A Nonhuman Primate Model to Evaluate Treatments for Long-Gap Ulnar Nerve Injury.

Among all upper extremity nerves, the ulnar nerve is both the most commonly injured and notoriously difficult to regenerate. Despite this, ulnar nerve injuries remain understudied. Nonhuman primates (NHPs) offer an ideal model for the human upper extremity, but existing NHP nerve trauma literature is biased towards median and radial injury models. To address this, a nonhuman primate ulnar nerve injury model was developed and regeneration assessed following sural nerve autografting using electrophysiological and histological techniques. Unilateral 4 cm ulnar nerve injuries were created at the m

Comparison of the effects of transcranial direct current stimulation and transcutaneous tibial nerve stimulation on the urgency and frequency of women with overactive bladder syndrome: study protocol of a randomized clinical trial.

BACKGROUND: Overactive bladder syndrome is common, with a prevalence of 12-17% among adults. Posterior tibial nerve stimulation is the primary nonpharmacological and conservative treatment for overactive bladder syndrome. While several human brain imaging studies have shown the involvement of supraspinal centers in bladder control, a literature review has found that no research has specifically investigated cortical stimulation through transcranial direct current stimulation as a treatment for overactive bladder syndrome in women. Therefore, this study aims to assess the potential benefits of

Sensory neuron PIEZO1 deletion inhibits dynamic light touch sensitivity in uninjured mice, prevents neuropathic light touch hypersensitivity, and drives compensatory changes in dorsal root ganglia.

Mechanotransduction is vital for sensing various mechanical stimuli, including blunt force and dynamic light touch. The sensation of a punctate mechanical force is very different from that of a brush swept across the skin, yet both involve mechanical stimulation of the skin and embedded sensory afferent endings. However, the sensory neuron mechanisms contributing to punctate vs light touch somatosensation, and how they might become dysregulated in nerve injury to cause pain, remain unclear. Here, we use mice with sensory neuron-specific PIEZO1 deletion to demonstrate sensory neuron PIEZO1 is r

Management of Popliteal Artery Interposition Graft Infection by Extra-anatomic Bypass.

ObjectivesInfection of peripheral interposition grafts is a rare but devastating complication following aneurysm repair. Typically, graft infection necessitates explantation and, if possible, revascularization of the limb. However, treatment complexity varies substantially depending on the location and extent of infection. This case describes the management of a popliteal artery interposition graft infection.MethodsWe describe an 84 year old male with a history of left popliteal artery aneurysm repair with PTFE interposition graft (found on workup a year prior for a gangrenous great toe) who p

Electrophysiological evidence of Hebbian plasticity in awake adult rats.

Hebbian stimulation, based on principles of spike timing-dependent plasticity (STDP), has been successfully used to enhance functional recovery in individuals with spinal cord injury (SCI). To advance therapies using Hebbian stimulation, this study aimed to establish STDP-based protocols targeting spinal motoneuron synapses in awake rats. Adult male and female Sprague-Dawley rats were implanted with stainless steel screws through the skull over the hindlimb area of the left motor cortex to enable epidural cortical stimulation. A custom-made cuff with embedded fine-wire electrodes was placed ar

Electrical Stimulation Therapy to Accelerate Nerve Regeneration Remains Effective Following Postoperative Application of Lidocaine.

BACKGROUND: Brief electrical stimulation (ES) of injured peripheral nerves for 1 hour has been shown to accelerate nerve regeneration with proximal action potential conduction to the neuron cell body, a requirement to elicit therapeutic benefit. Local anesthetic is often used to manage pain in patients. However, using lidocaine after ES therapy has been controversial. We assessed the effects of extraneural usage of lidocaine after ES therapy on nerve regeneration in a rodent nerve injury model. METHODS: Lewis rats underwent tibial nerve transection and immediate repair and randomized to 4 gro

Evaluating Mechanisms and Surgical Alternatives to Hyperselective Neurectomy for Treatment of Spasticity in a Rat Model of Spinal Cord Injury.

BACKGROUND: Hyperselective neurectomy (HSN) involves transection of most terminal nerve branches supplying a spastic muscle with subsequent recovery of strength resulting from collateral regeneration of spared motor neurons. However, the underlying mechanisms driving spasticity reduction have not been defined. This study aimed to address this knowledge gap and compare the efficacy of HSN to other forms of nerve injury as a therapy for spasticity in a rodent model of spinal cord injury. METHODS: Six weeks after T8 spinal transection, rats ( n = 90) were randomized into 5 groups: sham ( n = 18)

The Unusual Suspect-Tarsal Tunnel Syndrome Due to Flexor Digitorum Brevis Hypertrophy.

A rare case of tarsal tunnel syndrome caused by flexor digitorum brevis hypertrophy. The patient presented with medial plantar pain and paresthesia but denies any injury. Magnetic resonance imaging revealed a marked hypertrophy of the flexor digitorum brevis in close proximity to the medial and lateral plantar nerves, with herniation into the porta pedis. Surgical excision of the herniated muscle was performed, along with a tarsal tunnel release. The combined treatment resulted in symptomatic relief.

Intracellular amyloidosis in peripheral nerve and skeletal muscle.

Amyloidosis is an etiologically heterogeneous group of disorders pathologically characterized by the extracellular deposition of amyloid fibrils, leading to tissue damage. It commonly affects the peripheral nervous system and occasionally involves skeletal muscle. The pathogenic mechanisms driving cellular injury in amyloidosis remain elusive. In this study, we examined 3 sural nerve and 5 skeletal muscle biopsies from patients with amyloidosis to localize the deposition of amyloid fibrils. Histologically, the nerve biopsies showed axonal degeneration and Schwann cell (SC) changes; muscle biop

Outcomes of Posterior Tibial Tendon Transfer in Patients With Foot Drop: A Case Series of 11 Patients.

OBJECTIVE: Peroneal nerve (PN) injury is a common neuropathy that affects the lower extremities. It results in loss of dorsiflexion at the tibiotalar joint and eversion at the subtalar joint, ultimately leading to foot drop. Various etiological factors contribute to this pathology. Treatment options include conservative management, steroid therapy, nerve decompression, repair, and reconstruction. However, due to the uncertain outcomes of primary nerve procedures, secondary interventions such as tendon transfers (TTs) generally provide more satisfactory results. This is particularly effective i

Sciatic Nerve Palsy Caused by Tourniquet Use After Pediatric Knee Meniscal Surgery.

Sciatic nerve palsy because of tourniquet use in the pediatric population is rare. Here, we present a case of a 13-year-old girl with sciatic nerve palsy caused by tourniquet use after knee meniscal surgery. On the day after the operation, incomplete tibial and complete peroneal nerve palsy was observed. First, a sciatic nerve palsy caused by the tourniquet use and an iatrogenic common peroneal nerve injury caused by the meniscal suture device of lateral menisci was considered. However, the possibility of the iatrogenic common peroneal nerve injury was ruled out based on the examination findin

Single stage anatomic posterolateral corner reconstruction with modified LaPrade technique, combined with arthroscopic posterior cruciate ligament reconstruction with tibia nail in situ-A current technique.

The posterolateral corner (PLC) of the knee is a key anatomical structure which provides varus and rotational stability to the knee joint. Injuries to the PLC pose a great challenge for orthopedic surgeons due to their complex nature. PLC injuries rarely occur in isolation, with up to 95% being associated with ligament tears. Surgical reconstruction is indicated in Grade III injuries, defined clinically as >10 mm lateral opening on varus stress and >10° external rotation on the dial test, supported by MRI magnetic resonance imaging findings. Several techniques have been described for PLC reco
