An anatomical way of treating ankle syndesmotic injuries.

Treatment of tibiofibular syndesmotic ankle injury remains controversial in regard to the best method, although surgeons agree that the goal of treatment is reduction and operative stabilization. Ideally, the implant should stabilize the syndesmosis and allow physiologic micromotion and early mobilization, and conventional screws are limited in this regard. We reviewed use of the Ankle TightRope(®) fixation device for repair of syndesmotic injuries. From April to September 2006, 16 patients with evidence of syndesmotic injury were treated by means of ankle fracture open reduction with internal

Chronic exertional compartment syndrome in the medial compartment of the foot.

BACKGROUND: Chronic exertional compartment syndrome of the foot has been previously described. There has been debate over the exact number and location of compartments. The involvement of the medial compartment has been consistently described in case reports; however, there is little in the orthopaedic literature on the exact anatomy for the fasciotomy and the resultant long term outcomes. MATERIALS AND METHODS: We present the results of five patients (seven feet) who were surgically treated by medial compartment decompression, releasing both superficial and deep fascial layers. There were th

Operative management of ankle fractures in patients with diabetes mellitus.

BACKGROUND: Multiple studies have documented increased risks associated with treatment of ankle fractures in patients with diabetes mellitus. We reviewed our results in the largest series to date of this complex patient group to determine the frequency of complications. METHODS: Eighty-four patients with diabetes had open reduction and internal fixation using standard fixation techniques for acute, closed ankle fractures. The 51 men and 33 women had an average age was 49.3 (22 to 77) years. The average followup was 4.1 years (11 to 97 months). Seventy-five fractures were closed and nine were

Blisters associated with lower-extremity fracture: results of a prospective treatment protocol.

OBJECTIVES: To evaluate patient outcomes after treatment of lower-extremity fractures associated with blister formation and to assess complications after soft-tissue treatment using a prospective protocol. DESIGN: Retrospective evaluation of prospectively collected data. SETTING: Level I trauma center. PATIENTS/PARTICIPANTS: Between September 1999 and September 2003, 47 patients who had sustained a closed lower-extremity fracture with early development of fracture blisters in the zone of injury were followed. Blisters were characterized as either avoidable or unavoidable with respect to sur

Review: acute compartment syndrome of the foot.

Foot compartment syndrome is a serious potential complication of foot crush injury, fractures, surgery, and vascular injury. The purpose of this article is to summarize and review the existing literature on this entity. Long-term sequelae of foot compartment syndrome (FCS) include contractures, deformity, weakness, paralysis, and sensory neuropathy. These complications are poorly tolerated, and often necessitate multiple procedures for rehabilitation. Therefore, the threshold for considering compartment syndrome and performing fasciotomy must be low to minimize such outcomes. The existence of

The sural nerve in the foot and ankle: an anatomic study with clinical and surgical implications.

Operative procedures and traumatic injuries of the lateral foot and ankle place the sural nerve and its branches at risk. Anatomic studies quantifying the course of this nerve are sparse. In this study, 17 cadaver specimens were dissected to clarify its course and branching patterns. Despite multiple variations of the nerve and its branches, description of the course of a "typical" nerve trunk was possible. The nerve was found to lie in close proximity to the Achilles tendon at a level of 7 cm above the tip of the lateral malleolus. In the hindfoot, the nerve coursed 14 mm posterior and 14 mm

A composite neuro-teno-cutaneous forearm flap in the one-stage reconstruction of a large defect of the soft tissue around the ankle.

The vascular anatomy of the radial forearm flap, incorporating the brachioradialis and palmaris tendons together with the superficial radial nerve in 20 fresh cadavers, is described. The radial artery in the cadaveric forearm was irrigated and injected with blue latex. The number and distribution of the cutaneous branches supplying the skin, brachioradialis tendon, palmaris tendon, and superficial radial nerve, were then documented in relation to the distance from the radiocarpal joint. The radial artery was found to provide adequate blood supply to the above structures. This flap has been use
