Calcaneal fractures at Grady Memorial Hospital were reviewed for the period of 1973 to 1980. Sixty-two of these fractures were available for follow-up. Treatment was with compression dressings, plaster, pin reduction or open reduction. Grading was by the previously published criteria of Allen and Lindsey. This paper identifies a positive correlation between facet reduction ad successful treatment results. Recommendations are made for fracture classification, surgical treatment and postoperative management.

For over 100 years, many different types of external fixation have been used to immobilize fracture fragments. The greatest acceptance of external fixation has been with open fractures of the tibia. Three ankle fusions, all complex cases, were performed using the double-framed Hoffmann external fixation device. The deformities treated were a severe post-traumatic equinus deformity of the ankle and forefoot, a painful nonunion of a previous ankle fusion, and an equinus deformity secondary to a transmetatarsal amputation.

Twenty subtalar dislocations were reviewed in patients seen at the hospital complex of the University of Pennsylvania, 1950--1979. All patients were available for followup examination and roentgenographic review. These were 15 males and five females in the study. Utilization of a free omental transfer with a skin graft in two lower limbs for soft tissue and skin cover in difficult situations is reported. In 1 patient, it was used to cover open and damaged joints around the ankle, and in the other, for the reconstruction of a circumferential defect in a lower limb. The results were quite satisfactory.

Internal fixation of open fractures can be carried out according to the same indications employed in closed fractures in the presence of a type I wound. In type II and type III wounds early internal fixation may be indicated in victims of multiple trauma, in the elderly, in intra-articular fractures, in mutilated limbs, and in some fractures with associated vascular injuries. In these cases the higher risk of infection should be justified by the salvage of limb, life, or joint function. A muscle transposition technique for reconstruction of soft tissue loss and skin defects with exposed bone in the leg is described. Reconstruction with muscle flaps and freee skin grafts is a reliable and convenient one-stage operation. The gastrocnemius and soleus muscles were used to cover the upper and middle third of the leg. The method was used in 13 patients, and there were 3 failures in the series because of the impaired vascularity of the injuried leg. A relatively great number of supination abduction fractures did not need surgical treatment because displacement was not present. Significantly better results occurred with operative intervention in supination--eversion and pronation--eversion ankle fractures. Supination--abduction and supination--eversion ankle fractures are best treated by open reduction, anatomical restoration and stabilization of the fractures.

Two cases of tri-malleolar frautures of the ankle caused by feet becoming entangled in loose seat belts are reported. Open reduction and internal fixation of all three malleolar fragments was carried out in each case with a satisfactory end result.

"Satisfactory reduction" is insufficient in discussing ankle fractures; only perfect anatomic reduction will suffice. Two basic fracture types are found at the level of the syndesmosis, supination-external rotation (SE) and pronation-abduction (PA), reflecting the mechanism of injury. Sixty-seven patients with 86 fractures of the calcaneum were re-examined 2 to 6 years after their injury. These fractures of the calcaneum were of two types: thalamic, involving the posterior subtalar joint (85%); and nonthalamic, confined to the extra-articular area (15%). The reconstructive surgeon experienced in the use of rotated or free muscle and myocutaneous flaps, skilled in microvascular surgery, and interested in lower extremity trauma, may favorably alter the course of management of these historically difficult problems.
