Factors associated with surgical site infection after ankle fracture fixation: a retrospective study in northern Sweden.

BACKGROUND AND PURPOSE: Ankle fractures are common, and open reduction and internal fixation (ORIF) is standard treatment for unstable injuries. Surgical site infections (SSI) remain a significant clinical challenge. We aimed to estimate incidence and to identify patient- and treatment-related factors associated with SSI after ankle ORIF. METHODS: In this retrospective cohort, we included all adults (≥ 18 years) who underwent ankle ORIF at 2 trauma centers in northern Sweden from 2015 to 2024. Data from medical records covered demographics, comorbidities, injury characteristics, surgical deta

Retrospective comparison of minimal and extensive fixation strategies in operatively treated ankle fractures.

BACKGROUND: Ankle fractures are among the most common orthopedic injuries, with open reduction and internal fixation (ORIF) being the standard treatment for displaced fractures. This retrospective study compared operative time, radiographic and complication outcomes between two fixation strategy categories, minimal fixation and extensive fixation, in operatively treated ankle fractures. METHODS: A retrospective review included 95 patients treated surgically for displaced Danis Weber B or C ankle fractures between January 2017 and November 2019. Patients were grouped by fixation strategy at in

Measured Flap Planning for Adult Complex Syndactyly Release: A Technical Case Report.

INTRODUCTION: Adult untreated complex syndactyly is a rare clinical presentation and may pose a unique set of reconstructive challenges due to long-standing tissue adaptation. Various flap designs for syndactyly release are described, but pre-operative planning of the flaps is often based on visual estimation and surgeon experience. We report a systematic measurement-based flap planning technique (MFPT) in the reconstruction of complex syndactyly in an adult patient. CASE REPORT: A 23-year-old male presented with an untreated complex congenital syndactyly of the middle and ring fingers with f

[Strategies for the definitive management of pilon fractures].

Pilon fractures are among the most challenging injuries of the lower extremities and are associated with a high complication rate due to the frequently severe soft tissue damage. The currently established 2‑stage treatment concept with initial stabilization using a joint-bridging external fixator, subsequent soft tissue conditioning and thorough preoperative planning represents the current gold standard of care. Computed tomography with multiplanar and three-dimensional reconstruction is indispensable for detailed analysis of the fracture morphology and the development of an individually adapt

[Acute management of pilon fractures].

Pilon fractures are complex injuries involving the weight-bearing articular surface of the distal tibia and are frequently associated with relevant soft tissue damage. The treatment requires a structured strategy in which the condition of the soft tissues plays a central role. These fractures result either from low-energy rotational injuries or high-energy axial compression trauma. While the former typically occur in older patients and are associated with limited soft tissue injury, the latter often lead to complex fracture patterns with severe articular impaction and metaphyseal comminution.

Isolated Displaced Chaput Fracture (Rammelt Type III) With Significant Intra-articular Displacement: A Case Report.

The distal tibiofibular syndesmosis, which is essential for preserving ankle stability, is often involved in ankle injuries. Avulsion fractures of the anterior inferior tibiofibular ligament, also referred to as Chaput fractures, if left untreated, may go undetected on plain radiographs and result in chronic instability. For precise evaluation and treatment planning, advanced imaging techniques such as computed tomography (CT) are frequently necessary. We present the case of a 23-year-old man who fell from a height of 2 m and suffered a twisting injury to his right ankle and was seen at King H

Arthroscopic management of ankle ligament injuries: a new gold standard?

Ankle ligament injuries are among the most common lesions encountered in emergency medicine and in chronic contexts. The present study aimed to assess the role of arthroscopy, addressing 5 questions. (1) What are the indications for surgery in ankle ligament injury? - In the acute phase, conservative treatment is the rule, but surgery may be considered for lateral lesions, in first injury in athletes with high functional demand, and in case of associated proximal lesions in the anterior talofibular ligament and calcaneofibular ligament. Surgery is also indicated for unstable lesions in the dis

Donor Site Morbidity After Talar Mosaicplasty: Arthroscopic Medial Femoral Condyle vs Mini-Open Lateral Femoral Condyle Harvesting.

BACKGROUND: Osteochondral lesions of the talus (OLTs) are challenging injuries that may require osteochondral autograft transfer for large, cystic, or refractory defects. Although mosaicplasty can restore hyaline cartilage and subchondral bone, donor site morbidity remains a concern. This study compared knee donor site morbidity and ankle functional outcomes after 2 technique-donor site combinations used for talar mosaicplasty: arthroscopic medial femoral condyle harvesting and mini-open lateral femoral condyle harvesting. METHODS: In this retrospective comparative cohort study, 40 patients w

[Evaluation and risk assessment of geriatric ankle fractures? : What must be considered?].

The frequency of fractures in old age is steadily increasing including geriatric ankle fractures. These injuries are associated with increased morbidity and prolonged hospitalization. The consequences of courses prone to complications represent a relevant socioeconomic factor and the goal of treatment is to preserve the patient's mobility and quality of life. To ensure optimal perioperative care, the identification of geriatric patients, evaluation of the functional status and existing risk factors with subsequent interdisciplinary treatment are required. Various surgical procedures are availa

Gunshot Wounds and Orthopaedic Injury at a Pediatric Level I Trauma Center.

BACKGROUND: Gunshot wounds (GSWs) and firearm-related injuries are a leading cause of pediatric unintentional injury and death. Orthopaedic firearm injuries are also associated with higher than anticipated mortality, disproportionately affecting socioeconomically and racially disadvantaged patients. The purpose of this study was to characterize GSW injuries at our institution and to identify associated risk factors for these injuries in affected populations. METHODS: A retrospective review was performed of patients aged 0-17 years who were treated for a GSW from April 2012 to October 2023 at

Planned Fibular Osteotomy as a Method of Biological Dynamization in the Staged Damage-Control Management of a Fraser Type I Floating Knee.

INTRODUCTION: Floating knee injuries are among the most severe manifestations of musculoskeletal damage. They commonly come in with damaged soft tissues, a lot of blood loss, and people who are about to die from physiological collapse. It is never easy to take care of serious injuries. Every choice tries the delicate balance between biology and stability, especially when open wounds need both caution and urgency. CASE REPORT: A 22-year-old male, a chronic alcoholic with an O-negative blood group, was brought in after a high-velocity road traffic collision. Radiographs showed a Fraser Type I f

Optimising the utility of patient-reported outcome measurements during recovery after orthopaedic extremity fractures: protocol for a multicentre prospective cohort study.

INTRODUCTION: Recovery after extremity fracture is influenced by injury-related impairments, functional capacity, symptom severity and psychosocial well-being, emphasising the need for patient-reported outcome measures (PROMs). Despite increasing adoption of Patient-Reported Outcomes Measurement Information System (PROMIS) and legacy PROMs in orthopaedic research, substantial knowledge gaps remain regarding their measurement properties, comparability and clinical utility, particularly in fracture populations. Our aim is to develop meaningful guidance to clinicians regarding the use of PROMs in

The minimally invasive approach for lateral malleolus fractures plating demonstrating less pain, shorter operative time and earlier discharge compared to open approach: a prospective randomized controlled trial.

BACKGROUND: Open reduction and internal fixation (ORIF) has been a long-established and accepted technique for the treatment of lateral malleolar fractures. However, minimally invasive percutaneous plate osteosynthesis (MIPPO) may be preferred in cases with compromised soft tissue conditions or in the presence of comorbidities. MIPPO is increasingly being used in the treatment of long bone fractures. The aim of this study was to compare the clinical, functional and radiological outcomes of the MIPPO technique for lateral malleolus fractures with those of ORIF. METHODS: This prospective random
