Research priorities in lower leg soft-tissue injuries in older adults: A modified Delphi study (PRIME).

BACKGROUND: Lower leg soft-tissue injuries in older adults are common. These injuries can be complicated by frailty, comorbidities, and impaired healing; however, there is limited high-quality evidence to guide optimal management. This study aimed to establish a consensus-driven research agenda for lower leg soft-tissue injuries in older adults using a modified Delphi approach. METHODS: An international three-round modified Delphi study was conducted, with clinicians involved in the management of lower leg soft-tissue invited to participate. In Round 1, participants submitted open-text respon

Enterococcus thailandicus as an uncommon pathogen in severe lower extremity trauma: a case report.

BACKGROUND: Enterococcus thailandicus is a recently described enterococcal species with uncertain clinical significance. While Enterococcus faecium and Enterococcus faecalis are well recognized as nosocomial pathogens, clinical reports of E. thailandicus remain exceedingly rare. CASE PRESENTATION: We report the case of a 46-year-old Thai male construction worker in Taiwan who sustained a severe forklift-related lower extremity injury in January 2025, including extensive degloving, tibialis anterior tendon rupture, and fibula fracture. Despite multiple surgical debridements and soft tissue reco

Outcome of distal femoral replacement: Influence of trauma vs. elective indications - A prospective single-center study.

BACKGROUND: Distal femoral replacement (DFR) is often the last leg-saving option in advanced femoral bone defects after total knee arthroplasty (TKA) and periprosthetic distal femoral fractures. The aim of this study was to compare the midterm outcome of DFR following periprosthetic fracture or elective TKA revision. METHODS: This prospective cohort study included 93 DFR patients (49 trauma, 44 elective) from 2010 to 2021. DFR for revision in periprosthetic joint infection (PJI) or tumor resections were not included. Data on demographics, surgical history and reason of failure until DFR, revi

Diagnostic inversion at the suicide-homicide boundary: A bidirectional systematic review and forensic indicator crosswalk.

At the suicide-homicide boundary, the same physical finding may be interpreted in opposite directions depending on whether the initial hypothesis is homicide staged as suicide or suicide simulating homicide. This bidirectional systematic review synthesised two independently registered review arms: Arm A, homicides intentionally staged to resemble suicide; and Arm B, suicides that simulated or were initially suspected of homicide. PubMed/MEDLINE, Scopus and Web of Science were searched from inception to 3 April 2026 without language restrictions. Screening, extraction and quality appraisal were

Regional Anaesthesia for Tibial Fracture Surgery in the Third Trimester of Pregnancy.

PRESENTATION: A 27-year-old woman at 33 weeks' gestation presented with severe pain and swelling in her lower leg following a fall from a standing height at home. DIAGNOSIS: Imaging confirmed a severely comminuted tibial fracture. Given pregnancy-related concerns and historical debate regarding peripheral nerve blockade masking acute compartment syndrome (ACS), careful perioperative planning was undertaken with multidisciplinary input. TREATMENT: The patient elected for regional anaesthesia following shared decision-making. Intraoperative anaesthesia was achieved with a single-shot spinal an

The reliability and validity of a visual analog scale for rating the single leg squat test.

BACKGROUND: The single leg squat test (SLST) is commonly used for visual movement assessment in subjects with lower extremity injuries. While current methods utilize ordinal rating categorization, adopting a visual analog scale (VAS) may offer potential advantages. The study aims to determine the intra-rater, inter-rater reliability, and validity of a visual analog scale (VAS) for rating the single leg squat test (SLST) in subjects with lower extremity injuries. METHODS: 58 subjects with lower extremity injuries participated. Intra-rater reliability was assessed by a physical therapist at 1-w

Juvenile patricide by a female: Autopsy case report and literature review.

Parricide refers to the act of murdering one's father, mother, or a close relative without involving harm to children. Juvenile parricide represents a rare form of criminal behavior that, despite its infrequency, receives considerable media attention and provokes substantial public concern. Due to its exceptionally low base rate, forecasting its occurrence remains challenging, if not unattainable. We report a case of a 45-year-old man who was murdered by his juvenile daughter after he disapproved of her relationship with her boyfriend, as she held a grudge and acted in revenge, using a sharp w

The role of quadriceps strength and quadriceps-focused rehabilitation following medial meniscus posterior root repair: a systematic review.

BACKGROUND: Medial meniscus posterior root tear (MMPRT) can lead to biomechanical disruption of the medial compartment and accelerate the progression of osteoarthritis. Although surgical repair may improve pain and functional outcomes, postoperative rehabilitation is also considered important for recovery. The quadriceps plays a central role in knee joint stability, load distribution, and functional recovery, yet its role in rehabilitation following MMPR repair remains unclear. This study aimed to systematically summarize the available evidence regarding quadriceps strength and quadriceps-focu

Suicide by exsanguination from a puncture wound to a haemodialysis arteriovenous fistula: a forensic case report with forensic implications.

Fatal haemorrhage from a haemodialysis vascular access site is an uncommon but well-recognized cause of death in forensic practice, most frequently associated with incised or sharp-force injuries. We report a case of suicide by exsanguination resulting from deliberate needle puncture of an arteriovenous fistula in a haemodialysis patient who was a healthcare professional. A comprehensive forensic investigation, including death scene investigation, medicolegal autopsy, toxicological analyses, and evaluation of the available circumstantial evidence, was performed. A 63-year-old female nurse with

Age estimation of skin incised wounds: a combined biochemical, histopathological, and immunohistochemical approach in a rat model.

Accurate determination of wound age is crucial in forensic investigations, aiding in crime scene analysis and establishing the cause of death. However, this remains a challenging task in medicolegal practice. This study aimed to investigate the temporal dynamics of skin incised wound healing in adult male albino rats by employing biochemical, histopathological, and immunohistochemical analyses. Thirty-six adult male albino rats were utilized. Six served as controls, while the remaining thirty were equally divided into five groups. Standardized incisions were inflicted, and the animals were eut

The Distally Based Saphenous Neurocutaneous Perforator Flap: A Versatile Donor Site for Reconstruction of Complex Soft Tissue Defects of the Lower Leg.

BACKGROUND: Repair of both simple and complex defects in the distal lower leg continues to be a challenging task for reconstructive surgeons because of the local paucity of soft tissue available for transfer. The popular neurocutaneous flap, introduced by Masquelet et al in 1992, has provided a reliable and less technically demanding method for resurfacing defects of the lower leg. We herein present our experience with the versatile design of the distally based saphenous neurocutaneous perforator f lap to provide coverage of complex posttraumatic lower leg defects by harvesting multiple tissue

Viability of automating the landing error scoring system using inertial measurement units.

The Landing Error Scoring System (LESS) is an assessment tool used for identifying movement patterns linked with non-contact anterior cruciate ligament injuries during a double-leg jump-landing; however, the LESS is scored by experts using 2D video recordings, which limits large-scale screening. This study explores the viability of using inertial measurement unit (IMU) data to automate scoring of 17 LESS items, using conventional machine learning model architecture. Forty healthy participants completed six jumps each, and raw movement data from three IMU sensors placed on the sacrum and medial
