Effects of Aerobic and Interval Training on Peripheral Nerve Recovery in Male and Female Diabetic Rats.

Background Diabetes mellitus negatively affects the peripheral nervous system, often leading to diabetic neuropathy (DN), which impairs nerve regeneration. Although exercise therapy promotes peripheral nerve regeneration, its effects under diabetic conditions remain unclear. The aim of this study was to investigate the effects of aerobic exercise and interval training on nerve regeneration following tibial nerve crush injury in male and female type 1 diabetic and control rats. Methods Type 1 diabetes mellitus was induced in Wistar rats via intraperitoneal injection of streptozotocin (50 mg/kg)

An exploratory study of Type B variation of the sciatic nerve.

BACKGROUND: Sciatic nerve gives off branches that supply the back of the thigh, leg, and foot. Classically, this nerve emerges from the greater sciatic foramen below the piriformis muscle and subsequently divides into the common fibular and tibial nerves in the distal third of the posterior thigh. However, the course of the sciatic nerve varies among individuals, potentially resulting in nerve compressions. Understanding these variations helps prevent injuries during diagnostic and therapeutic procedures. This study examined the branching patterns of this nerve in the Scottish cadavers using t

Extracting robust single-trial somatosensory evoked potentials for non-invasive brain computer interfaces.

Objective.Reliable extraction of single-trial somatosensory evoked potentials (SEPs) is essential for developing brain-computer interface (BCI) applications to support rehabilitation after brain injury. For real-time feedback, these responses must be extracted prospectively on every trial, with minimal post-processing and artifact correction. However, noninvasive SEPs elicited by electrical stimulation at recommended parameter settings (0.1-0.2 msec pulse width, stimulation at or below motor threshold, 2-5 Hz frequency) are typically small and variable, often requiring averaging across multipl

Acute Effect of Percutaneous Tibial Nerve Stimulation on Postural Control: A Randomized Controlled Trial.

INTRODUCTION: Ankle sprains are common severe injuries leading to changes in muscular activity and postural control impairments. Postural control integrates neural components, including cognitive processing. Due to its relevance, neuromodulation techniques such as percutaneous tibial nerve stimulation (PTNS) have gained attention for improving balance, proprioception, and stability. However, the effects of PTNS on postural control remain insufficiently investigated. This study evaluated the acute effects of PTNS on postural control in healthy individuals. METHODS: Ninety-four healthy particip

Editorial Commentary: Remnant-Preserving Anterior Cruciate Ligament Reconstruction Shows Long-Term Advantages, but Patient Selection, Characterization of Remnant Tissue, and Efficacy in Patients at Risk of Postoperative Knee Instability Are Areas for Future Investigation.

Remnant preservation is one specific anterior cruciate ligament (ACL) reconstruction technique that has been explored to improve outcomes and involves retaining, as opposed to debriding, the "remnant" or native ACL tibial stump at the time of reconstruction. The ACL remnant tissue contains important mechanoreceptors and nerve endings, and preservation of this tissue is thought to enhance biological healing within the bone tunnels, graft ligamentization, maturation, synovialization, and proprioception, as well as reduce the risk of tunnel enlargement. Remnant preservation may also offer several

Cadaveric Study on the Anatomical Variations in the Branching Pattern of the Tibial Nerve to the Deep Muscles of the Posterior Compartment of the Leg.

OBJECTIVES: This study aimed to classify and assess the motor branching patterns of the tibial nerve (TN) to the flexor digitorum longus (FDL), tibialis posterior (TP), and flexor hallucis longus (FHL) muscles. METHODOLOGY: This observational cadaveric study examined the motor branching patterns of the TN by dissecting 60 adult lower limbs preserved in formalin. Standard dissection techniques exposed the TN from its origin at the apex of the popliteal fossa, where it arises from the sciatic nerve, to its bifurcation into the medial and lateral plantar nerves within the tarsal tunnel near the

A magnetic resonance imaging safe distance beyond the flexor hallucis longus tendon and the application of the safe arthroscopic approach to prevent neurovascular injury in posterior ankle arthroscopy.

BACKGROUND: The flexor hallucis longus (FHL) tendon is used as a medial landmark for posterior ankle arthroscopy. However, some posterior ankle pathologies require working medial to the FHL. The safe distance beyond the FHL to the neurovascular bundle has never been established. PURPOSE: To report the safe distances beyond the FHL and neurovascular bundle and the incidence of neurovascular injury after using "safe arthroscopic approach". METHODS: The 63 MRIs with posterior ankle pathologies were included. Measurements were the FHL-tibial nerve (TN) and FHL-posterior tibial artery (PTA) dista

Deletion of murine Sarm1 results in a microenvironment that delays peripheral nerve regeneration after injury.

Upon injury to the mammalian peripheral nervous system (PNS), severed axons undergo rapid SARM1-dependent programmed axon death (Wallerian degeneration), but a potential role for Sarm1 in PNS regeneration remains unclear. We show that in mouse dorsal root ganglia with their axons cut, Sarm1 delayed the activation of injury-induced transcriptional programs associated with axon outgrowth and immune function. After sciatic nerve crush in Sarm1-/- mice, axons rapidly extended through the nerve injury site, but growth stalled more distally. Slow axon regeneration in the distal nerve was accompanied

Accumulation of lipids after acute direct and indirect traumatic injuries in male and female mice.

BACKGROUND: Adipose infiltration and lipid droplet accumulation are implicated in metabolic diseases and are known to limit skeletal muscle repair and regeneration. However, their role in skeletal muscle injury, particularly volumetric muscle loss (VML), remains unclear. We aimed to characterize early lipid and adipokine responses following direct (i.e., VML) and indirect (i.e., denervation) traumatic injuries. METHODS: Adult male and female C57Bl/6J mice (n = 36) were randomized to VML injury to the posterior hindlimb, tibial nerve denervation, or remained injury Naïve. Three days post, in v

No clinical differences between outside-in and all-inside techniques in medial meniscus repair: emphasizing Minimal Clinically Important Difference (MCID) -based outcomes.

BACKGROUND: While the all-inside (AI) technique is widely preferred for its simplicity and efficiency, the outside-in (OI) method is less commonly used. Despite both techniques being effective, direct comparisons of their functional outcomes are limited. This study aims to compare the clinical efficacy of OI and AI repair techniques for zone-3 and zone-4 medial meniscus tears, emphasizing outcomes based on the Minimal Clinically Important Difference (MCID).We hypothesized that there would be no significant difference in outcomes between the two techniques. METHODS: A retrospective cohort stud

Sural Nerve Grafting in Peripheral Nerve Repair: The Expanding Role of Ultrasonography.

The sural nerve grafting plays a crucial role in the repair of peripheral nerve impairments when tension-free neurorrhaphy is unfeasible. The sural nerve is frequently utilized in sensory and mixed nerve repairs because of its anatomical reliability, sensory characteristics, and low donor site morbidity. This article critically reviews the current knowledge in the field and examines the anatomical and histological features of the sural nerve, along with contemporary harvesting techniques and clinical applications. High-resolution ultrasound (US) is highlighted as a non-invasive, real-time inst

Activity of protein kinase C controls the efficacy of cannabinoid receptor type 1 in the medial prefrontal cortex after neuropathic pain.

Many patients with chronic pain conditions suffer from depression. In our previous report, we found that afferent noxious inputs after painful nerve injury compromise activity-dependent endocannabinoid (eCB) signaling in the medial prefrontal cortex (mPFC), resulting in depression. Since protein kinase C (PKC) and cAMP/protein kinase A (PKA) pathways can regulate eCB receptor type-1 (CB1R)-mediated synaptic transmission, we explored the possible roles of PKC and PKA in causing reduced eCB signaling in neuropathic pain (spared nerve injury, SNI). Four weeks after SNI, rats developed both depres
