Corticospinal and Spinal-Reflex Excitability during Unipedal Stance Following Acute Lateral Ankle Sprain.

BACKGROUND: Acute lateral ankle sprains (ALAS) are linked to altered spinal-reflexive excitability that may predicate sensorimotor dysfunction. However, sensorimotor control is influenced by spinal and supraspinal mechanisms, necessitating examination of each following ALAS. We aimed to investigate the effects of ALAS on neural excitability during unipedal stance, relating these neural adaptations to clinical outcomes. METHODS: Twenty-six young adults were enrolled (13 ALAS: 19.6 ± 1.9 yr; 13 uninjured controls: 20.7 ± 2.4 yr). Spinal-reflex excitability was assessed at the soleus using the H

Heel kicking exercise rapidly improves pain and function in patients with acute lateral ankle sprain: a randomized controlled trial.

BACKGROUND: Ankle sprain is one of the common traumatic injuries in daily life, and PRICE (protection, rest, ice, compression pressure bandaging, elevation of the affected limb) principle is routinely recommended at its early stage. In traditional Chinese medicine (TCM), bone-setting technique (BST) can quickly relieve ankle pain and improve walking function, but physicians require high skills and physical strength. In this study, we designed an exercise-heel kicking exercise (HKE) based on the principles and methods of BST. This study aims to observe its true efficacy and safety. METHODS: 68

Reproducibility of a modified posterior talar glide test in ankle sprain conditions: a cross-sectional analysis on chronic ankle instability, copers, and healthy controls.

BACKGROUND: The posterior talar glide test (PTGT) is recommended for ankle sprain assessment, but it has limited scientific support. Therefore, the aims of this study were to assess the reproducibility of PTGT in two conditions: 1) the clinical experience of the clinician and 2) in a modified setting using a referential horizontal plane in three clinical conditions (chronic ankle instability (CAI), copers, and healthy controls). METHODS: Twenty-eight participants were recruited. PTGT measurements were performed using two raters, i.e., novice and experienced, and performed twice with and witho

Posterior Ankle Impingement Due to Hypertrophic Low-Lying Flexor Hallucis Longus Muscle: A Case Series and Literature Review.

Posterior ankle impingement syndrome (PAIS) is a clinical disorder with pain elicited when the foot is placed into a forced plantarflexion position, resulting in compression of bony and/or soft tissue structures located within the posterior ankle region. Numerous soft tissue structures exist in close proximity to the posterior ankle joint which can also result in impingement, with flexor hallucis tendinitis being associated in 63% to 85% of cases. The purpose of this study is to present 3 cases of both open and endoscopic surgical approaches for the treatment of PAIS caused by a hypertrophied

Functional Recovery in Active-Duty Military Personnel Following a Novel, Self-applicable, Stepwise, Target-Oriented Protocol for Lateral Ankle Sprain: A Prospective Cohort Study.

BACKGROUND: Due to limited access to hospital-based rehabilitation, many active-duty soldiers return to duty after lateral ankle sprain (LAS) without the opportunity for stepwise rehabilitation, despite the risk of delayed functional recovery and re-injury. This prospective cohort study aimed to introduce and evaluate the effectiveness of a low-cost, self-applicable, stepwise, target-oriented protocol for LAS (STOP-Sprain) in facilitating functional recovery. METHODS: A total of 60 participants were finally included and divided into 2 groups: those who followed the protocol (under protocol, g

Associations between diaphragm contractility and unilateral drop landing kinetics in individuals with bilateral recurrent lateral ankle sprains.

INTRODUCTION: Recurrent lateral ankle sprains (LAS) are linked to neuromuscular and respiratory impairments, including reduced diaphragm contractility and altered landing mechanics. These deficits related to recurrent LAS may contribute to compromised ankle joint stability and increased risk of recurrent injury. However, no studies have explored the associations between diaphragm function and landing kinetics. This study aimed to examine the associations between diaphragm contractility, peak ground reaction force (GRF), and loading rate during a unilateral drop-landing task in individuals with

Impact of systematic ultrasonography on lateral ankle sprain management.

BACKGROUND: Lateral ankle sprain (LAS) is the most common traumatic injury, yet its diagnosis and management remain suboptimal. This contributes to a high prevalence of residual symptoms, negatively impacting patient quality of life and increasing healthcare costs. We hypothesized that routine ultrasonography (US) following LAS could significantly modify the treatment strategy established by a specialist. Additionally, we hypothesized that US is an efficient tool for identifying osseous injuries and associated ligamentous injuries. METHODS: Fifty-one ankles with lateral instability diagnosed
