Lung abscesses are localized collections of pus within the pulmonary parenchyma, usually arising from necrotizing infection and commonly occurring in patients with risk factors such as immunosuppression or aspiration. Typical complications include pleural effusion, empyema and bronchopleural fistula. Direct invasion of the chest wall by a lung abscess is an exceptionally rare complication, with most reported cases related to tuberculous empyema (empyema necessitans) rather than being related to the primary pulmonary infection. The authors report a case of a 42-year-old male patient with chroni

INTRODUCTION: The splenic tuberculous involvement is common than other intra-abdominal solid organs such as the liver, pancreas, and kidney. The aim of this article is to present and share a review of the English-language literature on splenic tuberculosis (TB) to gain a better understanding of etiopathogenesis, epidemiological features, and diagnostic methods and provide guidelines for its management and to present our experience of six cases.
METHODS: The systematic search of the literature was performed on PubMed and Medline from 1950 to 2019 according to the Preferred Reporting Items for

BACKGROUND: Globally, Tuberculosis (TB) is the leading infectious cause of death with more than 10 million cases and 1 million deaths from the disease annually. TB can present as a primary extrapulmonary infection without pulmonary symptoms or infection often mimicking other pathologies, thereby making its clinical manifestations uniquely difficult to diagnose in the Emergency Department.
OBJECTIVE: We present five patients with rare cases of TB in an effort to underscore the importance of the EM physician keeping extrapulmonary infection in their differential.
DISCUSSION: Extrapulmonary TB

Background Vertebral osteomyelitis (VO) is an uncommon but potentially debilitating spinal infection associated with delayed diagnosis, prolonged hospitalization, and significant morbidity. General international consensus indicates that the incidence of VO is increasing, which likely reflects an aging population and an increase in use of advanced imaging. This paper aims to describe the demographic characteristics, risk factors, microbiology, and short-term outcomes associated with VO in Malta. Methods We performed a retrospective single-centre cohort study of adult patients (>18 years old) di

BACKGROUND/AIMS: This study aims to evaluate the global burden of adverse effects of medical treatment (AEMT) using data from the Global Burden of Disease Study (GBD) 2021.
METHODS: Data were extracted from the GBD 2021, covering 204 countries/territories from 1990 to 2021. AEMT was defined using ICD-9 and ICD-10 codes, encompassing complications from medical procedures, treatments, or healthcare exposures. Estimates were categorized into fatal and non-fatal outcomes and stratified by age, sex, year, and covariates, including the Socio-demographic Index (SDI). Mortality-incidence ratios (MIRs

Tuberculosis (TB) is a major public health challenge, primarily affecting the lungs. However, it can disseminate to any organ system, including the cardiovascular system. Among cardiac structures, the pericardium is the most commonly affected, often leading to constrictive pericarditis, whereas myocarditis remains rare. The clinical presentation of cardiac TB is heterogeneous and is frequently associated with extra cardiac manifestations, complicating the diagnostic process, particularly in the absence of definitive microbiological confirmation. In this context, advanced multimodal imaging is

Coccidioidomycosis, a disease induced by the dimorphic fungi Coccidioides immitis and Coccidioides posadasii, is endemic to the southwestern United States and occurs with a myriad of clinical and imaging manifestations, which range from limited pulmonary disease to disseminated multiorgan infections. Humans are infected when arthroconidia in the soil are disturbed and dispersed into the air. Once inhaled, arthroconidia form spherules, which trigger a host cell-mediated immune response. If this response is adequate, it results in granuloma formation, halting the infection. However, an insuffici

BACKGROUND: Maudsley Family Therapy (MFT), and its manualised version, Family-Based Therapy (FBT), are the only well-established treatment interventions for adolescent anorexia nervosa (AN), with treatment efficacy primarily measured by improvements in eating behaviours and weight restoration. A crucial component of this therapy is an intensive home-based refeeding intervention that requires a substantial commitment from parents for up to one year. While this treatment works to restore weight in a proportion of adolescents, very little is known about its impacts on family distress, relationshi

BACKGROUND: People living with HIV and hospitalised with sepsis in Africa are at risk of death due to tuberculosis but diagnostics for tuberculosis might be delayed or inaccessible for people presenting for critical care. We aimed to compare the effects of immediate empirical and high-dose antituberculosis therapy on 28-day mortality in adult people living with HIV with sepsis in east Africa.
METHODS: ATLAS was a phase 3, open-label, randomised, controlled, 2 × 2 factorial, superiority trial conducted at four hospitals in Tanzania and Uganda. Participants were aged 18 years or older, living w

Primary thyroid tuberculosis is a rare form of extra-pulmonary tuberculosis, with only a few cases recorded in the literature, and none of which had thyroid dysfunction. A 6-year-old child came to us with complains of fever and neck swelling for one month. A neck examination indicated a widespread enlargement in the thyroid region, which was more noticeable on the left side. Thyroid function tests indicated elevated TSH (thyroid stimulating hormone) and low T3, T4, indicating hypothyroidism. Contrast-enhanced computed CT revealed enlarged thyroid glands with a collection inside the left lobe.

Cryptococcus neoformans is an environmental pathogenic yeast which causes both life-threatening infection and mild, localized infection. Cutaneous cryptococcosis is an uncommon event that usually results from haematogenous dissemination after a primary infection in the pulmonary or other organs, although primary cutaneous infection is possible. The lesions of cutaneous cryptococcosis are not pathognomonic, varying from nodules to ulcers and plaques, and thus can mimic other skin disorders. This case highlights that clinicians need to observe the skin lesions carefully and improve the relevant

Tuberculous meningitis (TBM) is a severe presentation of the Mycobacterium tuberculosis that can cause sensorineural hearing loss (SNHL) similarly to other etiologies of meningitis. The pathophysiology for this group remains unclear. We report a series with 3 temporal bones of infants who died from TBM, showing acute damage to the inner ear, such as fibrosis of the cochlea, organ of Corti atrophy, neuronal loss and internal auditory canal with abundant pus cells, predominantly mononuclear. The cochlear aqueduct, endolymphatic sac/duct and Rosenthal's canal appear to be routes of dissemination

Extrapulmonary tuberculosis (EPTB) is the clinical entity that is gaining more recognition. Still, it may emerge in various ways, making it difficult to diagnose. Even though pulmonary tuberculosis is the most common variety, EPTB may affect many organ systems and often mimics other diseases, therefore postponing diagnosis, and treatment. Five rare types of EPTB, including testicular tuberculosis, esophageal tuberculosis, psoas abscess with spinal involvement, tuberculous mastitis, and parotid gland tuberculosis, are the main subjects of this case series. All patients who were HIV-negative wer

Liposomal amphotericin B (AmBisome) is a lipid-associated formulation of the broad-spectrum polyene antifungal agent amphotericin B. It is active against clinically relevant yeasts and moulds, including Candida spp., Aspergillus spp. and filamentous moulds such as Zygomycetes, and is approved for the treatment of invasive fungal infections in many countries worldwide. It was developed to improve the tolerability profile of amphotericin B deoxycholate, which was for many decades considered the gold standard of antifungal treatment, despite being associated with infusion-related events and nephr

INTRODUCTION: Tuberculosis remains a major public health problem in developing countries. Isolated splenic tuberculosis is a rare form of extrapulmonary tuberculosis, with few cases documented in the literature, occurring mainly in immunocompromised individuals. The purpose of this article is to contribute to the medical knowledge regarding this rare disease and to highlight the diagnostic challenges and management approach.
CASE PRESENTATION: A 6-year-old immunocompetent child was admitted with febrile splenomegaly. Initial diagnostic evaluations, including imaging, suggested a possible diag

The overlap of Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS) and Stevens-Johnson syndrome (SJS) caused by antituberculosis drugs represents an extremely rare event. This situation can manifest between 2 and 8 weeks after the first exposure to the medication. The overlap of these conditions can lead to atypical clinical manifestations, thus complicating the early diagnosis and the implementation of early treatment. This report describes the case of a patient who developed the DRESS/SJS overlap 35 days after starting antituberculosis treatment. The patient experienced severe skin

Sarcoidosis is a multiorgan, granulomatous disease of an unknown etiology. The characteristic feature of the disease is the formation of noncaseating granulomas. Spontaneous resolution occurs in most patients, but the clinical course may be chronic or progressive, complicated by pulmonary fibrosis, which is a major cause of mortality in sarcoidosis. Recent studies have provided new information on the immunological mechanisms of pulmonary fibrosis. Its pathogenesis includes the alteration of lymphocyte activity and the imbalance between their subpopulations, the polarization of macrophages to t

Isolated retinal involvement in ocular tuberculosis (TB) is not well understood and remains largely underreported. TB retinal lesions can be classified into two broad categories: retinal vasculitis and isolated retinal granuloma. TB retinal vasculitis is the more recognized phenotype and may present with perivascular infiltrates alone or alongside other ocular signs such as subvascular lesions, focal vascular tortuosity, and occlusive vasculitis. These additional signs are crucial for diagnosing TB retinal vasculitis in TB-endemic regions. However, isolated retinal granulomas due to TB are rar

Periodic fever syndrome is characterized by three or more febrile episodes in six months, each occurring at least seven days apart. Immune dysregulation in systemic autoinflammatory syndromes involves innate immunity (neutrophils, monocytes, and macrophages) and cytokines, mainly IL-1, with tumor necrosis factor (TNF), interferon alpha and beta, IL-2, IL-12, IL-18, and IL-23. Treatment includes colchicine, corticosteroids, immunosuppressants, and biologics. Prebiologic screening should be done to rule out tuberculosis, HIV, hepatitis B, and hepatitis C. Monitoring includes biomarkers of inflam

Histoplasmosis is an endemic mycosis in some regions affecting particularly immunosuppressed individuals. The global prevalence of disseminated histoplasmosis in systemic lupus erythematosus (SLE) patients is <1 %, but rises in endemic areas such as Colombia. Clinical manifestations in those patients are atypical, ranging from non-specific symptoms to severe cases of multi-organ failure. Given its rarity and the nonspecific nature of its symptoms, disseminated histoplasmosis in the context of systemic lupus erythematosus remains a diagnostic challenge. A 28-year-old female patient with a histo
