Ultrasound-guided application of percutaneous electrolysis as an adjunct to exercise and manual therapy for tibialis posterior tendinopathy: A randomized clinical trial.

Percutaneous electrolysis has been advocated as a therapeutic strategy for tendinopathies; however, no previous trials have investigated the effects of percutaneous electrolysis on the treatment of tibialis posterior tendinopathy. The aim of this study was to investigate the effects on pain and disability of adding percutaneous electrolysis to a manual therapy and exercise program in individuals with tibialis posterior tendinopathy. A randomized clinical trial (clinical registry: NCT05370092) was conducted. Forty-six (n=46) patients with tibialis posterior tendinopathy were randomized into man

A worldwide perspective on chronic Achilles tendon rupture: An ESSKA AFAS survey initiative.

PURPOSE: Management of chronic Achilles tendon ruptures (CATR) varies according to patient and injury characteristics, but clear guidelines regarding the evaluation and treatment options are still lacking. This study aims to identify tendencies regarding the evaluation and management of CATR among foot and ankle orthopaedic surgeons. The research question is if there is any tendency regarding evaluation, preoperative planning, choice of surgical approach and technique for management of CATR. METHODS: A web-based questionnaire was distributed through 56 national and international foot and ankl

Preoperative depression influences surgical decision-making and worsens outcomes after achilles tendon repair.

BACKGROUND: Ruptures of the Achilles tendon are common, functionally debilitating injuries that can be managed nonoperatively or surgically. Preoperative depression has been associated with worse outcomes in orthopedic procedures, but its impact on surgical decision-making and recovery after Achilles repair is unclear. PURPOSE: This study evaluates the impact of preoperative depression on operative treatment rates and postoperative outcomes following Achilles tendon repair. METHODS: TriNetX database identified adults with Achilles tendon rupture (2005-2024). Patients were stratified by preop

Does transcranial direct current stimulation enhance sensorimotor recovery in chronic ankle instability? A systematic review and meta-analysis.

PURPOSE: Chronic ankle instability (CAI) is related to sensorimotor dysfunction and maladaptive neuroplasticity. Transcranial direct current stimulation (tDCS) is considered a potential method for CAI rehabilitation, but its efficacy and underlying mechanisms remain inconclusive. This study aimed to comprehensively synthesise the current evidence of tDCS for sensorimotor recovery in CAI and evaluate its efficacy, moderating factors and potential mechanisms. METHODS: PubMed, Web of Science, Embase, SPORTDiscus and Scopus were searched in January 2026. The random effects model was employed to me

Walking on the Achilles tendons.

BACKGROUND: The 'walking Achilles tendon' technique can be safely adopted when the initial percutaneous repair fails to achieve sufficient grip in the proximal stump of the ruptured tendon. In such instances, two additional stab wounds are produced proximal to the initial most proximal ones to increase the tendon area available for suture grip. SOURCES OF DATA: A total of 36 patients had who sustained Achilles tendon rupture were included in the present study, divided in two groups: 'the walking on the Achilles tendon group' and 'traditional repair' group. AREAS OF AGREEMENT: There was no di

Comparative outcomes of Kirschner wire and lag screw fixation in pediatric medial malleolar fractures.

BACKGROUND: Salter-Harris type III and IV medial malleolar fractures (McFarland fractures) are uncommon but clinically significant injuries in skeletally immature patients, with a substantial risk of premature physeal closure (PPC) and subsequent growth disturbance. Various fixation methods, including Kirschner wires (K-wires) and cannulated screws, are used in clinical practice; however, comparative data on their outcomes in pediatric medial malleolar fractures are limited. METHODS: This retrospective study included 60 patients (38 males, 22 females; mean age 11.57 ± 1.78 years) with Salter-

Long-term functional outcomes of augmented primary repair using the peroneus brevis tendon for traumatic Achilles tendon ruptures: a retrospective cohort study.

BACKGROUND: Achilles tendon rupture is increasingly common and may result in significant functional impairment. Intraoperative findings such as frayed tendon ends, poor tendon quality, and larger tendon gaps may compromise the reliability of primary end-to-end repair alone. The purpose of this study was to evaluate the mid- to long-term functional outcomes of primary repair with peroneus brevis augmentation in closed traumatic Achilles tendon ruptures with unfavorable intraoperative findings. METHODS: This retrospective cohort study included patients with closed traumatic Achilles tendon ruptu

Surgical outcomes after superior peroneal retinaculum repair without bony procedures for chronic peroneal tendon instability: A consecutive case series.

BACKGROUND: Although bony procedures tailored to retromalleolar groove morphology have been advocated to reduce recurrence in chronic peroneal tendon instability, concerns remain regarding tendon adhesion and procedure-related morbidity. Consequently, it remains unclear whether isolated superior peroneal retinaculum repair alone is sufficient across different fibular morphologies, providing the rationale for the present study. PURPOSE: This study evaluated whether isolated superior peroneal retinaculum repair, without adjunctive bony procedures, can restore stability and function in chronic p

Neurocognitive Deficits Related to Ligamentous Ankle Injuries: A Systematic Review.

BACKGROUND: Lateral ankle sprains (LAS) are among the most common sports injuries, with up to 40% of individuals developing chronic ankle instability (CAI). While altering neurocognitive demands can affect lower limb biomechanics, the underlying mechanisms in CAI patients remain unclear. This systematic review aimed to summarise evidence on the neurocognitive deficits linked with ligamentous ankle injuries and CAI. METHODS: Following PRISMA guidelines (PROSPERO: CRD42023406395), a comprehensive search of five databases (up to September 24, 2024) identified studies examining neurocognitive per

Updated German National S1 Guideline on Pediatric Lower Leg Shaft Fractures.

Fractures of the diaphysis of the lower leg account for approximately 6% of all fractures in children and adolescents. A distinction must be made between isolated tibial shaft fractures, which are primarily managed conservatively, and combined lower leg shaft fractures (involving both the tibia and fibula). The latter are significantly less stable and therefore more frequently treated with osteosynthesis. If the fracture is non-displaced or tolerably displaced, immobilization in a long leg cast for about 4 weeks is sufficient. This requires that no secondary displacement occurs within the cast
