Clinical characteristics and factors associated with tuberculous meningitis outcomes in Thailand: A 13-year retrospective cohort study.

BACKGROUND: Tuberculous meningitis (TBM) remains the most severe manifestation of tuberculosis. Additional work is needed to characterize clinical features and outcomes of TBM in low- and middle-income countries (LMIC). METHODS: We retrospectively identified adults with suspected TBM at King Chulalongkorn Memorial Hospital (KCMH), Bangkok, Thailand, from 2012 to 2025. TBM was categorized as definite, probable, and possible, according to the uniform case definition. Modified Rankin Scale (mRS) was assessed at discharge and 1 year. Multivariable logistic regression was used to evaluate factors

Deciphering Tuberculous Meningitis: From Clinical Challenges to Novel Models and Pathogenic Pathways.

During and after the COVID-19 pandemic, Tuberculosis (TB) has reestablished with higher figures due to interruptions in the Directly Observed Treatment Short course (DOTS) despite underreporting. The rising consequences would have extended to extra-pulmonary forms of TB as well, including Tuberculous Meningitis (TBM). Considering the fact that TBM is the most dangerous and worst form of TB, we found the need to scan the literature to highlight various aspects of TBM. Epidemiology of TBM is proportionally less frightening, but the consequent mortalities and morbidities are more alarming than pu

Additive Effects of Glutathione in Improving Antibiotic Efficacy in HIV-M.tb Co-Infection in the Central Nervous System: A Systematic Review.

BACKGROUND: HIV and tuberculosis (TB) co-infection poses a significant health challenge, particularly when involving the central nervous system (CNS), where it leads to severe morbidity and mortality. Current treatments face challenges such as drug resistance, immune reconstitution inflammatory syndrome (IRIS), and persistent inflammation. Glutathione (GSH) has the therapeutic potential to enhance treatment outcomes by improving antibiotic efficacy, reducing inflammation, and mitigating immune dysfunction. METHODS: Relevant studies were identified through systematic searches of PubMed, Elsevi

From pain to meningitis: bacteria hijack nociceptors to promote meningitis.

Bacterial meningitis is a severe and life-threatening infection of the central nervous system (CNS), primarily caused by Streptococcus pneumoniae and Neisseria meningitidis. This condition carries a high risk of mortality and severe neurological sequelae, such as cognitive impairment and epilepsy. Pain, a central feature of meningitis, results from the activation of nociceptor sensory neurons by inflammatory mediators or bacterial toxins. These nociceptors, abundantly present in the meninges, trigger neuroimmune signaling pathways that influence the host immune response. However, the mechanism

Tuberculosis: An Update for the Clinician.

Tuberculosis (TB) remains a significant global health threat with high mortality and efforts to meet WHO End TB Strategy milestones are off-track. It has become clear that TB is not a dichotomous infection with latent and active forms but presents along a disease spectrum. Subclinical TB plays a larger role in transmission than previously thought. Aerosol studies have shown that undiagnosed TB patients, even with paucibacillary disease, can be highly infectious and significantly contribute to TB spread. Encouraging clinical results have been seen with the M72/AS01E vaccine. If preliminary resu

Safety and Tolerability of a Short Course of Linezolid for the Treatment of Predominantly Moderate to Severe Tuberculous Meningitis in Adults With Human Immunodeficiency Virus.

BACKGROUND: Tuberculous meningitis (TBM)-related deaths occur early, often within weeks after treatment initiation. Enhanced treatment early in the disease course with agents that effectively penetrate the central nervous system may improve outcomes in TBM. METHODS: We conducted a phase 2, open-label, randomized trial in Masaka, Uganda, to assess the safety and tolerability of linezolid 1200 mg once daily versus no linezolid with high-dose (35 mg/kg/d) or standard-dose (10 mg/kg/d) rifampin for 4 weeks in participants with definite or suspected TBM. The primary endpoint was any grade ≥3 adver

A rare and challenging pediatric case of drug toxicity and immune reconstitution inflammatory syndrome during the treatment of intracranial tuberculoma: A case report.

Intracranial tuberculoma represents one of the most severe complications of central nervous system tuberculosis (TB), with an incidence that is relatively low. In cases of intracranial tuberculoma, patients may develop drug toxicity and/or immune reconstitution inflammatory syndrome (IRIS) while receiving anti-TB treatment. The current study presented the case of a seven-year-old female patient with intracranial tuberculoma who developed drug-induced hepatotoxicity and IRIS during the course of treatment. During the follow-up of the patient, anti-TB drug-induced hepatitis developed, which led

Prolonged Corticosteroid Use in the Treatment of Tuberculous Meningoencephalitis: A Case Report.

Tuberculous meningoencephalitis is a rare manifestation of Mycobacterium tuberculosis (Mtb), with the most severe form and highest mortality. It can cause multiple complications, and treatment is difficult, as drugs cannot properly diffuse through the haemato-encephalitic barrier. We reported the case of a 17-year-old female patient who was admitted to the emergency room department with a fever for previous two weeks (up to 39 °C), dizziness, difficulty walking, and weight loss. Magnetic resonance imaging indicated possible meningoencephalitis, and a CT scan of the lungs visualised miliary inf

From tumor suspicion to tuberculoma discovery: A pregnancy case report and literature review.

This article presents the case of a 22-year-old woman at 29 weeks of pregnancy, who exhibited recent onset neurologic symptoms including paresthesia of the right side of the face, involuntary myoclonic movements in the ipsilateral hand, 1 week of weakness in the right side of the body, and imaging features that were highly suggestive of a brain tumor diagnosis. A challenging decision making process was conducted by a team of multidisciplinary experts, leading to a consensus on adopting a conservative approach involving the use of steroids and antiepileptic medications, to which the patient res

Tuberculous optochiasmatic arachnoiditis.

A 24-year-old woman developed multiple cerebral tuberculomas during treatment for central nervous system tuberculosis, requiring a protracted course of antituberculous therapy. Four years later, while on a second course of antituberculous therapy for relapsed disease, she developed deteriorating vision and further cerebral tuberculomas with inflammation; this was attributed to optochiasmatic arachnoiditis. She was treated at different stage with intravenous corticosteroids, second-line antituberculous therapy, infliximab and thalidomide. Her visual deficits initially improved but subsequently

Cerebral salt wasting syndrome in a patient who suffered a gunshot traumatic head injury: a case report.

Cerebral salt wasting syndrome is described as a hyponatremic condition in the context of a central nervous system injury. It is caused by a renal loss of sodium, and the primary distinguishing factor among similar conditions, such as the syndrome of inappropriate antidiuretic hormone secretion, is a reduction in the extracellular fluid volume. We describe here the case of a young adult who suffered brain injuries as a result of a gunshot and developed cerebral salt wasting syndrome during his in-hospital stay. Any type of cerebral attack can lead to hyponatraemic syndrome, but the most common
