BACKGROUND: Open primary repair is widely used for acute Achilles tendon rupture but is associated with wound-related complications. Endoscopic flexor hallucis longus (FHL) transfer has been proposed as an alternative strategy to reduce soft-tissue morbidity.
PURPOSE: To compare clinical outcomes and complication rates between open primary repair and isolated endoscopic FHL transfer in acute Achilles tendon ruptures.
METHODS: Patients who underwent surgical treatment within 7 days of injury between 2023 and 2024 were retrospectively reviewed. All ruptures involved the watershed region. Patie

Microvascular fibular flaps are an established option for reconstruction of large segmental bone defects, but their use in infants following trauma is exceptionally rare. A six-month-old male infant sustained a gunshot wound causing a 5-cm humeral diaphyseal defect. Definitive reconstruction at nine months of age employed a 7-cm osteocutaneous fibular free flap with a 4 × 2 cm skin paddle. The graft was inverted and fixed intramedullary with a K-wire; microvascular anastomoses connected the peroneal artery to a proximally transposed radial artery and flap veins to superficial veins using 10-0

Bilateral chronic Achilles tendon rupture is an exceptionally rare condition with limited evidence guiding optimal management. We present a unique within-patient comparison of two reconstructive techniques: endoscopic flexor hallucis longus (FHL) tendon transfer on one side and open FHL transfer on the contralateral side. This design eliminates interpatient variability and allows direct comparison of operative time, surgical exposure, cosmetic outcomes, and functional recovery. Functional outcomes were assessed at 12 months and confirmed at the 24-month follow-up. Both techniques resulted in e

This study aimed to investigate risk factors for poor functional recovery at 6 months after open reduction and internal fixation for ankle fracture, develop a predictive nomogram, and perform internal validation to support early identification of high-risk patients and formulation of individualized postoperative rehabilitation strategies. A retrospective cohort study was conducted including 582 patients with ankle fracture treated between January 2022 and December 2024. The primary outcome was poor functional recovery, defined as an AOFAS ankle-hindfoot score < 80 at 6 months postoperatively.

CASE: After an unremarkable arthroscopic evaluation for foot and ankle pain after a minor injury, an 18-year-old athlete had persistent pain and developed a flatfoot deformity. Magnetic resonance imaging revealed an osteochondral lesion of the calcaneocuboid joint and spring ligament failure. Arthroscopy confirmed the lesion and retrograde drilling provided safe access. Simultaneously, a comprehensive flatfoot reconstruction was performed, including gastrocnemius recession, spring ligament repair, and minimally invasive calcaneal osteotomy.
CONCLUSION: A hybrid surgical approach-combining art

BACKGROUND: Open reduction and internal fixation (ORIF) of geriatric ankle fractures is associated with infection and wound-related complications, especially in patients with decreased functionality and increased comorbidity. The purpose of this study was to propose a risk-related score to anticipate the development of surgical site infection (SSI) and/or wound complications after ORIF of surgical ankle fractures among frail, elderly patients.
METHODS: A national database was queried for elderly patients undergoing ORIF for lateral malleolar, bimalleolar, and trimalleolar ankle fractures. Ris

BACKGROUND: The optimal timing of weight-bearing after surgical fixation of isolated lateral malleolar fractures remains debated. Although delayed mobilization is commonly practiced, early weight-bearing has been proposed as a strategy to improve functional recovery without increasing complications. This study compared early versus delayed weight-bearing following operative treatment of AO/OTA 44-B1 fractures using a homogeneous fracture cohort.
METHODS: This retrospective cohort study included 89 adults who underwent open reduction and internal fixation for isolated lateral malleolar fractur

BACKGROUND: This study investigated whether the preoperative joint line convergence angle (JLCA) influences post-operative correction accuracy and clinical results in knee osteoarthritis (KOA) patients undergoing medial open-wedge high tibial osteotomy (MOWHTO). METHODS: A total of 169 knees that underwent MOWHTO were followed up for a minimum of 2 years. Preoperative and 2-year postoperative radiographic parameters were used to measure JLCA and the hip–knee–ankle angle (HKA). A post-operative HKA valgus of 0–4° was defined as accurate correction; values outside this range were considered abno

To synthesise the literature on pre- and intra-operative nerve mapping used to prevent iatrogenic nerve injuries during ankle arthroscopy. This scoping review followed the PRISMA Extension for Scoping Reviews (PRISMA-ScR) guidelines. A systematic search was conducted using MEDLINE and Embase on 24 July 2024. Findings extracted from eligible studies were tabulated and synthesised. The search identified 270 articles, of which 9 met inclusion/exclusion criteria and were included in the review. Two studies described a mapping technique without reporting any outcomes, and three studies evaluated ma

PURPOSE: Ankle fractures are common, with higher complication rates in elderly, comorbid patients and those with complex injury patterns. Minimally invasive techniques such as intramedullary fibular fixation have gained interest for their potential to reduce morbidity. This study compared outcomes of fibular nailing versus plating in high-risk ankle fractures and aimed to identify the patient profile most likely to benefit from intramedullary fixation.
METHODS: A retrospective, propensity score-matched case-control study was conducted at a level I Trauma Center. Patients with low-energy rotat

Distal tibial fractures present a therapeutic challenge due to limited soft-tissue coverage and the need to achieve stable fixation while preserving local vascularity. Minimally invasive plate osteosynthesis (MIPO) has emerged as a biologically favorable technique for managing selected extra-articular distal tibial fractures by minimizing soft-tissue disruption and maintaining fracture hematoma. This case series describes three patients with right-sided closed extra-articular distal tibial fractures classified as AO Foundation/Orthopaedic Trauma Association (AO/OTA) 43-A1.2, managed with MIPO

Open tibial fractures are challenging injuries due to limited soft tissue coverage, contamination, and the risk of infection or delayed union. The Ilizarov circular external fixation system provides stable fixation while preserving periosteal vascularity, minimizing further soft-tissue disruption, and allowing early weight bearing. We report a case of a 50-year-old man who sustained a Gustilo-Anderson type I open comminuted tibial fracture following a road traffic accident. Initial management included wound irrigation, sterile dressing, immobilization, and intravenous antibiotics. Definitive t

Open reduction and internal fixation is often the definitive surgical treatment for unstable ankle fractures, however, there is significant debate regarding the optimal postoperative weight-bearing protocol. This study aims to evaluate the total costs associated with immediate unprotected weight-bearing, immediate protected weight-bearing, early weight-bearing, and delayed weight-bearing protocols following ankle fracture ORIF. A search of the PubMed electronic database was conducted in July 2025. The primary outcomes of interest included the effects of different weight-bearing protocols on he

OBJECTIVES: Prior literature indicates fear of reinjury (FORI) as a limitation in recovery from various orthopaedic conditions. However, no studies evaluate whether fear affects return to activity after ankle fractures. This study, therefore, investigates whether FORI is associated with failure to return to preinjury activity level regardless of fracture subtype. This study also sought to determine the proportion of patients experiencing fear after ankle fractures.
DESIGN: Retrospective cohort study.
SETTING: Urban, quaternary care academic medical center.
PATIENTS/PARTICIPANTS: All patient
