Inhibition of SARM1 Reduces Neuropathic Pain in a Spared Nerve Injury Rodent Model.

INTRODUCTION/AIMS: The function of the sterile alpha and toll/interleukin receptor motif-containing protein 1 (SARM1) in neuropathic pain development has not yet been established. This protein has a central role in regulating axon degeneration and its depletion delays this process. This study aims to demonstrate the effects of SARM1 deletion on the development of neuropathic pain. METHODS: Thirty-two wild-type (WT) or SARM1 knockout (KO) rats underwent spared nerve injury (SNI) or sham surgery. Mechanical allodynia was assessed by electronic Von Frey and cold hyperalgesia by the acetone test.

Intra-septal sensory branch as an alternative to the sural nerve grafting in radial nerve reconstruction: Anatomical and histomorphological study.

INTRODUCTION: Nerve grafting with the sural nerve is a standard treatment method for radial nerve injury that requires another incision at the lateral ankle distal from the injured upper limb. The aim of this study was to investigate the common trunk (CTCB) of the inferior lateral brachial cutaneous nerve (ILBCN) and posterior antebrachial cutaneous nerve (PACN) as a possible donor inside the lateral intermuscular septum. MATERIALS AND METHODS: The arms and legs of 8 formalin-embalmed cadaver specimens were studied. The radial nerve, common trunk of the ILBCN and PACN, and the sural nerve wer

The Recurrence of Painful Neuromas of the Limbs Following TMR.

Background/Objectives: Neuropathic pain associated with neuromas is a complex clinical problem to treat. Targeted Muscle Reinnervation (TMR) has been demonstrated to treat pain both as a prophylactic procedure in amputated patients and in patients affected by painful neuromas. It is not clear what its role could be in chronic situations: the literature reports amazing results but also unsuccessful pain relief. Methods: A retrospective analysis was conducted on patients treated with TMR for long-lasting painful neuromas in the upper and lower limbs. Following a clinical and instrumental diagnos

Femoral to sciatic nerve transfer: A cadaver study.

BACKGROUND: Proximal sciatic nerve injuries are a challenge to treat due to the limited options for donor nerves and the long distance needed for regeneration. METHODS: In our cadaveric study using five human cadavers, we aimed to evaluate the feasibility of transferring the tibial and common peroneal components of the sciatic nerve to the femoral nerve motor branches of the vastus medialis (VM) and vastus lateralis without the need for interposition nerve graft. The femoral nerve branches of the VM and lateralis were exposed anteriorly. The sciatic nerve was exposed posteriorly and passed th

Spatiotemporal activation of lumbar sensorimotor networks.

Spinal cord injury (SCI) research is primarily conducted using rodent models, which has resulted in significant advances, including novel treatment strategies that promote recovery. Unfortunately, many of these treatments do not have the same efficacy once translated to human clinical trials. Large animal models, such as Yucatan miniature pigs (minipigs), may provide a superior alternative to translating findings to human clinical trials due to their anatomical similarities to humans. However, porcine models are not widely used, which may be due in part to our inadequate understanding of the f

Selective RNAi silencing of Schwann cell Piezo1 alleviates mechanical hypersensitization following peripheral nerve injury.

The present study was designed to investigate the role of Schwann cell (SC) Piezo1 in peripheral nociception. We first developed an AAV vector that has primary SC tropism after delivery into the sciatic (or tibial) nerve. This was achieved by packing AAV-GFP transcribed by a CBA promoter using a capsid AAVolig001 to generate AAVolig001-CBA-GFP. Six weeks after intraneural injection of AAVolig001-CBA-GFP in naive rats, GFP expression was detected selectively in both myelinating SCs (mSCs) and non-myelinating SCs (nmSCs). A dual promoter and bidirectional AAV encoding a U6-driven short hairpin R

Optimizing Outcomes in Mangled Lower Extremity Reconstruction: Insights from a Retrospective Study of 93 Patients and Their Functional Scores.

Background/Objectives: Over the past 25 years, reconstructive techniques and patient management advancements have significantly improved outcomes in mangled lower extremity injuries. Functional results of limb salvage have been demonstrated to surpass those of primary amputations. Developments such as local fasciocutaneous flaps, vacuum-assisted closure, and hyperbaric oxygen therapy have enhanced the reconstructive ladder. Despite progress, the utility of the Mangled Extremity Severity Score (MESS) and Gustilo-Anderson classification remains debated, particularly in their prognostic value for

Frequency dependence of cortical somatosensory evoked response to peripheral nerve stimulation with controlled afferent excitation.

Objective.H-reflex targeted neuroplasticity (HrTNP) protocols comprise a promising rehabilitation approach to improve motor function after brain or spinal injury. In this operant conditioning protocol, concurrent measurement of cortical responses, such as somatosensory evoked potentials (SEPs), would be useful for examining supraspinal involvement and neuroplasticity mechanisms. To date, this potential has not been exploited. However, the stimulation parameters used in the HrTNP protocol deviate from the classically recommended settings for SEP measurements. Most notably, it demands a much lon

Fibular head osteotomy: A new technique for better exposure of postero-lateral tibial plateau fracture.

OBJECTIVE: Various osteotomy techniques have been explored for exposing the posterolateral tibial plateau in previous studies. However, these methods are often complex, cause significant damage to normal anatomical structures, compromise knee joint stability, and pose risks to knee function, thus limiting their clinical application. This study proposes a new fibular head osteotomy technique for treating posterolateral tibial plateau fractures, aiming to achieve favorable surgical outcomes. METHODS: Thirteen patients who underwent surgical treatment for posterolateral tibial plateau fractures

Superficial peroneal to tibial nerve transfer for restoring plantar flexion in partial sciatic nerve injuries: illustrative case.

BACKGROUND: Peripheral nerve injuries in the lower extremities impose significant functional and quality-of-life challenges. Loss of plantar flexion is a major deficit. Novel nerve transfers are being introduced to restore function. A novel nerve transfer-partial superficial peroneal to a gastrocnemius branch of the tibial nerve-aimed at restoring plantar flexion is presented in a patient with an incomplete proximal thigh-level sciatic nerve injury. OBSERVATIONS: The case of a 27-year-old policeman with a right sciatic nerve injury from a gunshot wound sustained 11 months earlier, characteriz

One High-Volume Posteromedial Knee Capsular Injection Can Provide Safe Anesthetic Infiltration Across the Entire Posterior Knee Capsule.

BACKGROUND: Anesthetic injection through the central or posterolateral knee capsule for analgesia after knee surgery risks popliteal vessel and tibial and peroneal nerve injury. We evaluated the distribution of a high volume of fluid injected through only the posteromedial capsule and compared it to a technique involving injections through the posteromedial and posterolateral capsules. METHODS: Four fresh cadaveric knees were injected with 50 ml of Omnipaque 350 diluted 1:4 with normal saline through the posteromedial knee capsule. In four other specimens, we followed a published protocol by

The mechanotransduction protein STOML3 is required for proprioceptor plasticity following peripheral nerve regeneration.

Nerve regeneration is associated with the plasticity of sensory neurons such that even muscle afferents directed to the skin form mechanosensitive receptive fields appropriate for the new target. STOML3 is an essential mechanotransduction component in many cutaneous mechanoreceptors. Here, we asked whether STOML3 is required for functional and anatomical plasticity following peripheral nerve regeneration. We used a cross-anastomosis model adapted to the mouse, in which the medial gastrocnemius nerve was redirected to innervate hairy skin previously occupied by the sural nerve. We recorded from

Sural nerve injury risk during endoscopic gastrocnemius recession: Comparison of two approaches.

BACKGROUNDS: The gastrocnemius recession was originally introduced as a method of treating isolated gastrocnemius contractures among pediatric population, however currently this procedure allows to treat various foot pathologies. Different modifications of open technique has been developed over the years and recently endoscopic approach become a widespread technique among many surgeons. It provides smaller incision and shorter operative time, however might be associated with the higher risk of sural nerve damage as a result of poor visualization. The aim of this study was to evaluate two diffe
