Macroscopic comparison of open Metzenbaum and ultrasound-guided fasciotomy techniques for surgical treatment of the hindlimb proximal suspensory ligament desmopathy in horses: A cadaveric anatomical study.

OBJECTIVE: To macroscopically characterize and compare the open Metzenbaum (FOM) and ultrasound-guided (FUG) plantar fasciotomy techniques for decompression of the hindlimb proximal suspensory ligament (PSL), and to determine whether either approach induces intraligamentous splitting (ILS). STUDY DESIGN: Cadaveric experimental study. SAMPLE POPULATION: Paired hindlimbs from 10 adult horses with no history of hindlimb lameness. METHODS: Each horse contributed one limb to FOM and the contralateral to FUG. All procedures were performed by a single surgeon, and incision measurements were obtain

[Translated article] Mid- to long-term radiological and functional outcomes after calcaneoplasty with Achilles tendon reinsertion for insertional tendinopathy.

BACKGROUND: Insertional Achilles tendinopathy (IAT) significantly impairs function and quality of life, particularly in young athletes. When conservative treatment fails, surgical options include Zadek osteotomy and calcaneoplasty with tendon reinsertion. Although certain preoperative radiological parameters, such as calcaneal inclination and tuberosity length, may influence functional outcomes, comprehensive studies analyzing their impact on calcaneoplasty results remain limited. OBJECTIVES: To evaluate the mid- and long-term effectiveness of calcaneoplasty with Achilles tendon reinsertion i

Defining Potential Neurovascular Risk Zones in Superficial Plantar-Medial Release: An Anatomical Study.

Background: The superficial plantar-medial release (S-PMR) refers to a group of surgical procedures involving the release of the plantar aponeurosis and adjacent medial plantar soft tissues that are used in selected cases of plantar fasciitis and cavovarus foot deformity. The procedure aims to address pain and contracture of the plantar aponeurosis and intrinsic foot muscles, which may contribute to pathological foot alignment and gait instability. Due to the close proximity of highly variable neurovascular structures in the plantar region, precise anatomical knowledge and a patient-specific,

Sensory Nerves at Risk During Hallux Valgus Surgery: Anatomical Insights and Preventive Strategies-A Systematic Review.

BACKGROUND: Hallux valgus is a common forefoot deformity. Sensory complications after correction can result from iatrogenic injury to the medial branch of the medial dorsal cutaneous nerve (MDCNm), lateral dorsal cutaneous nerve (LDCN), medial plantar cutaneous nerve (MPCN), and lateral plantar cutaneous nerve (LPCN). We synthesized evidence and landmarks to aid nerve identification and reduce injury. METHODS: Following PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines, PubMed, Embase, and Cochrane were searched to February 2025 using 'nerve anatomy', 'hal

Ultrasound-Guided Peripheral Nerve Blocks for Percutaneous Treatments of Common Tendinopathies.

OBJECTIVES: Ultrasound-guided percutaneous needle fenestration (UPNF) and percutaneous needle tenotomy (UPNT) are minimally invasive procedures commonly used to treat chronic tendinopathies. However, these techniques can be painful, potentially limiting patient tolerance, satisfaction, and procedural feasibility. Although local anesthetics (LAs) provide analgesia, their potential inhibitory effects on tenocyte activity and the efficacy of orthobiologics raise concerns regarding their impact on tendon healing. The objectives of this study is to propose a practical, anatomy-based approach for th

Peripheral Nerve Entrapment Mimics of Overuse Syndromes: A Symptom-First Diagnostic Framework.

Repetitive use injuries are commonly seen by musculoskeletal providers, usually in the form of tendinopathies. However, focal peripheral nerve entrapment can mimic or coexist with tendon pathology, particularly when weakness, sensory symptoms, or imaging findings are subtle or nonspecific. This review provides a practical, symptom-first framework to help clinicians distinguish tendon-dominant, nerve-dominant, and mixed presentations in clinical practice. Five clinically relevant tendon-nerve mimic pairs are emphasized: (1) rotator cuff tendinopathy versus suprascapular neuropathy; (2) lateral

Increased Lateral Calcaneal Body and Subtalar Joint Visualization Utilizing a Modified Sinus Tarsi Approach.

BACKGROUND: The traditional sinus tarsi approach (TST) for open reduction and internal fixation (ORIF) of calcaneus fractures has gained popularity because of adequate fracture exposure with lower wound complication rates. Visualization, as compared with more extensile approaches, can be limited, however. This study introduces a modified sinus tarsi approach (MST) that extends the TST incision approximately 2 cm posterodorsally as well as 2 cm anteroplantarly, to enhance exposure. The aim of this study is to quantify the amount of exposure obtained with the traditional sinus tarsi approach and

Superficial peroneal to tibial nerve transfer for restoring plantar flexion in partial sciatic nerve injuries: illustrative case.

BACKGROUND: Peripheral nerve injuries in the lower extremities impose significant functional and quality-of-life challenges. Loss of plantar flexion is a major deficit. Novel nerve transfers are being introduced to restore function. A novel nerve transfer-partial superficial peroneal to a gastrocnemius branch of the tibial nerve-aimed at restoring plantar flexion is presented in a patient with an incomplete proximal thigh-level sciatic nerve injury. OBSERVATIONS: The case of a 27-year-old policeman with a right sciatic nerve injury from a gunshot wound sustained 11 months earlier, characteriz

Cadaveric Study on the Anatomical Variations in the Branching Pattern of the Tibial Nerve to the Deep Muscles of the Posterior Compartment of the Leg.

OBJECTIVES: This study aimed to classify and assess the motor branching patterns of the tibial nerve (TN) to the flexor digitorum longus (FDL), tibialis posterior (TP), and flexor hallucis longus (FHL) muscles. METHODOLOGY: This observational cadaveric study examined the motor branching patterns of the TN by dissecting 60 adult lower limbs preserved in formalin. Standard dissection techniques exposed the TN from its origin at the apex of the popliteal fossa, where it arises from the sciatic nerve, to its bifurcation into the medial and lateral plantar nerves within the tarsal tunnel near the

Enhancing surgical safety in lateral calcaneal plating: a cadaveric study on screw placement and fluoroscopic optimisation.

BACKGROUND: Screw placement in lateral plating of calcaneal fractures presents a challenge due to the proximity of medial neurovascular structures. This study aims to identify high-risk zones, determine appropriate screw lengths, and define optimal fluoroscopic angles to enhance intraoperative visualisation and improve surgical safety. METHODS: Thirty-three fresh-frozen cadaveric specimens were used. Following a lateral extensile approach, locking screws were inserted through an anatomical plate into seven predefined zones using sleeve-guided drilling, without plate bending or freehand angula

Safety and Feasibility of the Plantar Portal Technique in the Surgical Management of Plantar Fasciitis: A Cadaveric Study.

INTRODUCTION: This study investigates the proximity of the portals and nearby motor and sensory nerves in endoscopic release of the plantar fascia and the safety and efficacy of the addition of a novel third (plantar) portal to improve access to the calcaneal spur. METHODS: Nine fresh-frozen lower extremity cadavers were examined in this study. In addition to the conventional medial and lateral portals for endoscopic plantar fascia release, a third (plantar) portal was utilized to debride the calcaneal spur area. The fascia was released with the use of an obturator cannula. Each cadaver was c

The Unusual Suspect-Tarsal Tunnel Syndrome Due to Flexor Digitorum Brevis Hypertrophy.

A rare case of tarsal tunnel syndrome caused by flexor digitorum brevis hypertrophy. The patient presented with medial plantar pain and paresthesia but denies any injury. Magnetic resonance imaging revealed a marked hypertrophy of the flexor digitorum brevis in close proximity to the medial and lateral plantar nerves, with herniation into the porta pedis. Surgical excision of the herniated muscle was performed, along with a tarsal tunnel release. The combined treatment resulted in symptomatic relief.

Nociceptor spontaneous activity is responsible for fragmenting non-rapid eye movement sleep in mouse models of neuropathic pain.

Spontaneous pain, a major complaint of patients with neuropathic pain, has eluded study because there is no reliable marker in either preclinical models or clinical studies. Here, we performed a comprehensive electroencephalogram/electromyogram analysis of sleep in several mouse models of chronic pain: neuropathic (spared nerve injury and chronic constriction injury), inflammatory (Freund's complete adjuvant and carrageenan, plantar incision) and chemical pain (capsaicin). We find that peripheral axonal injury drives fragmentation of sleep by increasing brief arousals from non-rapid eye moveme

Timing of surgical intervention in peripheral nerve injuries from gunshot wounds: Management and review of the literature.

BACKGROUND: Gunshot wounds (GSWs) can result in various peripheral nerve injuries (PNIs), ranging from direct nerve transection to neuropraxia caused by the ballistic shockwave mechanism. PNIs from GSWs can be treated with either early or delayed intervention, with the literature supporting both approaches and sparking a debate between early and delayed intervention for PNIs from GSWs. Here, we present a case that underwent delayed exploration of the right common peroneal nerve after GSW and a literature review comparing early versus delayed intervention for PNIs from GSWs.
