC) (Helicoll, EnColl Corp, Fremont, CA, US) is a uncrosslinked skin substitute designed to function as a biologically active ECM scaffold that supports tissue regeneration rather than serving s

Liquefied petroleum gas (LPG) is commonly associated with explosions, fires, and thermal injuries; however, inhalational exposure can also cause severe or fatal toxicity in the absence of combustion. We comment on a 2026 Legal Medicine report of fatal acute butane poisoning in an elderly woman who was found in an empty bathtub after multiple LPG cartridges had been punctured. The case highlights the potential for gases heavier than air to accumulate in low-lying, recessed spaces and cause lethal inhalational exposure. Butane toxicity may involve multiple mechanisms, including hypoxia, central
