BACKGROUND: Tuberculous meningitis is fatal if untreated and can lead to lifelong neurological sequelae. However, to our knowledge, there are no data on the number of children affected by this disease. We aimed to estimate the global disease burden and attributable mortality of childhood tuberculous meningitis by WHO regions, age groups, treatment status, and HIV status in 2019.
METHODS: We developed a Bayesian mathematical model to estimate the number of children aged 0-14 years who developed tuberculous meningitis, died from tuberculous meningitis, and did not die from tuberculous meningiti

Opsoclonus myoclonus ataxia syndrome (OMAS) is a rare neuroinflammatory disorder that is typically associated with paraneoplastic and postinfectious processes. Opsoclonus myoclonus ataxia syndrome has not been previously reported in association with tuberculous meningitis (TBM). This report presents a unique case in which TBM manifested as OMAS, highlighting the complex interplay between tuberculosis and autoimmune neurological conditions. A 1.5-year-old previously healthy girl, presented with acute-onset jerky movements, opsoclonus, irritability, and reduced sleep over 4 weeks. A neurological

A woman in her 30s presented with sudden onset right-sided weakness, speech difficulties and transient loss of consciousness. She had a medical history of migraine, hypothyroidism and cervical lymphadenopathy. On examination, her National Institutes of Health Stroke Scale (NIHSS) score was 8 due to dense right-sided hemiparesis. CT brain imaging showed no intracranial haemorrhage but revealed incidental findings of left supraclavicular and cervical lymphadenopathy. CT intracranial angiogram did not show large vessel occlusion. She received thrombolytic treatment for ischaemic stroke. NIHSS imp

A male patient in his 20s presented with new-onset seizures. Computed tomography head plain was normal. However, in view of persistent occipital headache, magnetic resonance imaging brain with magnetic resonance venogram scan was done which showed left lateral sinus thrombosis. The patient developed fever spikes after admission. Hence, cerebrospinal fluid analysis was sent which was suggestive of a tuberculous etiology. The patient was started on intravenous steroids and antitubercular treatment along with low-molecular-weight heparin. The patient's general condition improved significantly fol

BACKGROUND: The residual risk of mortality or neurological disability is high in tuberculous meningitis (TBM), but there are not many effective treatments for TBM. Malnutrition is a modifiable risk factor for patients with tuberculous; however, the relationship between nutritional risk and neurological prognosis is not clear. In the present study, we aimed to explore the association between malnutrition risk and neurological outcome in patients with TBM.
METHODS: A retrospective cohort study was conducted from December 2010 to January 2021. Malnutrition risks were evaluated by nutritional sca

BACKGROUND: Meningeal tuberculosis (TBM) is the most severe form of extrapulmonary tuberculosis due to its high mortality and long-term sequelae in survivors.
METHODS: A cross-sectional study of diagnostic tests was carried out in a private clinical laboratory in Lima, Peru. All cerebrospinal fluid (CSF) samples from patients with suspected meningitis were analyzed with cytochemical and biochemical studies, as well as smear microscopy, India ink, the FilmArray Meningitis/Encephalitis panel, Xpert® MTB/RIF or Xpert MTB/RIF Ultra, and culture for common bacterias, fungi or mycobacterial.

Tuberculous meningitis remains the most lethal form of tuberculosis. Despite significant physiological differences adults and children with tuberculous meningitis receive similar treatment and are often grouped together in research. Consequently, differences in tuberculous meningitis characteristics across the lifespan are poorly understood but may be relevant to developing more effective and age-appropriate interventions. In this review we discuss potential age-specific considerations in pathogenesis and pathophysiology, and review literature over the last 5 years to describe clinical charact

To develop and validate a nomogram model for differentiating cryptococcal meningitis (CM) from tuberculous meningitis (TBM) in HIV-infected patients, given the diagnostic challenges due to shared clinical manifestations and limitations of existing methods. A retrospective analysis extracted 207 HIV cases (112 CM, 95 TBM). Candidate predictor variables covering general information, blood biochemical, and cerebrospinal fluid(CSF) examination indicators were collected. Least absolute shrinkage and selection operator (LASSO) regression and ten-fold cross-validation identified key predictors, which

Studies describing the global burden of meningitis often exclude HIV- or tuberculosis (TB)-related etiologies, thereby presenting a limited view of meningitis etiology in low- and middle-income countries. This study provides an updated evaluation of the etiology of meningitis and treatment outcomes in Uganda given advancements in molecular and TB diagnostics. We conducted a prospective observational cohort study from December 2018 to October 2023, for which adults with suspected meningitis were recruited from three referral hospitals in Uganda. We used a comprehensive diagnostic algorithm to d

BACKGROUND: Several ongoing clinical trials are evaluating high-dose rifampicin (up to 35 mg/kg) for tuberculous meningitis (TBM). However, rifampicin pharmacokinetics at higher doses is not fully characterized, particularly in cerebrospinal fluid (CSF), the site of TBM disease.
METHODS: In a randomized controlled trial, adults with HIV-associated TBM were assigned to experimental arms of high-dose rifampicin (oral, 35 mg/kg; intravenous, 20 mg/kg) plus linezolid, with or without aspirin, or a control arm that received the standard of care with 10 mg/kg of oral rifampicin. BACKGROUND: Nocardial brain abscesses are extremely rare and predominantly affect immunocompromised patients, exhibiting a high overall mortality rate. Tuberculosis infections, although they can occur in immunocompetent individuals, are more prevalent in those with compromised immune systems. Tuberculous meningitis (TBM), the most severe manifestation of tuberculosis, is associated with a high fatality rate. Co-infection with both pathogens is unusual. To our knowledge, this is the first reported case of multiple brain abscesses caused by Nocardia abscessus (N. abscessus) in a young immunocomp

We enrolled 135 adults with tuberculous meningitis (TBM), including 83% with HIV. Participants with baseline C-reactive protein (CRP) ≥40 mg/L had 3 times higher odds of an 8-week modified Rankin scale ≥4 (adjusted odds ratio, 2.78; 95% CI: 1.28-6.04; P = .010). CRP is a viable prognostic biomarker in TBM.

BACKGROUND: Herpes simplex encephalitis is the most common form of encephalitis, while tuberculous meningitis is a rare condition accounting for less than 1% of all tuberculosis cases. Since the symptoms are similar in both diseases, differentiating between them is a clinical challenge, even after performing cerebrospinal fluid analysis, as the final diagnosis remains based on the specific investigations that were unusually positive for both diseases in our patient, which have not been reported previously, making this case the first in medical literature on this subject.

Tuberculous meningitis (TBM) is a critical form of tuberculosis with high morbidity and mortality. Paradoxical reactions (PR) are frequently observed in neurotuberculosis, with diverse manifestations. A selective immune dys/upregulation leads to cytokine surge. The exact immune pathogenesis is not known. This review explores the established literature-based knowledge in PR in HIV-ve TBM to visualize the gray zone in neurotuberculosis and possible immune therapeutic adventures. We undertook this systematic review as per the preferred reporting items for systematic reviews and meta-analyses guid

PURPOSE: Tuberculous meningitis (TBM), a severe and often fatal form of tuberculosis, showing high mortality and long-term neurological sequelae. Recent evidence suggests that microRNAs play a crucial role in TBM pathogenesis and may serve as potential biomarkers for diagnosis and disease progression.
METHODS: Eight TBM patients and three healthy controls were recruited. Whole blood samples RNA was extracted and processed. Sequencing was performed on the Illumina platform with a 50-bp single-end read configuration. Data preprocessing included quality control (FastQC), adapter trimming (Cutada

Tuberculosis (TB) is the leading cause of death from a single infectious agent, with approximately 1.2 million deaths reported in 2023. While TB primarily affects the lungs, it can also spread to other organs, where it is classified as extrapulmonary TB. Tuberculous meningitis (TBM) is the most severe form of extrapulmonary TB, affecting 1–5% of TB cases. Delayed diagnosis contributes to its high mortality and severe neurological complications, with approximately 10% of affected individuals dying or suffering permanent neurological damage. When combined with adjunctive therapy, early detection

Tuberculosis (TB) is one of the top 10 causes of death and the leading cause of morbidity from a single infectious agent. Tuberculous meningitis (TBM) is a subtype of tuberculosis which constitutes about 5% of all extrapulmonary tuberculosis and about 1-2% of active tuberculosis. This study was conducted to analyse the annual incidence of TBM in Malaysia from 2015-2020. Through this study we were also able to study the demographic characteristics and clinical profiles of TBM patients. It was a cross sectional study using data collected from Malaysia national case-based TB registry (MyTB) betwe
