Achilles Tendon Repairs in Adolescent and Young Adult Athletes <25 Years Old: A Retrospective Multicenter Case Series.

BACKGROUND: The outcomes of Achilles tendon tears have been minimally investigated in adolescents and young adults, with sparse literature-based evidence to guide management. PURPOSE/HYPOTHESIS: The purpose of this study was to describe the demographics, presentation, treatment, complications, and long-term functional follow-up including patient-reported outcomes (PROs), and return-to-sport (RTS) rates of adolescent and young adult athletes with Achilles tendon ruptures. It was hypothesized that despite high activity demands, perioperative complication rates would be low and RTS rates high.

Walking on the Achilles tendons.

BACKGROUND: The 'walking Achilles tendon' technique can be safely adopted when the initial percutaneous repair fails to achieve sufficient grip in the proximal stump of the ruptured tendon. In such instances, two additional stab wounds are produced proximal to the initial most proximal ones to increase the tendon area available for suture grip. SOURCES OF DATA: A total of 36 patients had who sustained Achilles tendon rupture were included in the present study, divided in two groups: 'the walking on the Achilles tendon group' and 'traditional repair' group. AREAS OF AGREEMENT: There was no di

Comparative outcomes of Kirschner wire and lag screw fixation in pediatric medial malleolar fractures.

BACKGROUND: Salter-Harris type III and IV medial malleolar fractures (McFarland fractures) are uncommon but clinically significant injuries in skeletally immature patients, with a substantial risk of premature physeal closure (PPC) and subsequent growth disturbance. Various fixation methods, including Kirschner wires (K-wires) and cannulated screws, are used in clinical practice; however, comparative data on their outcomes in pediatric medial malleolar fractures are limited. METHODS: This retrospective study included 60 patients (38 males, 22 females; mean age 11.57 ± 1.78 years) with Salter-

Neurocognitive Deficits Related to Ligamentous Ankle Injuries: A Systematic Review.

BACKGROUND: Lateral ankle sprains (LAS) are among the most common sports injuries, with up to 40% of individuals developing chronic ankle instability (CAI). While altering neurocognitive demands can affect lower limb biomechanics, the underlying mechanisms in CAI patients remain unclear. This systematic review aimed to summarise evidence on the neurocognitive deficits linked with ligamentous ankle injuries and CAI. METHODS: Following PRISMA guidelines (PROSPERO: CRD42023406395), a comprehensive search of five databases (up to September 24, 2024) identified studies examining neurocognitive per

Updated German National S1 Guideline on Pediatric Lower Leg Shaft Fractures.

Fractures of the diaphysis of the lower leg account for approximately 6% of all fractures in children and adolescents. A distinction must be made between isolated tibial shaft fractures, which are primarily managed conservatively, and combined lower leg shaft fractures (involving both the tibia and fibula). The latter are significantly less stable and therefore more frequently treated with osteosynthesis. If the fracture is non-displaced or tolerably displaced, immobilization in a long leg cast for about 4 weeks is sufficient. This requires that no secondary displacement occurs within the cast

Impact of surgical timing on wound complications following ankle fracture fixation: A 22-year retrospective cohort study.

BACKGROUND: Ankle fractures are common injuries, with a significant proportion requiring surgical intervention. Despite established treatment principles, the optimal timing for open reduction and internal fixation (ORIF) remains debated. Concerns regarding wound infection risk have led some surgeons to delay fixation, while others advocate for early intervention. This study aimed to assess the impact of surgical timing on postoperative wound infections. METHODS: A retrospective review was conducted on 1130 patients with surgically treated closed ankle fractures at a single institution between

Home-Based Digital Exercise Program for Patients After Open Repair of Acute Achilles Tendon Rupture: Noninferiority Randomized Controlled Trial.

BACKGROUND: Achilles tendon rupture significantly affects patient mobility and quality of life. Postoperative rehabilitation is critical to regain ankle function, strength, and return to activity. Home-based digital rehabilitation has emerged as an accessible alternative to traditional clinic-based physiotherapy; however, high-quality evidence comparing these approaches after Achilles tendon repair remains scarce. OBJECTIVE: This randomized controlled trial aimed to evaluate whether a home-based digital exercise program (DEP) is noninferior to clinic-based face-to-face physiotherapy for resto

Acute Compartment Syndrome Following a Low-Energy Ankle Injury: Case Report and Review of the Literature.

A healthy 21-year-old male active-duty sailor experienced acute compartment syndrome (ACS) of the right lower extremity after sustaining an inversion ankle injury during a summer football practice. In the emergency room, he was diaphoretic, with diffuse tenderness over the lateral leg and ankle and mild paresthesia over the dorsum of the foot. He had palpable pedal pulses and no pain with passive great toe extension. Initial radiographs demonstrated no acute osseous abnormality. However, the emergency department team ordered a creatine kinase (CK) out of concern for rhabdomyolysis. It returned

Fixation for calcaneal tuberosity fracture (beak fracture) using preformed "L-shape" hook plate.

OBJECTIVES: Surgical management of calcaneal tuberosity fractures is challenging, as patient outcomes can be compromised by skin flap necrosis and implant failure. We propose a technique utilizing a prefabricated L-shaped hook plate, which represents an innovative clinical orthopedic surgical approach. METHODS: In this retrospective study, patients with Beavis type II calcaneal tuberosity fractures underwent internal fixation using a preformed "L-shape" hook plate (2015-2020). Data on operative time, complications, and healing time were recorded. Functional outcomes were evaluated using the A

Clinical and radiological outcomes of bimalleolar ankle fractures treated with open reduction and internal fixation: a prospective observational study.

PURPOSE: Bimalleolar ankle fractures are common injuries that typically result from trauma such as falls or road traffic accidents. Open reduction and internal fixation (ORIF) is the standard treatment and aims to restore proper alignment, stability, and ankle function. Despite the overall effectiveness of ORIF, patient outcomes may vary depending on age, sex, fracture type, and the timing of surgery. This study evaluates the clinical, radiological, and functional outcomes of bimalleolar ankle fractures treated with ORIF and identifies factors influencing recovery and complications. METHODS:

American Fellowship-Trained Foot & Ankle and Trauma Surgeons Fix the Majority of Calcaneal Fractures in the First Years of Independent Practice.

BACKGROUND: Calcaneal fractures are among the most challenging foot injuries managed by orthopaedic surgeons. Given their high technical demand and inherent complication profile, calcaneal fractures are often, but not always, referred to subspecialists with fellowship training in either Foot and Ankle (FA) or Trauma surgery. The primary purpose of this study was to evaluate practice trends and to determine whether calcaneal fracture ORIF volume and early complication rates differ among early-career American orthopaedic surgeons (typically within their first 1-2 years of practice) by fellowship

Outcome of Open Distal Tibial Physeal Fractures: A Descriptive Report of Two Cases.

Open distal tibial physeal injuries are rare but clinically significant due to their proximity to the ankle joint and the potential impact on future growth in skeletally immature patients. These injuries pose unique challenges, including a higher risk of delayed healing, premature physeal closure, infection, and long-term deformity. We report two pediatric cases of open Salter-Harris type II distal tibial physeal fractures associated with Gustilo-Anderson type III soft-tissue injuries. Both patients underwent urgent debridement, stabilization with K-wire fixation, and multidisciplinary soft-ti

Effects of transcranial magnetic stimulation on cortical network modulation and functional recovery in patients with chronic ankle injury following surgery: study protocol for randomised controlled trial.

INTRODUCTION: Single-modality motor rehabilitation is often insufficient to address the complex functional restoration needs of patients with chronic ankle injuries following surgery. Research indicates that non-invasive neuromodulation techniques, such as intermittent theta-burst stimulation (iTBS), may enhance motor recovery by increasing cortical excitability and facilitating neural network reorganisation. Compared with exercise therapy alone, this study aims to explore the rehabilitation efficacy of combining iTBS with exercise therapy in patients with chronic ankle injuries following surg
