Burden and risk factors for plantar fasciopathy in the military population from 2006 to 2015: a retrospective cohort study.

INTRODUCTION: Ankle and foot injuries are highly prevalent in the US Armed Forces, incurring medical and personnel costs for the Department of Defense. The purpose of this study was to evaluate the burden and risk factors of plantar fasciopathy (PF) in the US Military. METHODS: The Defense Medical Epidemiology Database identified all diagnosed PF cases in military service members from 2006 to 2015. A custom spreadsheet calculated incidence of PF and relative risk between sexes, military occupations and ranks. Relative risk of injury per demographic category (sex, age, service branch, rank and

Orthopaedic care pathway for diabetic foot disease : experience from a UK rural district general hospital.

AIMS: The diabetic foot is the major cause of non-traumatic amputations of the lower limb in the UK. There are well-established guidelines and pathways for the management of these patients in tertiary centres, but their management in district general hospitals (DGHs) remains inconsistent. The aim of this study was to describe a dedicated orthopaedic pathway, set up and managed by a multidisciplinary foot team in a DGH. METHODS: Patients are managed on either an emergency, urgent, or elective pathway. We describe how these pathways have worked in our DGH: a total of 104 patients were managed i

A Comparative Analysis of Suture-Button and InternalBrace Fixation for Purely Ligamentous Lisfranc Injuries.

BACKGROUND: Purely ligamentous Lisfranc injuries are challenging to manage. Traditional open reduction and internal fixation provides rigid stability but risks cartilage damage, hardware failure, and the need for removal. Flexible fixation methods such as the suture-button (SB) and InternalBrace (IB) systems aim to maintain reduction while allowing physiological motion. This study compared clinical and radiographic outcomes of SB and IB fixation for these injuries. MATERIALS AND METHODS: A retrospective comparative study included 64 patients (SB, n = 34; IB, n = 30) with acute, purely ligamen

Sinus Tarsi Versus Extensile Lateral Approach for Sanders Type II-IV Calcaneal Fractures: A Comparative Analysis of Functional Outcomes, Return to Work, and Cosmetic Parameters.

Background/Objectives: The optimal surgical approach for displaced intra-articular calcaneal fractures (DIACFs) remains controversial. While the extensile lateral approach (ELA) has traditionally been preferred for complex fractures, the sinus tarsi approach (STA) has gained popularity due to its potentially lower soft tissue morbidity. However, comparative data focusing on patient-centered outcomes remain limited. This study aimed to compare clinical, radiological, functional, cosmetic, and complication outcomes between STA and ELA in Sanders type II-IV DIACFs. Methods: A retrospective compar

Screw characteristics and intramedullary fixation for Jones fractures and associated outcomes: A systematic review and meta-analysis.

OBJECTIVE: A Jones fracture is a common injury in foot and ankle care. Open reduction internal fixation (ORIF) versus conservative treatment has a strong literature background supporting ORIF for the active patients. Standard treatment for fixation of the Jones fracture is intramedullary screw fixation. However, there is limited research on how screw characteristics influence outcomes. This systematic review aimed to compare the outcomes based on screw characteristics in the treatment of Jones fractures. METHODS: A search of the Cochrane, EBSCO, Scopus, and PubMed databases from 2000 to 2023

Evaluation of treatment options for tibialis anterior tendon ruptures.

BACKGROUND: Tibialis anterior tendon (TAT) ruptures are rare injuries with limited literature. While operative intervention may improve function and reduce pain, the optimal timing and choice of surgical technique remain unclear. PURPOSE: To review the management of TAT ruptures across three institutions within the same academic health system and to evaluate surgical management patterns. METHODS: Patients greater than 18 years of age with TAT rupture or pathology treated between 2007 and 2024 by three board-certified foot and ankle surgeons were included. Minimum follow-up was three months.

Concomitant Starfish Procedure and Thumb On-Top-Plasty: Surgical Technique and a Report of 2 Cases.

Two patients, a 65-year-old male and a 61-year-old female, presented with 5-digit partial hand amputations following pressor-induced necrosis and an industrial accident, respectively. Both patients had thumb metacarpophalangeal disarticulations with healthy interossei. These patients underwent combined Starfish procedures and thumb on-top-plasty utilizing the necessarily resected distal index metacarpals. Both patients went on to bony union, functional opposition, and use of their myoelectric prostheses. For patients with 5-digit partial hand amputations, the use of the resected distal 3 cm of

Touch and manual action in chemotherapy-induced peripheral neuropathy: a mixed-methods study.

Chemotherapy induced peripheral neuropathy (CIPN) is a frequent side effect of a number of chemotherapeutic agents which are widely used in the treatment of common cancers. Sensory symptoms primarily affect the fingers and toes and include numbness, tingling and pain. However, there is limited understanding of how these touch impairments may impact manipulation skills. Questionnaire and focus group methods were used to describe the experience of CIPN side effects impacting manual activities in 25 self-selected participants recruited from a cancer charity web site advertisement. Participants’ r

A Displaced and Entrapped Anterior Malleolus (Tillaux Fragment) Causing an Irreducible Syndesmosis and Ankle Fracture-Dislocation in an Adolescent: A Case Report.

INTRODUCTION: Tillaux fractures are Salter-Harris type III injuries of the anterolateral distal tibial epiphysis seen in adolescents. Entrapment of the Tillaux fragment within the syndesmosis is rare and may lead to an irreducible ankle fracture-dislocation. CASE REPORT: A 13-year-old girl sustained an ankle fracture-dislocation following an external rotation injury. Imaging revealed a displaced Tillaux fracture with an associated posterior malleolus fracture. Closed reduction was unsuccessful. Open reduction revealed incarceration of the Tillaux fragment within the syndesmosis, preventing re

Outcomes Following Regenerative Peripheral Nerve Interface Surgery and Associations With Preoperative Mental Health: A Retrospective Analysis of Upper-Extremity Neuromas.

PURPOSE: Preoperative mental health challenges are associated with lower patient-reported outcomes following traumatic hand surgery. However, the association between mental health and recovery following regenerative peripheral nerve interface (RPNI), a surgical treatment option for upper-extremity (UE) neuromas, is not well characterized. This study investigated the outcomes of RPNI for UE neuromas in amputees and nonamputees, and evaluated for associations between preoperative mental health and patient-reported outcomes. We hypothesized that RPNI would improve pain and function, irrespective

The extensor digitorum brevis muscle flap in lower limb reconstruction: A long-term follow-up.

BACKGROUND: The extensor digitorum brevis flap (EDB) is a small local muscle flap used primarily for the reconstruction of small tissue defects in the lower leg. Few studies investigate the long-term outcomes of the EDB flap, and concerns have been raised regarding long-term morbidity. The aim of this study was to investigate the long-term functional outcomes of the EDB flap. METHODS: All patients who had a lower limb reconstruction using a pedicled EDB muscle flap between 1997 and 2021 were identified and asked to participate in the study. Twenty-one patients agreed to participate and were i

Microvascular Fibular Flap for Traumatic Humeral Defect in an Infant: A Case Report and Literature Review.

Microvascular fibular flaps are an established option for reconstruction of large segmental bone defects, but their use in infants following trauma is exceptionally rare. A six-month-old male infant sustained a gunshot wound causing a 5-cm humeral diaphyseal defect. Definitive reconstruction at nine months of age employed a 7-cm osteocutaneous fibular free flap with a 4 × 2 cm skin paddle. The graft was inverted and fixed intramedullary with a K-wire; microvascular anastomoses connected the peroneal artery to a proximally transposed radial artery and flap veins to superficial veins using 10-0

Outcomes of Artery-Only Digital Replantation.

PURPOSE: There is a growing consensus that replanted fingertips can survive with arterial anastomosis alone. This study aimed to evaluate outcomes among individuals undergoing artery-only replantation compared with those undergoing both arterial and venous replantation and to identify factors associated with outcomes. METHODS: This single-center, noninterventional, retrospective cohort study included 177 patients (207 digits) who underwent replantation of Tamai zones I, II, and III at our institution between January 2000 and December 2024. Bilateral Simultaneous Chronic Achilles Tendon Rupture Treated With Open and Endoscopic Flexor Hallucis Longus Tendon Transfer: A Case Report.

Bilateral chronic Achilles tendon rupture is an exceptionally rare condition with limited evidence guiding optimal management. We present a unique within-patient comparison of two reconstructive techniques: endoscopic flexor hallucis longus (FHL) tendon transfer on one side and open FHL transfer on the contralateral side. This design eliminates interpatient variability and allows direct comparison of operative time, surgical exposure, cosmetic outcomes, and functional recovery. Functional outcomes were assessed at 12 months and confirmed at the 24-month follow-up. Both techniques resulted in e

Sensory Survey 3D: an open source utility for the annotation of projected fields for sensory neural interfaces.

BACKGROUND: One goal of neuroprosthetics is to restore sensation to those who have lost it due to disease or injury. This is often accomplished with electrical stimulation of nervous tissue. Common approaches to quantifying evoked sensations for somatosensory prostheses involve asking participants to draw or annotate flattened 2-dimensional (2D) representations of body parts, which can distort the 3-dimensional (3D) projected fields. In particular, patches of skin between the fingers and toes often go unrepresented in 2D, leaving them unquantified. Here, we present a 3D annotation tool that al
