Difficult fractures of the calcaneus are therapeutic problem today. From 1965 to 1977 147 patients were non-operative treated in the department of general surgery - including cases from department of orthopedic surgery. 67 patients with fractures of the os calcis were controlled between 3 and 15 years after accident. 132 injuries of the Lisfranc joint were reviewed in order to show methods of diagnosis and treatment. Injuries of this joint are rare. Almost one third are open fractures and/or dislocations, mostly comminuted lesions, associated with severe soft tissue trauma and so decreasing blood supply of the foot. Most dislocations and fractures can be treated conservatively by closed reductions and following fixation by means of Kirschner wires. The traditional view of calcaneal fractures in children is that this injury and its consequences are generally less severe than its adult counterpart. Ulcers on the sole of the foot are very uncomfortable. A poor blood supply and a tendency to slow healing are common in these patients. Because of its weight-bearing and unique sensory function the glabrous skin of the sole is very specialized. The vascular-pedicle "wrap-around" free flap was used in five patients for reconstruction in amputations distal to the metacarpophalangeal joint. In five other patients who had normal thumbs but had lost all fingers, the technique was expanded to include the second toe with great toe wrap-around flap. All patients did well, but significant bone peg resorption occurred in six patients and in three others the bone peg fractured.

Stubbing injuries to the hallux, previously unreported, result in specific fractures and fracture dislocations of the interphalangeal joint. Report about two cases of severe injuries to the thumb. One severe crush injury was treated primarily; in the other case of complete amputation a late reconstruction was performed. Methods and modern concepts for these problems are discussed. The importance of long vein grafts in microsurgery is pointed out. The advantage of reconstructing all structures primarily is stressed. The accessory bones and ossification centers in the foot and ankle were studied in 3,460 radiographs of patients over 7 years of age who consulted our clinic for various reasons from January 1967 to December 1979. Internal fixation of open fractures can be carried out according to the same indications employed in closed fractures in the presence of a type I wound. In type II and type III wounds early internal fixation may be indicated in victims of multiple trauma, in the elderly, in intra-articular fractures, in mutilated limbs, and in some fractures with associated vascular injuries. Defects or clefts may be seen in the growing epiphysis and are usually observed just before puberty. The basal epiphysis of the proximal phalanx of the great toe is the most common site but similar defects have been observed in a number of other epiphyses. At least some of these defects develop within a single normal appearing epiphysis and are not associated with signs or symptoms suggestive of fracture. The mechanism of formation of these defects is not clear. The defects probably close spontaneously sometime around late puberty.

A case of avulsion fracture of the lateral sesamoid is reported in association with dislocation of themetatarsophalangeal joint of the hallux. The radiographic criteria to establish the diagnosis of fracture in sesamoid bones are outlined. Review of the literature reveals only one other reported case associated with a hallux dislocation, and in that case, the radiographic diagnosis was not clearly established.

Knee problems are the most common injury sustained in both male and female runners. Other problems include toe and forefoot injuries, inferior heel and arch pain, shin splints, ankle pain, calf and Achilles tendon pain, groin and hip pain, and stress fractures. The average total treatment for all injuries required 4.2 physician visits. Orthotics were used for more than 60% of the injuries, with successful results. A thorough understanding of running injuries should assist the physician in the treatment of these problems.

Experience with forty-nine patients with surgically treated transchondral fractures of the talar dome seen between 1957 and 1977 at our institution has indicated that surgical treatment yields a good long-term result. Analysing 17 cases of navicular fracture, authors divide the fracture dislocations in two groups. It is possible to prolong a postero-medial surgical approach to the leg as far as the tarsal canal in order to gain access to the posterioir aspect of the diaphysis and of the shaft of the tibia up to the tibio-tarsal line.
