INTRODUCTION: Open traumatic Achilles tendon rupture is uncommon, particularly in the pediatric population. Unlike closed spontaneous ruptures - which predominantly affect middle-aged adults - sharp lacerating injuries in children pose challenges due to contamination risk and the close relation of neurovascular structures in the posterior ankle. We report a case of complete open Achilles tendon rupture in a 9-year-old boy following a sharp metallic object injury, treated with primary tendon repair using the Modified Kessler technique. This case adds to the limited pediatric literature and may g

BACKGROUND: Modified Broström repair is the gold standard for chronic lateral ankle instability. Internal brace augmentation was introduced to increase ligament strength and allow earlier rehabilitation, but concerns exist regarding surgical site infection (SSI).
PURPOSE: To compare the incidence of SSI following anterior talofibular ligament (ATFL) repair with internal brace augmentation and immediate permissive weight bearing versus ATFL repair without augmentation and restricted rehabilitation.
STUDY DESIGN: Retrospective cohort study.
METHODS: Patients undergoing open ATFL repair with i

BACKGROUND: Osteoporosis may impair fracture healing and fixation durability after ankle ORIF. This study evaluated postoperative complications of preoperative osteoporosis.
METHODS: Patients undergoing ankle ORIF from 2004 to 2022 with ≥ 2 years of follow-up were identified from a multi-institutional database. Those with and without preoperative osteoporosis, defined with ICD-10 code M80 & M81, were compared using 1:1 propensity score matching; outcomes were assessed at 90 days and 2 years.
RESULTS: Matched cohorts included 4190 patients. At 90 days, the osteoporosis group had higher device

INTRODUCTION: Posteromedial talar process (Cedell) fractures are rare and frequently missed, particularly in polytrauma patients. Bilateral involvement is exceedingly uncommon and presents unique diagnostic and management challenges.
CASE REPORT: A 20-year-old male sustained bilateral posteromedial talar process fractures following a fall from height. Associated injuries included an L3 bony Chance fracture, a right ankle fracture-dislocation with a medial malleolar fracture, and a left distal tibial plafond fracture. Computed tomography (CT) confirmed bilateral posteromedial talar process fra

The simultaneous occurrence of an open bimalleolar ankle fracture with ipsilateral calcaneal fracture and Lisfranc fracture-dislocation within a single extremity represents an extraordinarily rare constellation of injuries resulting from high-energy trauma. When associated with a Gustilo-Anderson Type IIIB soft-tissue defect, traditional open reduction and internal fixation (ORIF) carries an increased risk of deep hardware infection, wound breakdown, and osteomyelitis. The extensive soft-tissue injury further complicates management and may preclude immediate internal fixation. This report demo

INTRODUCTION: Extra-articular distal tibial fractures are difficult to manage because of poor soft-tissue coverage, limited vascularity, and the tendency for malalignment and delayed union. Closed intramedullary interlocking nailing has become an increasingly preferred treatment option because it preserves fracture hematoma and periosteal blood supply.
AIMS: The aim of the study was to evaluate the clinical and functional outcomes of closed intramedullary interlocking nailing in extra-articular distal tibial fractures.
MATERIALS AND METHODS: This prospective observational, single-center stud

Chronic exertional compartment syndrome (CECS) is an exercise-related cause of lower extremity pain characterized by reproducible pain, tightness, cramping, and neurologic symptoms that improve with rest. Diagnosis may be delayed because chronic lower leg pain has a broad differential diagnosis, and currently used diagnostic strategies remain imperfect. We report the case of a young woman who began experiencing bilateral lower extremity pain during high school sports that progressed to pain with prolonged standing. Treadmill-provoked bilateral four-compartment pressure testing reproduced sever

To determine whether heel-free compression bandaging is non-inferior to heel-wrapped bandaging for maintaining interface pressure in adults with venous leg ulcers. Randomised, controlled, parallel-group, non-inferiority trial. Adults with open venous leg ulcers (CEAP C6) treated at a French university hospital were randomised to heel-free or heel-wrapped (control) bandaging for 4-6 weeks. The primary outcome was interface pressure 48 h after the final bandage application, analysed in the per-protocol population. Secondary outcomes were ankle oedema, change in interface pressure and patient pre

INTRODUCTION: A talar cyst is a fluid-filled sac in the talus bone, often caused by chronic stress fractures or traumatic injuries. A talar cyst if left untreated can result in chondromalacia of talus cartilage and ultimately arthritis. A timely intervention of cyst can have very good prognosis. An open cyst decompression and bone grafting is a standard procedure followed, but it is more invasive and sometimes involves medial malleolus osteotomy. If the same is done by arthroscopy, the morbidity of open procedure can be avoided.

INTRODUCTION: Insertional Achilles tendinopathy (IAT) is common in running athletes and, when refractory to conservative treatment, may require surgery. Open debridement and reinsertion are associated with wound-healing complications and prolonged recovery, which are particularly problematic in sprinters, whose performance depends heavily on the gastrocnemius-soleus-Achilles complex. Endoscopic double-row (SpeedBridge™) Achilles reinsertion may reduce soft-tissue morbidity, while concomitant gastrocnemius tightness is increasingly recognized in the pathogenesis of IAT. INTRODUCTION: Bosworth fracture-dislocations are rare ankle injuries characterized by posterior entrapment of the fibula behind the tibia, making closed reduction inherently difficult. Their subtle radiographic appearance often delays diagnosis. This case highlights the diagnostic challenges and operative considerations associated with this uncommon and frequently irreducible injury pattern.
CASE REPORT: A 34-year-old man presented after a fall with a posterior malleolus fracture, distal fibular fracture, and tibiotalar dislocation. Closed reduction improved alignment, but persistent mortise

PURPOSE: This study compares the syndesmosis congruity and functional outcomes after syndesmotic reconstruction by open reduction and internal fixation (ORIF) of posterior malleolar fractures (PMFs) sized > 5% versus no fixation in type C ankle fractures.
METHODS: This multicenter prospective cohort study compared ORIF of PMFs (FIX) with no fixation (NO-FIX). Both groups underwent fibular and medial malleolus fixation according to AO principles, with trans-syndesmotic stabilization screws in case of instability. The primary outcome was syndesmotic alignment assessed on post-operative bilater

OBJECTIVE: The aetiology of hard-to-heal (chronic) wounds is usually categorised as either 'diabetic foot', 'venous leg ulcer (VLU)', 'arterial', 'pressure injury', 'postoperative' and 'other'. Due to the homogeneity of patients with a diabetic foot ulcer, the majority of studies regarding the efficacy of placental allografts have been in this population of patients, with a small fraction conducted with patients with VLUs. However, insufficient research has been performed regarding advanced modalities for the treatment of other, open hard-to-heal wounds resulting from alternative aetiologies,

AIM AND BACKGROUND: Pilon fractures are rare but severe injuries associated with high-energy trauma, often resulting in significant articular and soft tissue damage. Their management is challenging due to complications such as infection, nonunion, and post-traumatic arthritis, which often require complex staged surgical interventions. Drug-induced hypersensitivity syndrome (DIHS), a life-threatening reaction characterised by rash and systemic involvement, presents additional challenges in orthopaedic contexts due to its association with high-dose corticosteroid therapy and increased infection

BACKGROUND: Sural nerve injury is an uncommon complication following foot and ankle surgery, including procedures performed for Haglund deformity. Postoperative sural nerve entrapment remains rarely reported and may be challenging to diagnose.
CASE PRESENTATION: A 56-year-old woman presented with persistent lateral ankle pain and paraesthesia 12 months after open lateral heel surgery for Haglund deformity. Clinical examination revealed a trigger point at the distal portion of the surgical scar associated with a positive Tinel's sign. A diagnostic local anaesthetic injection provided temporary

INTRODUCTION: Ankle sprain is one of the most common sports injuries. Approximately 40% of patients with anterior talofibular ligament (ATFL) rupture develop chronic ankle instability, characterised by persistent balance dysfunction. Although virtual reality-based balance training has shown beneficial effects in non-surgical populations with chronic ankle instability, its efficacy in patients after ATFL repair remains unclear. This study aims to evaluate the effects of virtual reality-based balance training on motor function recovery in patients after ATFL repair.

Necrotizing soft tissue infections (NSTIs) are rapidly progressive, life- and limb-threatening emergencies requiring immediate surgical source control. Group B Streptococcus (GBS; Streptococcus agalactiae) is a rare cause of NSTIs in adults. We present a 44-year-old woman with poorly controlled diabetes who developed progressive right foot pain and swelling after minor ankle trauma. Examination showed spreading erythema, crepitus, dusky discoloration, malodor, and a deep plantar wound, accompanied by marked leukocytosis and diabetic ketoacidosis. Computed tomography demonstrated infection exte

INTRODUCTION: Open lower limb fractures require multidisciplinary orthoplastic management. With an ageing population, older patients present with distinct injury patterns, greater comorbidity and reduced physiological reserve. In this study, we compared presentation, management and outcomes between younger and older patients.
METHODS: A 4-year retrospective complete case cohort study (2018-2022) was conducted at a tertiary major trauma centre. Adult patients with open tibia, fibula or ankle fractures were included. Patients were stratified into younger adult (<65 years) and older adult (≥65 y

Chronic ankle instability (CAI) is a common sports-related musculoskeletal disorder characterized by recurrent sprains and neuromuscular control deficits. It affects a wide range of individuals, from recreational to elite athletes, and poses a substantial healthcare burden. This study proposes an AI-enabled digital twin framework for sports health applications, offering both interpretability and clinical deployability. The framework identifies CAI and enables subtype stratification using a wearable electromyography (EMG) sensor-driven hierarchical multi-domain fusion model, generates fine-grai
