Unseen but unforgettable - refractive bacilli in the gram stain of a muscular abscess.

The epidemiology of tuberculosis has been altered due to the increased use of immunosuppressive therapies (chemotherapy and biologic agents), the human immunodeficiency virus (HIV) epidemic, and the changing migration patterns. Extrapulmonary tuberculosis occurs most often in individuals with HIV infection. Osteoarticular tuberculosis is rare in Western countries, with its diagnosis being performed through culture and/or molecular detection, however muscular tuberculosis without coexistent skeletal involvement is rare, with only 30 cases reported in the literature. The authors present the case

Pelvic support osteotomy with subsequent femoral lengthening: a reliable treatment for late tuberculous coxitis in skeletally immature patients.

INTRODUCTION: Tuberculous (TB) coxitis, constituting 15-20% of musculoskeletal tuberculosis cases, often leads to late-stage complications. Joint replacement yields unsatisfactory results. This study aims to know about the efficacy of pelvic support osteotomy (PSO) with subsequent femoral lengthening in treating late TB coxitis in skeletally immature patients. METHODS: Thirteen pediatric patients with late TB coxitis underwent retrospective analysis from 2016 to 2024. PSO with femoral lengthening, utilizing limb reconstruction systems (LRS) external fixator, was performed. Outcomes were asses

Imaging in Pott's spine: A review of typical and atypical imaging features and diagnostic challenges.

Pott's spine is associated with high morbidity, long-term disabling sequelae and even mortality. The classical paradiscal form, where there is destruction of end plates of adjacent 2-3 vertebral bodies along with intervening disc involvement and associated paravertebral abscess is well recognized. However, due to its varied clinical and radiological presentations, it remains a diagnostic challenge often resulting in delay in diagnosis. This is especially true of countries where tuberculosis has been almost eliminated but now are witnessing increase in incidence both due to immigration and HIV

Accidental needle inoculation of bacillus Calmette-Guerin vaccine in medical worker - TBC arthritis.

Despite the increase of mycobacterial infection, the incidence of tuberculous hand infection remains extremely low. We report a case of a 51-old pediatric nurse, who inadvertently inoculated her index finger via needlestick, while performing BCG immunization of the baby. Two days after the event, painful swelling and erythema of the distal interphalangeal joint region of the index finger appeared. Further diagnostic confirmed rare case of mycobacterial finger joint infection. It is important that hospital stuff should be informed about the possibility of BCG accident transmission to reduce the

Comparison Between the Efficacy of Oblique Lumbar Debridement Using an Expandable Channel Combined With Posterior Percutaneous Internal Fixation and Traditional Anterior-Posterior Surgery for Single-Segment Lumbar Tuberculosis.

AIM: Lumbar tuberculosis can cause spinal instability and neurological deficits, often requiring surgery. Traditional anterior-posterior surgery is effective but highly invasive, leading to greater trauma and longer recovery. Minimally invasive techniques, such as oblique lumbar debridement with posterior percutaneous fixation, may reduce surgical damage and improve recovery. However, their efficacy remains unclear. This study compares this minimally invasive approach with conventional surgery to assess its feasibility as an alternative treatment. METHODS: A retrospective analysis was conduct

Inhibition of miR-199b-5p Suppresses the Tuberculosis-Induced Inflammation in Spinal Tuberculosis via Targeting Gcnt2.

Spinal tuberculosis (STB) is more common in children and young adults and can lead to neurological deficits and spinal deformity in severe cases. Therefore, it is important to find a biomarker for studying the pathogenesis of STB. RT-qPCR was utilized to measure the expression of miR-199b-5p in STB. Moreover, the GPL33147 database was utilized to validate the expression of miR-199b-5p in both STB patients and healthy individuals. Pearson's correlation analysis was employed to explore the correlation between miR-199a-5p and Glucosaminyl (N-acetyl) transferase 2 (Gcnt2) in peripheral blood monon

Tuberculous monoarthritis in a rituximab-treated patient with rheumatoid arthritis: diagnostic and therapeutic challenges in a tuberculosis-endemic region.

A woman in her 60s with seropositive rheumatoid arthritis developed tuberculous arthritis of the wrist following rituximab therapy, despite negative latent tuberculosis (TB) screening. Although rituximab is considered safer than anti-TNF agents in terms of TB risk, this case highlights a rare yet serious complication. Diagnosis was confirmed via synovial fluid analysis after persistent monarthritis unresponsive to standard RA therapy. Management was complicated by drug-induced liver injury and ototoxicity, leading to multiple modifications of anti-tubercular therapy. The therapeutic dilemma ce

Spinal tuberculosis, pathophysiology and radiological presentation, three case reports.

Prompt diagnosis and treatment of spinal tuberculosis are key in preventing its neurological and physical sequelae. This affection, also known as Pott's disease, should be considered a differential diagnosis in patients presenting with unexplained back pain that can lead to neurological symptoms and eventually paraplegia. Mycobacterium tuberculosis, the etiological agent of tuberculosis, spreads from the lungs to the spine via venous or arterial pathways, causing lesions apparent upon imaging. Radiological findings include osseous destruction, disk collapse, abscess formation, and spinal defor

Atypical chest wall pain: paravertebral tuberculosis mimicking costochondritis.

This case describes a woman in her 30s presenting with a 2 month history of thoracic pain, initially suspected to be costochondritis. The pain was unresponsive to conservative management, and imaging revealed a paravertebral mass at the T6-T7 level. The differential diagnosis included malignancy, sarcoma and tuberculosis (TB). A positive QuantiFERON-TB Gold test suggested TB exposure, before a biopsy subsequently confirmed spinal TB. She was started on quadruple anti-tuberculosis therapy and showed significant clinical improvement. This case emphasises the importance of considering TB in the d

An Unusual Case of Tuberculosis Infection of the Hand with Peritonitis.

Tuberculosis (TB), primarily a pulmonary disease, manifests extrapulmonary involvement in 20% of cases, though concurrent infections in distant sites like the hand and abdomen are exceedingly rare. This report presents an exceptionally rare case of concurrent tuberculosis osteomyelitis of the hand and peritoneal tuberculosis in a 19-year-old male-an unusual dual presentation not commonly documented in the literature. The patient had an 8-month history of progressive right little finger swelling, initially misdiagnosed as an enchondroma, and subsequently developed acute abdominal pain and ascit

Quantification and detection of Mycobacterium tuberculosis, HIV-1 co-localization, and reciprocal expansion in spinal tuberculosis tissues.

OBJECTIVES: The paucibacillary nature of spinal tuberculosis (STB) challenges diagnosis. Tissue sites of Mycobacterium tuberculosis (Mtb) and HIV-1 co-infection are suggested to favor reciprocal replication, and reservoir expansion, including in people living with HIV-1 (PLWH) on antiretroviral treatment (ART); however, evidence is scant. This study aimed to evaluate the utility of droplet digital polymerase chain reaction (ddPCR) to detect and quantify Mtb DNA in spinal biopsy tissue, with sensitivity validated against culture and Xpert Mtb/rifampicin (MTB/RIF) Ultra, and determine whether Mt

A novel clinical classification of spinal tuberculosis for decision-making guidelines from Chinese spine surgeons: a multicenter retrospective cohort study.

BACKGROUND: Spinal tuberculosis (TB) presents substantial therapeutic challenges as it can lead to severe structural and neurological complications. However, evidence-based therapeutic decision-making is hindered by the limitations of existing classification systems, which lack anatomical specificity, quantifiable parameters, and reliability. METHODS: The classification system proposed herein categorizes spinal TB into four types based on clinical manifestations and radiologic features: type I (mild): single-segment involvement, with <50% vertebral collapse and preserved stability; type II (m

Combined posterior-anterior surgery with autologous bone grafting for multi-segmental spinal tuberculosis.

BACKGROUND: Multi-segmental spinal tuberculosis is a severe infectious disease of the spine affecting multiple vertebral bodies, often leading to spinal instability, progressive kyphosis, and irreversible neurological impairment. Its diagnosis and treatment are notably challenging due to the extensive distribution of lesions and severe bone destruction. METHODS: A retrospective analysis was conducted on 95 patients with multi-segmental spinal tuberculosis involving four or more vertebrae, who were treated at the Department of Orthopedics, General Hospital of Ningxia Medical University, from J
