Warfarin is a widely used oral anticoagulant with a narrow therapeutic index and multiple drug-drug interactions that may significantly alter its anticoagulant effect. Torsemide, a loop diuretic commonly prescribed for heart failure and fluid overload, shares metabolic pathways with warfarin, raising the possibility of clinically significant interactions. However, the evidence regarding this interaction remains limited and inconsistent. A 66-year-old male with a history of type 2 diabetes mellitus, hypertension, chronic kidney disease, and prior aortic valve replacement on chronic warfarin the

Chronic lower-extremity ulcers remain a major therapeutic challenge. Conventional treatments-including compression therapy, debridement, dressings, antibiotics, pharmacologic agents, hyperbaric oxygen, and skin grafting-frequently fail to provide durable closure, largely due to limited modulation of the wound-healing cascade. Autologous platelet-rich plasma (PRP) is gaining interest as a regenerative adjunct capable of delivering concentrated endogenous growth factors. We report a 58-year-old woman with more than 20 previous skin grafting procedures for recurrent lower-leg ulcers, arising from

BACKGROUND: Compressed air-induced colonic barotrauma is a rare but potentially fatal injury type with distinctive forensic significance. Non-iatrogenic pneumatic injuries of the gastrointestinal tract are infrequently documented, yet are associated with high morbidity and mortality, frequently arising in occupational settings because of workplace misconduct or the misuse of pneumatic equipment.
CASE PRESENTATION: We report a fatal case of transanal high-pressure compressed air insufflation in a 15-year-old adolescent. Witness testimony and the accused's confession established that a compress

Shotgun injuries provide a unique challenge to both forensic pathology and acute clinical care due to their complex internal ballistic behaviour and variable injury patterns. This report describes a case of a 33-year-old man who initially presented with apparent haemodynamic compensation following a mid-range abdominal shotgun injury; subsequently, he developed rapid respiratory deterioration and died. Post-mortem examination revealed a massive left-sided haemothorax caused not by direct thoracic injury, but by trans-diaphragmatic migration of blood from the abdominal cavity through multiple p

Complex suicide involving sharp-force injury and drowning may closely mimic homicide and requires careful reconstruction of the terminal mechanism. We report an anonymized case of a 27-year-old man found prone in a bathtub containing water mixed with blood after emergency entry into his apartment. Blood traces were documented in several rooms. Autopsy identified 18 stab-incised wounds of the precordial and epigastric regions, hesitation-type superficial injuries, old linear forearm scars, and no defensive injuries. Seven wounds penetrated the thoracic cavity; however, the limited right ventric

BACKGROUND: Clostridium septicum is a highly virulent, gas-forming, anaerobic Gram-positive bacillus known to cause rapidly progressive necrotizing soft tissue infection that is often associated with gastrointestinal malignancy or immunocompromised states. Mortality remains extremely high, with many patients dying within the first 24 h of onset. Although aggressive surgical debridement is the cornerstone of treatment, cases requiring hemipelvectomy are exceedingly rare, and postoperative management of exposed pelvic organs has not been described in the relevant literature.
CASE PRESENTATION:

BACKGROUND: Vancomycin is a core drug for the treatment of methicillin-resistant Staphylococcus aureus, but its neurologic adverse effects are rarely reported. This case is the first to report a possible association between intravenous vancomycin and episodic migraine. Rapid identification of drug-induced migraine can avoid unnecessary invasive testing and medication delays and has direct clinical value.
OBJECTIVE: To report and document a case of episodic migraine that was highly correlated with the timing of intravenous vancomycin administration, assessing causality and its general clinical

This case report describes a rare delayed presentation of a retained knife fragment following penetrating abdominal trauma and prior exploratory laparotomy. A 23-year-old male patient presented seven months after sustaining a stab wound with progressive left lower abdominal, buttock, and radicular leg pain, along with new-onset testicular discomfort. His initial injury required emergent laparotomy for multiple small bowel and mesenteric injuries. Despite an initially uncomplicated postoperative course, persistent and worsening symptoms prompted reevaluation. Computed tomography (CT) revealed a

Cytophagic histiocytic panniculitis (CHP) is a rare and potentially life-threatening inflammatory panniculitis characterized by lobular lymphohistiocytic infiltration and cytophagic "bean-bag" histiocytes. Because its early features overlap with infectious panniculitis and hemophagocytic lymphohistiocytosis (HLH), diagnosis is often delayed, contributing to significant morbidity. We describe a rapidly progressive pediatric case of CHP triggered by antecedent trauma and scrub typhus infection. A 10-year-old girl presented with seven days of high-grade fever and painful erythematous nodules over

BACKGROUND: Infectious bone and soft tissue defects caused by multidrug-resistant bacteria (MDRB) represent infrequent yet challenging medical conditions. Vascularized fibular grafting constitutes a crucial treatment modality in limb reconstruction procedures, offering improved recovery by supplying nutrients and structural support, particularly in large defects and compromised vascularity. The present study assessed the long-term outcomes of flow-through fibula osteocutaneous flaps for reconstructing multidrug-resistant infected bone and soft tissue defects.
METHODS: Between January 2015 and

BACKGROUND: NovoSorb® Biodegradable Temporising Matrix (BTM) is a synthetic polyurethane bilayer matrix, used in trauma and burns reconstruction. However, there is little evidence of its use for paediatric limb ischaemia. We present a review of literature and three paediatric cases of sepsis-induced ischaemic limb injuries, reconstructed using BTM.
METHODS: Data were collected on demographics, management and outcomes of three patients cared for by the senior author and plastic surgery service. The operative regimen and challenges faced are discussed.Three children, aged 1-8 years, were manage

INTRODUCTION: Necrotizing fasciitis (NF) following minor trauma in healthy individuals is rare and poses a considerable diagnostic challenge. Due to its nonspecific early presentation, NF is associated with high rates of misdiagnosis and mortality, underscoring the critical need for early recognition and intervention. CASE PRESENTATION: A previously healthy 37-year-old male developed rapidly progressive necrotizing fasciitis of the scrotum and right lower extremity following minor bicycle-related abrasions. Upon admission, the patient presented with a high-risk Laboratory Risk Indicator for Ne

This case highlights the crucial role of forensic sciences in reconstructing the dynamics of a firearms-related case involving two victims. G.D. died at the scene after being struck by three shots from a 12-gauge firearm loaded with 11/0 buckshot, causing massive bilateral hemothorax and aortic laceration. Autopsy revealed multiple pellet wounds consistent with this ammunition. C.D. was hospitalized with multiple injuries, including three entry wounds to each shoulder and grazing wounds to the left temporal region and right thoracic wall. Examination of seized evidence-cartridge cases, an unex

What role can a rigorous methodological approach play in establishing scientific evidence? This study examines the death of a 36-year-old homeless man in Northern Italy in the late 1990 s. Found unconscious with signs of assault, he was hospitalised in a coma with facial trauma. A brain CT scan showed multiple contusions and a subarachnoid haemorrhage. Due to his condition, he was transferred to another hospital, where he died shortly after. The forensic autopsy, based on the presence of purulent material and leukocytic infiltration in the brain, identified cardiorespiratory failure due to men

Necrotizing fasciitis (NF) is a rapidly progressive, life-threatening infection involving the fascia and subcutaneous tissues. Chest wall NF is rare and typically occurs following thoracic surgery or penetrating trauma. We report a 54-year-old man with type 2 diabetes mellitus who developed bilateral chest wall NF after sustaining a pectoralis major muscle injury while lifting a heavy engine part. Imaging demonstrated bilateral chest wall abscesses with mediastinal extension. During hospitalization, he also developed cellulitis of the contralateral lower limb, which subsequently progressed to

BACKGROUND: Falls are a leading cause of injuries in older adults, often resulting from slipping and tripping. Perturbation-based balance training (PBT) applied on treadmills is an emerging task-specific approach to falls prevention but standardized training protocols are lacking. This review presents an overview of PBT protocols on treadmills, their theoretical justification and proposes recommendations for standardized reporting. METHODS: A systematic search was conducted in PubMed, Embase, Web of Science, CINAHL, CENTRAL and Clinical Trials Registration on May 9, 2025 to identify RCTs, pilo

Calf injuries are common, and they are often described in the gastrocnemius, soleus, or plantaris muscle. They are also documented in the flexor hallucis longus and the tibialis posterior, although less frequently. We present a case of an acute flexor digitorum longus muscle strain, a rare injury, often underreported and not well-documented in the literature. We describe the case of a 34-year-old active female undergoing rehabilitation for an incomplete tetraplegia, with suspected multiple sclerosis and left limb monoparesis. She sustained acute pain in her right leg during eccentric plantar f

Part I of this article discussed the potential functional importance of genetic mutations and alleles of the human cytochrome P450 2D6 (CYP2D6) gene. The impact of CYP2D6 polymorphisms on the clearance of and response to a series of cardiovascular drugs was addressed. Since CYP2D6 plays a major role in the metabolism of a large number of other drugs, Part II of the article highlights the impact of CYP2D6 polymorphisms on the response to other groups of clinically used drugs. Although clinical studies have observed a gene-dose effect for some tricyclic antidepressants, it is difficult to establ

BACKGROUND: Non-healing wounds in older patients can have multiple causes, including infection, inflammatory disease and malignancy. Diagnostic uncertainty can delay appropriate treatment.
CASE PRESENTATION: A man in his seventies presented to the emergency department with a lower leg ulcer. The lesion was initially considered to be secondary to minor trauma with infection and was treated with clindamycin. As the ulceration progressed, biopsies were performed, with pyoderma gangrenosum and squamous cell carcinoma as differential diagnoses. Histopathology showed only ulceration and a mixed inf
