Comparison of transverse and longitudinal incisions in tibia intramedullary nailing: Implications for pain, numbness, and functional recovery.

BACKGROUND: The current standard of care for surgically managed tibia shaft fractures is intramedullary nailing. Chronic postoperative knee pain is the most common complication of the infrapatellar technique in intramedullary nailing. Literature suggests it may be related to infrapatellar nerve injury. OBJECTIVE: This study compares the incidence of anterior knee pain after tibia nail insertion between conventional longitudinal and transverse incisions. METHODS: This clinical trial study was conducted in our Level I trauma center from February 2022 to August 2024. Patients with closed diaphy

Antagonism of GABA-A receptor inhibits the effects of progesterone on nociceptive behaviors and electrophysiological alterations in a rat model of neuropathic pain.

Growing evidence highlights the neuroprotective potential of progesterone in mitigating various forms of nervous system injury. In previous research, we demonstrated that progesterone ameliorates both electrophysiological and behavioral deficits associated with chronic constriction injury (CCI) of the sciatic nerve in rats. However, the precise mechanisms underlying these effects remain poorly understood. This study aimed to elucidate the involvement of GABA-A receptors in mediating the therapeutic effects of progesterone on nociceptive behaviors, specifically thermal hyperalgesia and mechanic

Early Results of Optimized Nerve Management With Electrical Stimulation for Lower Extremity Neuroma.

BACKGROUND: Nerve injury is the most common complication following foot and ankle surgery, with painful neuroma reported in up to 10% of procedures. Current treatment often yields varying degrees of pain relief. Electrical stimulation (ES) through peripheral neuromodulation is an emerging technology associated with improvement in nerverelated pain and acceleration of neural regeneration. This study assessed the short-term outcomes of combining nerve reconstruction techniques with ES in providing early pain relief for patients with symptomatic lower extremity neuromas. METHODS: We describe a s

Peripheral Nerve Transfers: Core Principles and Workhorse Techniques.

Despite the long history of experience in peripheral nerve reconstruction, dating back to the 1870s, recent decades have seen an explosion of innovation. Advancements have been made in primary repair, secondary repair, nerve grafts, tendon transfers, and, most recently, nerve transfers. These innovations address the distance-to-target problem faced by surgeons and provide improved functional outcomes. Key principles for successful nerve transfers include proper patient selection, preoperative discussions, intraoperative decisions, and postoperative management. In this review, we discuss 3 comm

What are the effects of the MyoKinesthetic system on Medial Tibial Stress Syndrome in high school athletes?

INTRODUCTION: Medial Tibial Stress Syndrome (MTSS) is a painful overuse stress injury that plagues many athletes in the secondary school population. MTSS affects up to 20 % of runners and females more than their male counterparts. Best practice standards for the treatment of MTSS do not exist in current literature. The MyoKinethetic system (MYK) is a global treatment paradigm that targets the nerve root responsible for compensations. The objective of this study is to evaluate the effect of MYK treatment on pain and function in the high school population with MTSS symptoms. METHOD: A multiple-

Prevention of nerve growth and evoked pain with a nerve cap graft device.

Neuroma following nerve injury and/or amputation is a debilitating condition with significant impacts on quality of life. Several approaches exist to prevent or treat neuroma and/or reduce associated pain; however, these approaches are not consistently effective, facile, or widely accessible. The present study characterizes a xenogeneic nerve cap graft device (NCGD) composed of decellularized porcine nerve. The NCGD was assessed for its ability to inhibit nerve growth, neuroma formation, and pain in rodent models of sciatic neurectomy and tibial neuroma transposition. The NCGD provided a neuro

Conical Chitosan Conduits Combined With Methylcobalamin for Sciatic Nerve Transposition Repair.

BACKGROUND: The repair technology of peripheral nerve injuries has made great progress, but the simultaneous repair and promotion of nerve regeneration in multiple distal nerves remains a challenging task. The current cylindrical nerve conduits are unsuitable for nerve transposition repair. This study aims to assess the effect of conical chitosan conduits (different inner diameters at both ends) on nerve transposition repair, in conjunction with methylcobalamin (MeCbl). METHODS: In this study, a conical chitosan conduit was used to bridge a 2 mm defect between the proximal common peroneal ner

Soleus H-Reflex Up-Conditioning during Sciatic Nerve Regeneration in Rats Improves Recovery of Locomotion.

Operant conditioning of the spinal stretch reflex or its electrical analog, the H-reflex, induces plasticity in the brain and spinal cord that increases (up-conditioning) or decreases (down-conditioning) the reflex elicited by primary afferent input to the spinal motoneuron. In rats in which the sciatic nerve is transected and repaired, soleus (SOL) H-reflex up-conditioning during regeneration strengthens primary afferent reinnervation of SOL motoneurons and improves recovery of the SOL H-reflex. This suggests that H-reflex up-conditioning could improve functional recovery after nerve injury a

Effects of Aerobic and Interval Training on Peripheral Nerve Recovery in Male and Female Diabetic Rats.

Background Diabetes mellitus negatively affects the peripheral nervous system, often leading to diabetic neuropathy (DN), which impairs nerve regeneration. Although exercise therapy promotes peripheral nerve regeneration, its effects under diabetic conditions remain unclear. The aim of this study was to investigate the effects of aerobic exercise and interval training on nerve regeneration following tibial nerve crush injury in male and female type 1 diabetic and control rats. Methods Type 1 diabetes mellitus was induced in Wistar rats via intraperitoneal injection of streptozotocin (50 mg/kg)

An exploratory study of Type B variation of the sciatic nerve.

BACKGROUND: Sciatic nerve gives off branches that supply the back of the thigh, leg, and foot. Classically, this nerve emerges from the greater sciatic foramen below the piriformis muscle and subsequently divides into the common fibular and tibial nerves in the distal third of the posterior thigh. However, the course of the sciatic nerve varies among individuals, potentially resulting in nerve compressions. Understanding these variations helps prevent injuries during diagnostic and therapeutic procedures. This study examined the branching patterns of this nerve in the Scottish cadavers using t

Extracting robust single-trial somatosensory evoked potentials for non-invasive brain computer interfaces.

Objective.Reliable extraction of single-trial somatosensory evoked potentials (SEPs) is essential for developing brain-computer interface (BCI) applications to support rehabilitation after brain injury. For real-time feedback, these responses must be extracted prospectively on every trial, with minimal post-processing and artifact correction. However, noninvasive SEPs elicited by electrical stimulation at recommended parameter settings (0.1-0.2 msec pulse width, stimulation at or below motor threshold, 2-5 Hz frequency) are typically small and variable, often requiring averaging across multipl

Acute Effect of Percutaneous Tibial Nerve Stimulation on Postural Control: A Randomized Controlled Trial.

INTRODUCTION: Ankle sprains are common severe injuries leading to changes in muscular activity and postural control impairments. Postural control integrates neural components, including cognitive processing. Due to its relevance, neuromodulation techniques such as percutaneous tibial nerve stimulation (PTNS) have gained attention for improving balance, proprioception, and stability. However, the effects of PTNS on postural control remain insufficiently investigated. This study evaluated the acute effects of PTNS on postural control in healthy individuals. METHODS: Ninety-four healthy particip

Editorial Commentary: Remnant-Preserving Anterior Cruciate Ligament Reconstruction Shows Long-Term Advantages, but Patient Selection, Characterization of Remnant Tissue, and Efficacy in Patients at Risk of Postoperative Knee Instability Are Areas for Future Investigation.

Remnant preservation is one specific anterior cruciate ligament (ACL) reconstruction technique that has been explored to improve outcomes and involves retaining, as opposed to debriding, the "remnant" or native ACL tibial stump at the time of reconstruction. The ACL remnant tissue contains important mechanoreceptors and nerve endings, and preservation of this tissue is thought to enhance biological healing within the bone tunnels, graft ligamentization, maturation, synovialization, and proprioception, as well as reduce the risk of tunnel enlargement. Remnant preservation may also offer several
