Isolated Proximal Tibiofibular Joint Reconstruction With Semitendinosus Tendon Autograft in a Collegiate Athlete.

BACKGROUND: Proximal tibiofibular joint (PTFJ) instability is a rare and often underrecognized cause of lateral knee pain and dysfunction, most commonly presenting as anterolateral subluxation of the fibular head after trauma. While conservative management is first-line, persistent instability may require operative intervention. INDICATIONS: We present the case of a 20-year-old male athlete who sustained a PTFJ injury during sport and noted instability. After a successful PTFJ diagnostic taping trial, he elected for surgical reconstruction after completing his competitive season. Imaging conf

Anatomical Variation of the Posterior Tibial Neurovascular Bundle During Posterior Ankle Arthroscopy for Posterior Ankle Impingement Syndrome: A Case Report.

We report the case of a 32-year-old male professional soccer player presenting with a one-year history of posterior ankle pain due to posterior ankle impingement syndrome secondary to an elongated Stieda process and loose bodies. Preoperative review of MRI revealed an anatomical variation of the posterior neurovascular bundle in relation to the flexor hallucis longus (FHL) tendon at the level of the ankle joint. In this case, the neurovascular bundle was situated posterior and lateral to the FHL tendon, as opposed to medial to the FHL tendon, as seen in typical anatomy. This finding is signifi

Ultrasound-Guided Peripheral Nerve Blocks for Percutaneous Treatments of Common Tendinopathies.

OBJECTIVES: Ultrasound-guided percutaneous needle fenestration (UPNF) and percutaneous needle tenotomy (UPNT) are minimally invasive procedures commonly used to treat chronic tendinopathies. However, these techniques can be painful, potentially limiting patient tolerance, satisfaction, and procedural feasibility. Although local anesthetics (LAs) provide analgesia, their potential inhibitory effects on tenocyte activity and the efficacy of orthobiologics raise concerns regarding their impact on tendon healing. The objectives of this study is to propose a practical, anatomy-based approach for th

Ollier's Disease Regenerate Healing Rates: Is there Cause for Concern?

BACKGROUND: Ollier's disease, characterised by multiple enchondromas, can result in significant limb length discrepancy and angular deformities. Anecdotal evidence suggests faster healing rates and a risk of premature consolidation during lengthening. The aim is to evaluate healing rates in Ollier's and assess the need for change in practice. MATERIALS AND METHODS: A total of 12 patients with Ollier's disease underwent limb lengthening utilising an external fixator or intramedullary lengthening nail. A latency period of 6 days was followed by a lengthening rate of 1 mm per day. Parameters, in

Sciatic and Posterior Femoral Cutaneous Nerve Anomalies and Their Clinical Implications for Sciatica and Piriformis Syndrome: A Cadaveric Case Report.

The sciatic nerve (SN), the largest nerve in the human body, normally exits the pelvis inferior to the piriformis muscle (PM) before dividing into its two terminal branches, the tibial nerve (TN) and the common fibular nerve (CFN). Variations in the anatomy of the SN and the posterior femoral cutaneous nerve (PFCN) may occur during embryologic development and can have important clinical implications, including piriformis syndrome, sciatica, and increased susceptibility to iatrogenic injury. This case report describes a rare concurrent variation of both the SN and PFCN identified during routine

Impaired gastrocnemius medialis muscle-tendon decoupling reduces gait efficiency in spastic paresis syndrome.

INTRODUCTION: Muscle-tendon unit interactions are crucial for efficient walking but remain unstudied in adult-onset spastic paresis. This cross-sectional study evaluated the relative contributions of the Achilles tendon, muscle fascicles, and muscle belly to the dynamics of the gastrocnemius medialis during gait in individuals with stroke and incomplete spinal cord injury, focusing on Equinus and Non-equinus patterns. METHODS: Sixty participants (20 with stroke, 18 with incomplete spinal cord injury, 22 healthy controls) underwent 3-dimensional gait analysis synchronized with ultrasound. Part

[Tibial nerve somatosensory evoked potentials localize the responsible level in tandem spinal stenosis: a case report].

An 84-year-old man had experienced unsteadiness while walking and coldness in both lower limbs for about 18 months, with gradual progression. Neurological examination revealed decreased tendon reflexes in the lower extremities, a positive Romberg sign, and a wide-based gait. MRI demonstrated spinal canal stenosis in the cervical and lumbar regions. Because concomitant peripheral neuropathy was suspected, electrophysiological studies were performed. Nerve conduction studies showed no evidence of peripheral neuropathy. Tibial nerve somatosensory evoked potentials (SEP) revealed prolonged N21-P38

Allograft Strut Fixation to Humeral Bone: A Biomechanical Comparison of Suture-Tape Cerclage, Wire Cerclage, and Screws.

BACKGROUND: Shoulder arthroplasty is increasingly utilized to treat shoulder arthritis, cuff tear arthropathy and proximal humerus fractures. Onlay allograft cortical strut augmentation is a common technique used in the management of humeral bone loss, periprosthetic fractures, and during revision surgeries to enhance stability and potentially reconstitute bone. This study investigated the biomechanical performance of three commonly used strut fixation methods used to secure an allograft strut to host diaphyseal bone: suture-tape cerclage, wire cerclage and lag screw fixation. METHODS: Eight

Targeting A3 adenosine receptor signaling to enhance analgesia in nonresponders to spinal cord stimulation in male nerve-injured rats.

Spinal cord stimulation (SCS) is a clinically approved therapy for neuropathic pain (NP), yet a substantial subset of patients fails to respond or shows minimal efficacy, so-called nonresponders. It remains unclear how to resolve this SCS nonresponsiveness. Using a NP model induced by tibial-sparing nerve injury (SNI-t) in male rats, we investigated whether targeting the A3 adenosine receptor (A3AR), which is enriched in microglial cells, could convert nonresponders into responders. The dose-response relationship of 2-Cl-IB-MECA (a highly selective A3AR agonist) to inhibit mechanical hypersens

Multiparametric Quantitative MRI of Peripheral Nerves to Differentiate Demyelinating From Axonal Polyneuropathies.

BACKGROUND: Differentiating demyelinating from axonal polyneuropathies helps for accurate diagnosis and treatment. However, the current clinical tools lack sensitivity to confirm demyelination in peripheral nerves, particularly for proximal nerves. HYPOTHESIS: Multiparametric quantitative MRI (qMRI) of peripheral nerves can differentiate demyelination from axonal loss. STUDY TYPE: Retrospective. SUBJECTS: Twenty patients with Charcot-Marie-Tooth type-1 (CMT1, demyelinating, females = 12), 12 patients with CMT2 (axonal, females = 6), 25 patients with hereditary neuropathy with liability to p

Targeted muscle reinnervation surgery modulates apoptosis and synaptic plasticity to improve motor function following tibial nerve injury in rats.

BACKGROUND: Targeted muscle reinnervation (TMR) is represents an advanced neural-machine interface that enhances prosthetic control and facilitates motor recovery in amputees. Although TMR is known to connect residual nerve fibers and supply neurotrophic factors, its impact on spinal cord motor neurons remains understudied. This study investigated the effects and possible mechanisms of TMR on spinal motor neurons in a rat model of tibial nerve transection (TNT). MATERIALS AND METHODS: There were 30 Sprague Dawley rats grouped into control, TNT, and TMR groups. TMR was grafted proximal tibial

Conservative Safety Assessment of a Nitinol Microhook Coaptation Device on Intact Nerve.

INTRODUCTION/AIMS: Nerve Tape is a novel nerve coaptation device that eliminates the technical challenges of microsuturing. The device incorporates tissue-engaging Nitinol microhooks within a flexible backing of decellularized porcine small intestine submucosa (SIS). Preclinical nerve injury models have demonstrated device safety and effective promotion of regeneration and recovery. Here, we further assess the impact of the device's components on nerve tissue and function, using a highly conservative model in which the device is applied to intact nerves. METHODS: Nerve Tape, sutures, or SIS w

Double Cross-Leg Reverse Sural Flap for Coverage of an Extensive Single-Leg Soft-Tissue Defect: A Case Report.

Extensive soft-tissue defects of the distal lower extremity following high-energy trauma represent a major reconstructive challenge, particularly when involving the heel and ankle. The reverse sural flap is a well-established option for distal leg and foot coverage; however, its dimensions are traditionally limited, and large defects often require free tissue transfer. We report the case of a 23-year-old male who sustained a high-speed motorcycle accident resulting in an extensive right heel and ankle soft-tissue defect with exposed calcaneus, associated with calcaneal fracture, subtalar dislo

The Specialized Proresolving Mediator Maresin 1 Alleviated Neuropathic Pain Exacerbated by High-Fat Diet via Regulating Neuroinflammation.

BACKGROUND: Clinical observations have suggested an association between a high-fat diet (HFD) and neuropathic pain (NP). Studies also revealed that maresin 1 (MaR1) can regulate macrophage phagocytosis, modulate tissue repair and inflammatory resolution, and influence the progression of NP. However, the specific regulatory mechanisms of MaR1 in HFD-induced NP remain unclear. OBJECTIVE: The aim of this study is to explore the effect of MaR1 on neuroinflammation in mice with NP induced by an HFD. METHODS: A tibial nerve crush model with a HFD was used to establish HFD-induced chronic pain. The

Involvement of the reticulospinal system in the control of locomotion.

Human stepping movements are known to be co-ordinated by a neural coupling of upper and lower limbs. The aim of this study was to evaluate a potential involvement of the reticulospinal system in the interlimb co-ordination by neural coupling during locomotion. During stepping electrical stimuli (ES) were applied to the right tibial nerve to assess the mechanisms underlying neural coupling of lower limbs, and loud acoustic stimuli (LAS) were released, known to activate the reticulospinal system. EMG activity of the tibialis anterior (TA) and gastrocnemius medialis (GM) of both sides was recorde

Characterization of sensory nerve conduction abnormalities at the time of ALS diagnosis.

OBJECTIVES: Although sensory nerve abnormalities have been reported in amyotrophic lateral sclerosis (ALS), their distribution at diagnosis, relative to motor involvement and clinical onset phenotype remains incompletely characterized. We aimed to systematically compare sensory and motor nerve conduction abnormalities at ALS diagnosis and determine whether sensory involvement follows an onset-dependent pattern similar to motor dysfunction. METHODS: In this prospective cross-sectional study, 40 newly diagnosed ALS patients enrolled in the Iran University ALS Registry (March 2022-March 2023) un

Analgesic Effects of Novel Spinal Cord Stimulation Waveforms in Rats With Painful Tibial Nerve Injury.

OBJECTIVES: Neuropathic pain is associated with hyperexcitability of spinal cord dorsal horn neurons. Spinal cord stimulation (SCS) can reduce this hyperexcitability and provide analgesia. We evaluated the effects of two novel SCS waveforms, designed with long pulse widths and short interpulse intervals with slowly varying amplitudes, on neuropathic pain behaviors and dorsal horn neuronal activity in a rat model of tibial nerve injury (TNI). MATERIALS AND METHODS: Rats underwent TNI followed by implantation of epidural stimulation leads. Novel waveforms, specifically stochastic amplitude modu
