OBJECTIVE: Various osteotomy techniques have been explored for exposing the posterolateral tibial plateau in previous studies. However, these methods are often complex, cause significant damage to normal anatomical structures, compromise knee joint stability, and pose risks to knee function, thus limiting their clinical application. This study proposes a new fibular head osteotomy technique for treating posterolateral tibial plateau fractures, aiming to achieve favorable surgical outcomes.
METHODS: Thirteen patients who underwent surgical treatment for posterolateral tibial plateau fractures

BACKGROUND: Peripheral nerve injuries in the lower extremities impose significant functional and quality-of-life challenges. Loss of plantar flexion is a major deficit. Novel nerve transfers are being introduced to restore function. A novel nerve transfer-partial superficial peroneal to a gastrocnemius branch of the tibial nerve-aimed at restoring plantar flexion is presented in a patient with an incomplete proximal thigh-level sciatic nerve injury.
OBSERVATIONS: The case of a 27-year-old policeman with a right sciatic nerve injury from a gunshot wound sustained 11 months earlier, characteriz

BACKGROUND: Anesthetic injection through the central or posterolateral knee capsule for analgesia after knee surgery risks popliteal vessel and tibial and peroneal nerve injury. We evaluated the distribution of a high volume of fluid injected through only the posteromedial capsule and compared it to a technique involving injections through the posteromedial and posterolateral capsules.
METHODS: Four fresh cadaveric knees were injected with 50 ml of Omnipaque 350 diluted 1:4 with normal saline through the posteromedial knee capsule. In four other specimens, we followed a published protocol by

Nerve regeneration is associated with the plasticity of sensory neurons such that even muscle afferents directed to the skin form mechanosensitive receptive fields appropriate for the new target. STOML3 is an essential mechanotransduction component in many cutaneous mechanoreceptors. Here, we asked whether STOML3 is required for functional and anatomical plasticity following peripheral nerve regeneration. We used a cross-anastomosis model adapted to the mouse, in which the medial gastrocnemius nerve was redirected to innervate hairy skin previously occupied by the sural nerve. We recorded from

BACKGROUNDS: The gastrocnemius recession was originally introduced as a method of treating isolated gastrocnemius contractures among pediatric population, however currently this procedure allows to treat various foot pathologies. Different modifications of open technique has been developed over the years and recently endoscopic approach become a widespread technique among many surgeons. It provides smaller incision and shorter operative time, however might be associated with the higher risk of sural nerve damage as a result of poor visualization. The aim of this study was to evaluate two diffe

BACKGROUND: The current standard of care for surgically managed tibia shaft fractures is intramedullary nailing. Chronic postoperative knee pain is the most common complication of the infrapatellar technique in intramedullary nailing. Literature suggests it may be related to infrapatellar nerve injury.
OBJECTIVE: This study compares the incidence of anterior knee pain after tibia nail insertion between conventional longitudinal and transverse incisions.
METHODS: This clinical trial study was conducted in our Level I trauma center from February 2022 to August 2024. Patients with closed diaphy

Growing evidence highlights the neuroprotective potential of progesterone in mitigating various forms of nervous system injury. In previous research, we demonstrated that progesterone ameliorates both electrophysiological and behavioral deficits associated with chronic constriction injury (CCI) of the sciatic nerve in rats. However, the precise mechanisms underlying these effects remain poorly understood. This study aimed to elucidate the involvement of GABA-A receptors in mediating the therapeutic effects of progesterone on nociceptive behaviors, specifically thermal hyperalgesia and mechanic

BACKGROUND: Nerve injury is the most common complication following foot and ankle surgery, with painful neuroma reported in up to 10% of procedures. Current treatment often yields varying degrees of pain relief. Electrical stimulation (ES) through peripheral neuromodulation is an emerging technology associated with improvement in nerverelated pain and acceleration of neural regeneration. This study assessed the short-term outcomes of combining nerve reconstruction techniques with ES in providing early pain relief for patients with symptomatic lower extremity neuromas.
METHODS: We describe a s

Despite the long history of experience in peripheral nerve reconstruction, dating back to the 1870s, recent decades have seen an explosion of innovation. Advancements have been made in primary repair, secondary repair, nerve grafts, tendon transfers, and, most recently, nerve transfers. These innovations address the distance-to-target problem faced by surgeons and provide improved functional outcomes. Key principles for successful nerve transfers include proper patient selection, preoperative discussions, intraoperative decisions, and postoperative management. In this review, we discuss 3 comm

INTRODUCTION: Medial Tibial Stress Syndrome (MTSS) is a painful overuse stress injury that plagues many athletes in the secondary school population. MTSS affects up to 20 % of runners and females more than their male counterparts. Best practice standards for the treatment of MTSS do not exist in current literature. The MyoKinethetic system (MYK) is a global treatment paradigm that targets the nerve root responsible for compensations. The objective of this study is to evaluate the effect of MYK treatment on pain and function in the high school population with MTSS symptoms.
METHOD: A multiple-

Neuroma following nerve injury and/or amputation is a debilitating condition with significant impacts on quality of life. Several approaches exist to prevent or treat neuroma and/or reduce associated pain; however, these approaches are not consistently effective, facile, or widely accessible. The present study characterizes a xenogeneic nerve cap graft device (NCGD) composed of decellularized porcine nerve. The NCGD was assessed for its ability to inhibit nerve growth, neuroma formation, and pain in rodent models of sciatic neurectomy and tibial neuroma transposition. The NCGD provided a neuro

BACKGROUND: The repair technology of peripheral nerve injuries has made great progress, but the simultaneous repair and promotion of nerve regeneration in multiple distal nerves remains a challenging task. The current cylindrical nerve conduits are unsuitable for nerve transposition repair. This study aims to assess the effect of conical chitosan conduits (different inner diameters at both ends) on nerve transposition repair, in conjunction with methylcobalamin (MeCbl).
METHODS: In this study, a conical chitosan conduit was used to bridge a 2 mm defect between the proximal common peroneal ner

Operant conditioning of the spinal stretch reflex or its electrical analog, the H-reflex, induces plasticity in the brain and spinal cord that increases (up-conditioning) or decreases (down-conditioning) the reflex elicited by primary afferent input to the spinal motoneuron. In rats in which the sciatic nerve is transected and repaired, soleus (SOL) H-reflex up-conditioning during regeneration strengthens primary afferent reinnervation of SOL motoneurons and improves recovery of the SOL H-reflex. This suggests that H-reflex up-conditioning could improve functional recovery after nerve injury a

While much has been written about the persistence and symptoms of Complex Regional Pain syndrome, the role of peripheral nerve surgery as a treatment remains overlooked, especially in the lower extremity of the pediatric population. The purpose of this report is to describe the application of lower extremity peripheral nerve decompression to address specific injuries and entrapments, to provide meaningful relief and functional recovery in a pediatric patient with CRPS. A 14-year-old male presented with severe CRPS after several traumatic events involving the left lower extremity. After seeing
