We studied a series of seventeen cases of subtalar dislocation of the foot. There were on anterior, twelve medial, and four lateral dislocations. Of the seventeen dislocations, fourteen were closed and three, open. Two of the four lateral dislocations were open. Associated fractures involving the talocalcaneal or talonavicular joints were noted in eight feet, including three of the four with a lateral dislocation. Ankle injuries are common in adolescence, seldom initially seen by orthopaedists, and often treated primarily by emergency department personnel. It is often not appreciated that the distal tibial growth plate fuses in a mediolateral direction, leaving it open and vulnerable to growth plate injuries for a longer period during adolescence. Fractures of the lateral plafond of the tibia are often not appreciated due to overlap of the fibula. A review of the injuries at the Winnipeg Children's Hospital has revealed these are not as uncommon as often supposed. A 3 year experience yielded 26 patients

Presented is a case report of a tibialis posterior tendon rupture associated with a closed ankle fracture. The treatment is open reduction and fixation at the bony fragments with primary repair of the tendon. This injury, although rare, illustrates that soft-tissue injuries must be considered in the management of unusual or complex fractures.

The results of 31 fractures of the tibial shaft treated with closed Ender nailing and early weight-bearing are reported. Twenty-five of these fractures were closed and six open. All fractures united between 6 and 16 weeks after operation. No case of deep wound infection was encountered. Comminuted fractures of the proximal and distal third were prone to angular deformities and needed cast protection post-operatively. Nail reinsertion became necessary in two patients. Severe plantarflexion of the foot associated with adduction and supination resulted in medial subtalar dislocation in three patients. Closed manipulation in two patients and open reduction in one restored normal anatomy of the foot, with no complications in a follow-up of 1 to 2 years.

Because of the anatomical and functional peculiarities of the astragallus surgical and conservative treatment of central fractures are often unsatisfactory. A follow-up study of 40 patients with 42 central talus fractures were performed over an average of 9 years after the initial trauma. The number of poor results was extremely high; only primary surgical or conservative treatment showed good or fair results. Best results were obtained by primary surgical treatment whereas fractures operated at later stages showed worse results. Therefore fresh central astragallus fractures should be operated

Massive trauma involving the foot was treated by external fixation in 33 cases and followed for 1 to 3 years. Amputations were performed in seven (21%) because of uncontrollable deep sepsis in two and massive injury in five. The other 26 (79%) had functionally acceptable feet at follow-up, but all had some stiffness and cosmetic deformity. Eleven medial subtalar dislocations, two of them open, were treated during the decade from 1969 to 1978 in four metropolitan Detroit hospitals. There was one open lateral dislocation. Twenty=eight consecutive patients with displaced malleolar fractures were treated with anatomically exact open reduction, rigid internal fixation and immediate postoperative exercises, following the principles of the Swiss ASIF group. There were no operative or postoperative complications. One patient had a minor abscess over a loosened screw, while 1 patient developed pes equinus that required treatment. AT follow-up 2 to 12 months after the operation, 25 patients were walking normally. The movements of the ankle joint were excellent or good in 25 patients. The method is recommended.

This paper discusses some of the problems encountered in the diagnosis and treatment of 371 ankle fractures treated at the University of Kansas Medical Center over a 14-year period, 1965-1979. Seven of these injuries required major reconstructive procedures after failure of initial treatment and 11 fractures required eventual ankle arthrodesis. The causes of failures of the initial treatment of these 18 cases are discussed.
