Clinical characteristics of central nervous system talaromycosis: A comparative analysis with cryptococcal meningitis and tuberculous meningitis in patients with AIDS.

BACKGROUND: The incidence of central nervous system (CNS) talaromycosis, an emerging complication in patients with acquired immunodeficiency syndrome (AIDS), is increasing, accompanied by a high mortality rate. Limited understanding of whether its clinical manifestations differ from those of common AIDS-associated CNS infections, such as cryptococcal meningitis (CM) and tuberculous meningitis (TBM), makes diagnosis challenging. Therefore, we aimed to systematically elucidate the key differences in clinical manifestations, diagnostic approaches, and treatment strategies among CNS talaromycosis,

Ring-enhancing demyelinating lesions in the brain and optic neuritis: atypical presentation of paediatric neuropsychiatric SLE.

Paediatric-onset systemic lupus erythematosus (SLE) is a rare but clinically significant condition, often presenting with central nervous system (CNS) involvement. The most common CNS manifestations include headaches, seizures and psychosis. We present two cases of neuropsychiatric SLE with unusual clinical and imaging features. The first case involves an adolescent girl who presented with unilateral vision loss and elevated intracranial pressure. MRI revealed large well-defined ring-enhancing lesions in the bilateral subcortical white matter. She underwent extensive evaluation for infectious,

Brainstem Tuberculous Abscess in a 63-Year-Old Adult With Unrepaired Tetralogy of Fallot.

BACKGROUND: Tetralogy of Fallot (TOF) is a congenital heart anomaly usually corrected surgically in childhood. Reports of adults with unrepaired TOF indicate that they are at high risk of endocardial infections and sequelae such as distant abscesses. CASE PRESENTATION: A 63-year-old man born and raised in Vietnam presented with unrepaired TOF and 5 days of dizziness, dysarthria, and left-sided weakness. Magnetic resonance imaging (MRI) of the brain demonstrated a 2.9-cm rim-enhancing pontine lesion with associated edema and a punctate acute infarct in the right cerebellum. Echocardiogram of t

Concurrent Spinal Dural Arteriovenous Fistula and Varicella-Zoster Virus Meningoencephalitis Unmasked by Corticosteroid-Associated Deterioration: A Case Report on the Diagnostic Value of Serial mNGS.

BACKGROUND: Concurrent spinal dural arteriovenous fistula (SDAVF) and varicella-zoster virus (VZV) meningoencephalitis are exceptionally rare, and overlapping features can delay diagnosis. This case adds to the literature by illustrating how corticosteroid exposure before exclusion of vascular and infectious mimics may be followed by neurological deterioration, and by emphasizing the diagnostic value of serial metagenomic nextgeneration sequencing (mNGS). CASE PRESENTATION: A 48-year-old man developed insidious bilateral lower-limb weakness that progressed to numbness, sphincter dysfunction,

Intracerebral hemorrhage in disseminated histoplasmosis in advanced HIV infection: A case report.

BACKGROUND: Disseminated histoplasmosis is a severe systemic mycosis typically seen in immunocompromised individuals. Though CNS involvement occurs in a small subset of patients, associated intracranial hemorrhage is an exceptionally rare and poorly documented complication. CASE PRESENTATION: A 26-year-old woman with untreated HIV from an endemic region presented with weight loss, jaundice, and lymphadenopathy. Her clinical course progressed to seizures and meningeal signs, initially suggesting tuberculous meningitis. However, neuroimaging revealed a left temporo-occipital hematoma and subara

Cerebral tuberculoma in pregnancy (Jan 1975-May 2025): a systematic review and descriptive analysis of 33 published cases.

BACKGROUND: Cerebral tuberculoma is an infrequent but life-threatening manifestation of tuberculosis whose diagnosis and treatment are uniquely complicated by pregnancy-related physiological changes. Evidence to guide clinicians remains fragmented and largely anecdotal. METHODS: We systematically searched MEDLINE, LILACS and grey literature to May 2025 for reports of cerebral tuberculoma in pregnant or postpartum women. Twenty-seven studies (24 case reports, 2 descriptive studies, 1 case series) from 20 countries met inclusion criteria, yielding individual-level data on 33 patients. Demograph

Co-expression of MARCKS and GSDMD pathway genes in tuberculous meningitis: a multi-omics analysis of blood-brain barrier disruption.

INTRODUCTION: Tuberculous meningitis (TBM), the most severe form of Mycobacterium tuberculosis infection, is characterized by high mortality and neurological sequelae, largely attributed to blood-brain barrier (BBB) disruption. While recent studies identified GSDMD-mediated endothelial pyroptosis as a key mechanism of inflammatory BBB damage, the full molecular landscape in TBM remains unclear. METHODS: This study employed an integrated multi-omics approach, combining bulk and single-cell RNA sequencing of clinical and murine datasets with experimental validation, to identify central mediator

Disseminated Tuberculosis With Cavitary Lung Disease in a Very Preterm Infant.

Tuberculosis (TB) is an important cause of poor health outcomes and mortality for pregnant women and their infants globally. Screening during pregnancy helps diagnose TB at a critical period to avert morbidity and mortality but may be overlooked. TB incidence in the United States is low but has been increasing since 2020, alongside a global resurgence. The Centers for Disease Control and Prevention (CDC) recommends TB screening for all individuals with epidemiologic risk factors, including birth, residence, or prolonged travel in medium- or high-prevalence settings; immunosuppression; and clos

The Clinicoradiological Profile of Ring-enhancing Lesions in the Brain among Children 1 Month-18 Years of Age in a Tertiary Care Hospital.

INTRODUCTION: Ring-enhancing lesions (RELs) in the pediatric brain represent a complex and heterogeneous group of pathological conditions that pose significant diagnostic and therapeutic challenges for clinicians and radiologists. Neuroimaging techniques, particularly magnetic resonance imaging (MRI) with contrast enhancement, play a pivotal role in characterizing these lesions. However, radiological findings alone are often insufficient, necessitating comprehensive clinical correlation and, in some cases, histopathological confirmation. This research aimed to provide a comprehensive analysis

Incidental Papilledema Revealing Neurotuberculosis: A Case Report of a Diagnostic Challenge.

Background: Tuberculosis (TB) remains a major global health problem, particularly in children, who are at higher risk of extrapulmonary and disseminated disease. Ocular tuberculosis is a rare manifestation and often presents with nonspecific signs, leading to delayed diagnosis. Case Presentation: We report the case of a five-year-old previously healthy boy presenting with a two-month history of persistent fever. Initial investigations revealed a positive tuberculin skin test and interferon gamma release assay, with chest CT findings suggestive of miliary TB. Despite negative microbiological te

Triple Central Nervous System Pathogen Infections and Vascular Events in Advanced Human Immunodeficiency Virus-Implications for Early Diagnosis and Multidisciplinary Management: A Case Series from Two Tertiary Centers.

BACKGROUND: Central nervous system (CNS) infections remain a major cause of morbidity and mortality in people living with HIV (PLHIV), particularly in the setting of advanced immunosuppression and recent initiation of antiretroviral therapy (ART). While individual opportunistic CNS infections are well-documented, simultaneous infections with multiple pathogens are rare and diagnostically challenging. METHODS: We report a case series of 11 HIV-positive individuals aged 25-50 years, presenting within 3 months of ART initiation to two tertiary centers over a 2-year period. All patients exhibited

Tuberculous meningoencephalitis concurrent with autoimmune glial fibrillary acidic protein astrocytopathy: a case report.

BACKGROUND: Tuberculous meningoencephalitis and autoimmune glial fibrillary acidic protein (GFAP) astrocytopathy display similar clinical features, cerebrospinal fluid (CSF) profiles, and neuroimaging patterns, which contribute to a high rate of misdiagnosis. Co-existence of these two diseases is extremely rare, and the clinical characteristics of this overlap syndrome remain poorly understood. CASE PRESENTATION: A 67-year-old male patient was admitted with symptoms of fever, cognitive slowing, and limb weakness. Brain magnetic resonance imaging (MRI) showed multiple parenchymal signal abnorm

Miliary Tuberculosis: A Comprehensive Review of Epidemiology, Clinical Manifestations, Diagnosis, Treatment, and Complications.

Miliary tuberculosis (TB) represents a potentially fatal form of disseminated TB resulting from the lymphohematogenous dissemination of Mycobacterium tuberculosis. Although accounting for approximately 1-2% of all TB cases in immunocompetent adults, its mortality rate exceeds 30%. This review screened peer-reviewed, original research studies via four databases (PubMed, EMBASE, MEDLINE, and CENTRAL) to synthesize up-to-date evidence. Key risk factors include human immunodeficiency virus (HIV) co-infection, anti-tumor necrosis factor-α (TNF-α) agents and chronic malnutrition. The clinical presen

Disseminated Nocardia pseudobrasiliensis infection in a case of juvenile systemic lupus erythematosus on immunosuppressive therapy.

Nocardiosis is an uncommon infection that presents as a chronic, debilitating illness with radiographic manifestations simulating lung cancer or tuberculosis. Immunocompromised hosts may develop a fulminant disease resembling acute bacterial pneumonia or disseminated disease. We report a case of a juvenile lupus who developed disseminated Nocardia pseudobrasilienisis infection in form of necrotizing pneumonia, subcutaneous nodules and central nervous system (CNS) lesion in the right parietal lobe, following inadvertent hiking of steroids after a minor organ flare. This case was successfully tr
