BACKGROUND: Multilevel fractures are breaks of a single bone at multiple levels, often as a combination of diaphyseal and metaphyseal sections. They are usually caused by the effect of strong direct force. Combined injuries of a bone are fractures accompanied by joint distortion. BACKGROUND: Extensive lower extremity tissue defects pose difficulties for surgeons. Considering the natural contour, aesthetics, biomechanics and restoration of motor function, matching and customizing a flexible and large flap is the ideal solution. Restoring circulation early can reduce the risk of infection and flap necrosis. Decapitation resulting from hanging is an infrequent phenomenon, often influenced by multiple biomechanical factors. We report a case of a 72-year-old male who committed suicide by hanging, leading to complete decapitation. The drop height was approximately 10.668 m, with a static nylon rope of 9 mm thickness used as ligature material. Suicides with a high number of sharp force injuries are extremely uncommon, in particular in combination with another suicide method.
