[OverView of infectious and non-infectious diseases in French Polynesia in 2025].

Tahiti or the "myth of Paradise", Bora Bora, "the Pearl of the Pacific". Who has never wanted to take a plane and come and land on the heavenly beaches of Polynesia, a French territory at the antipodes of mainland France lost in the middle of the Pacific? However, we do not imagine that 60% of Polynesians live below the metropolitan low-income threshold or that life expectancy is lower than that of the mainland due to the high prevalence of cardiovascular diseases with three quarters overweight population. In addition to non-transmissible metabolic diseases, various pathologies common to temper

Tuberculosis-triggered cytokine storm with hemophagocytic lymphohistiocytosis and tuberculous spondylitis in an apparently immunocompetent host: a case report and literature review.

Hemophagocytic lymphohistiocytosis (HLH) secondary to disseminated tuberculosis (TB) is a rare, life-threatening hyperinflammatory syndrome. We present a 60-year-old man with recurrent fever and syncope. Workup revealed cytopenias, hyperferritinemia (peak 5,802 ng/mL), elevated C-reactive protein (CRP), and hepatic dysfunction, fulfilling HLH-2004 criteria. Imaging showed miliary lung nodules and tuberculous spondylitis at T9. Bone marrow biopsy confirmed hemophagocytosis, and next-generation sequencing identified Mycobacterium tuberculosis. This case demonstrates that disseminated TB can trig

Challenges and advancements in the diagnosis of central nervous system tuberculosis.

Central nervous system (CNS) tuberculosis is one of the most severe forms of extrapulmonary TB, often leading to substantial morbidity and mortality owing to its complex clinical presentation and frequent diagnostic delays. Despite global TB control efforts, CNS TB continues to present unique difficulties owing to its nonspecific clinical manifestations, the paucibacillary nature of cerebrospinal fluid (CSF), and the limitations of conventional diagnostic methods. The diagnostic dilemma is further compounded in resource-limited settings where advanced molecular tools and neuroimaging may not b

FUNCTIONAL AND RADIOLOGICAL OUTCOME FOLLOWING EXTENDED POSTERIOR CIRCUMFERENTIAL DECOMPRESSION IN THE TUBERCULOSIS OF DORSAL SPINE.

Spinal Tuberculosis ranks as one of the most common extrapulmonary varieties of tuberculosis. The study outlines the Extended Posterior Circumferential Decompression (EPCD) procedure for managing tuberculous spondylitis, a prevalent extrapulmonary form of tuberculosis. EPCD involves 360-degree dural decompression, anterior column debridement, and reconstruction following posterior instrumentation. This technique addresses both the infection and associated complications, particularly beneficial in cases with or without paraplegia. EPCD aims to improve outcomes by effectively tackling the pathol

Imaging Assessment of Nontraumatic Pathologic Conditions at the Craniovertebral Junction: A Comprehensive Review.

Nontraumatic pathologic conditions of the craniovertebral junction encompass a range of conditions affecting the complex anatomy of this region without direct physical injury. These conditions include congenital syndromes that predispose individuals to ligamentous laxity, potentially leading to instability. Additionally, rare but noteworthy cases such as Grisel syndrome, a cause of pediatric torticollis, may arise without a traumatic trigger. Inflammatory diseases, including rheumatoid arthritis, ankylosing spondylitis, and crystal deposition, can lead to cervical instability and spinal cord c
