Up and down - The impact of calcaneal fracture on medial gastrocnemius fascicle behavior and ankle biomechanics during heel-rise performance.

BACKGROUND: Calcaneal fractures (CF) often result in reduced foot mobility and plantar flexor strength, despite intensive rehabilitation. While treatment mostly focuses on bone restoration, structural and functional adaptation of the ankle plantar flexors during movement is less understood. RESEARCH QUESTION: To investigate medial gastrocnemius fascicle behavior and ankle biomechanics in patients with CF during bipedal heel-rise. METHODS: Repetitive heel-rise was analyzed in 18 patients with unilateral CF (BMI: 27.3 ± 3.2 kgm-2, Age: 50 ± 13 years) and 18 healthy controls (BMI: 26.3 ± 3 kgm-

An Unusual Combination of Injuries Involving Lateral Dislocation of the First and Fifth Metatarsophalangeal Joints Along With Fractures of the Other Toes on the Same Foot: A Report of a Rare Case.

Dislocations of the metatarsophalangeal (MTP) joints following trauma are uncommon and often underreported. Most documented cases involve the first MTP joint, or occasionally one or two of the lesser MTP joints, with the direction of dislocation predominantly dorsal. The rarity of these injuries, particularly those affecting multiple joints or exhibiting atypical patterns, poses challenges in both diagnosis and management. In some instances, diagnosis is difficult without imaging. Prompt reduction is essential to prevent neurovascular complications. MTP joint dislocations can be reduced using

Extended Subtalar Approach in the Surgical Treatment of Depressed Intra-Articular Calcaneus Fractures With Depression of the Medial Fragment of the Posterior Facet.

BACKGROUND: Several alternative incisions have been developed for the surgical treatment of calcaneus fractures, including the extensile lateral approach, medial approach, combined lateral and medial approach, limited posterolateral approach, and sinus tarsi approach. This study aimed to describe a modified approach in which we extended the sinus tarsi approach and examined the reduction success in intra-articular fractures involving the medial column. METHODS: Patients with calcaneus fracture surgery between 2017 and 2021 were reviewed retrospectively. Computed tomography was obtained immedi

Acute Compartment Syndrome: A Short Narrative Review of the Risk Factors, Complications, and Medicolegal Impact of a Missed Diagnosis.

Acute compartment syndrome (ACS) is a frequently encountered surgical emergency in trauma and orthopaedics. Timely diagnosis and prompt surgical fasciotomy of the involved compartments prevent complications, which can cause long-term morbidity, rehabilitation, renal failure, multiorgan failure, and even death if management is delayed or diagnosis is missed. The aim of this review was to identify and summarise existing literature on ACS, its complications, and the medicolegal impact when a diagnosis is delayed or missed. A narrative review was conducted using evidence from EMBASE, PubMed, and G

Assessing the role of posterior to anterior calcaneal screw fixation in tibiotalocalcaneal nailing after ankle trauma: A retrospective comparative study.

Tibiotalocalcaneal (TTC) nailing is commonly used to treat severe ankle trauma, especially in patients with poor bone quality or compromised soft tissue. Although biomechanical studies suggest that posterior-to-anterior (P-A) screws enhance stability, the clinical impact of these screws remains uncertain. This study aims to compare complication rates between patients who underwent TTC nailing with and without P-A screws after ankle trauma. A retrospective analysis was conducted on 33 patients who underwent TTC nailing for ankle trauma between 2016 and 2022. Patients were divided into two group

Technical Tip: The Use of Arthroscopic Portals to Safeguard Medial Neurovascular Structures During Suture Button Fixation.

IntroductionAnkle fractures with concurrent syndesmotic instability are commonly encountered orthopaedic injuries. Suture button syndesmotic fixation has emerged as a favorable alternative to traditional screw fixation due to its comparable biomechanical stability and reduced hardware removal rates. However, concerns persist regarding potential injury to medial neurovascular structures during tibial cortical button deployment and a medial incision is often recommended for verification.TechniqueThis paper describes a safe and reproducible technique for suture button fixation that offers an alte

A 50-year perspective on the use and potential of artiodactyl calcanei in bone adaptation studies.

Sheep and deer calcanei are important models for studying cortical (compact) and trabecular (cancellous) bone adaptation because they are amenable to direct strain measurement (due to lack of surrounding muscles), experience relatively simple/unidirectional bending, exhibit osteon remodelling, and have the most pronounced regional variations in mineralization and other histological characteristics reported in any bone. This simple loading environment is characterized by bending that produces prevalent/predominant tension on the plantar side and predominant compression on the dorsal side of the

Better management of Sanders Ⅱ and Ⅲ calcaneus fractures via a tailored distractor-assisted percutaneous approach versus sinus tarsi approach: a comparative cohort study with 2-year follow-up.

BACKGROUND: The surgical strategy of displaced intra-articular calcaneal fractures (DIACFs) remains technically challenging. While sinus tarsi approach (STA) is widely applied for DIACFs, increasing concerns regarding the wound-related sequelae drive surgeons to target and advance minimally invasive surgery (MIS). This study aims to introduce a tailored distractor-assisted MIS and compares its medium-term outcomes with conventional STA approach reduction and fixation in patients with Sanders Ⅱ and Ⅲ calcaneus fractures. METHODS: From Jan 2021 to Jun 2023, 133 cases (133 feet) diagnosed as DIA

Persuasive of lateral supra malleolar flap for distal leg and foot reconstruction. A cross-sectional study.

INTRODUCTION: Distal leg and foot have unique anatomical characteristics, and multiple etiologies may cause defects in various patterns that mandate reconstruction. Local flaps, such as perforator flaps, can give good outcomes with less complication. The lateral supra malleolar flap can effectively cover the distal leg and foot defect. In this article, we highlight the utility of this flap and its outcome. MATERIAL AND METHODS: This study is a descriptive, retrospective, multi-center, hospital-based performed to assess the efficacy of lateral supra malleolar flap in the reconstruction of dist

Iatrogenic medial malleolar fracture and stress fracture considerations in total ankle joint replacement: A multicenter retrospective study.

Total ankle replacement (TAR) is a challenging but rewarding treatment option for primary or post-traumatic tibiotalar osteoarthritis. Acute and latent postoperative stress fractures of the medial malleolus, however, pose a common potential complication that may arise during joint resection, especially during implantation of the tibial component. This is a multicenter retrospective study highlighting considerations for iatrogenic medial malleolar fractures during and after TAR surgery by assessing relationships between fractures and medial malleolus width (MMW). 159 patients undergoing primary

The sural nerve & the calcaneus: A radiographic and anatomical study.

BACKGROUND: The sinus tarsi approach (STA) is a commonly used minimally invasive surgical approach for the fixation of joint depression-type calcaneus fractures. As the sural nerve (SN) is particularly at risk during the STA, understanding the SN's course in relation to bony landmarks on fluoroscopic imaging is crucial. METHODS: Dissection of 19 post-mortem humans were performed to expose the SN. The SN's course was mapped relative to radiographic bony landmarks, anatomical plates for fracture fixation, and the STA incision location. RESULTS: The SN typically enters the cranial part of the c
