Technical Tip: The Use of Arthroscopic Portals to Safeguard Medial Neurovascular Structures During Suture Button Fixation.

IntroductionAnkle fractures with concurrent syndesmotic instability are commonly encountered orthopaedic injuries. Suture button syndesmotic fixation has emerged as a favorable alternative to traditional screw fixation due to its comparable biomechanical stability and reduced hardware removal rates. However, concerns persist regarding potential injury to medial neurovascular structures during tibial cortical button deployment and a medial incision is often recommended for verification.TechniqueThis paper describes a safe and reproducible technique for suture button fixation that offers an alte

Association of Surgical Timing With Complications and Patient-Reported Outcomes After Achilles Tendon Repair.

BACKGROUND: Achilles tendon ruptures pose a challenging recovery for patients, and complications after surgical repair are often associated with poor patient outcomes. The optimal timing for surgery remains a topic of debate and has not been extensively studied. PURPOSE: To determine whether the time from Achilles rupture injury to surgical repair is associated with postoperative complication rates and long-term patient-reported outcomes (PROs). STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: Patients undergoing surgical treatment for an Achilles rupture between 2016 and 2022 wer

Anatomical Structures at Risk in Percutaneous Distal Bunionette Correction.

Bunionette deformity is an incredibly pervasive issue in our society with almost a quarter of individuals being affected by it. As it is so common, there are numerous techniques and approaches to correct the deformity. Currently, there is a growing trend that favors percutaneous osteotomy of the bunionette. As there are multiple osteotomy sites, there are anatomical considerations that must be made at each one. The purpose of this study was to investigate the anatomic structures at risk during distal osteotomy of bunionette deformity using a Shannon burr. Using 11 fresh cadaver specimens, the

Design of a Multicenter, Randomized Controlled Trial for the Treatment of Peripheral Neuropathic Pain (COMFORT Study) with a Micro-Implantable Pulse Generator.

BACKGROUND: Peripheral Nerve Stimulation (PNS) is an established therapy for chronic neuropathic pain of peripheral origin, typically following nerve injury. However, there is a paucity of Randomized Controlled Trials (RCTs) demonstrating the therapeutic benefits of PNS. The goals of the current study (COMFORT Study) are to document the safety and efficacy of the Nalu Neurostimulation in a PNS RCT, compared to conventional medical management (CMM). METHODS/DESIGN: This is a prospective, multicenter, RCT evaluating the treatment of neuropathic pain with PNS therapy. One of the following four r

Talar Arthroscopic Reduction and Internal Fixation (TARIF): A Novel All-Inside Soft-Tissue-Preserving Technique.

BACKGROUND: Talar arthroscopic reduction and internal fixation (TARIF) is an alternative approach for the operative fixation of talar fractures that may be utilized instead of more traditional open approaches such as medial, lateral, or even dual anterolateral. The TARIF approach allows for nearly anatomic fracture reduction and fixation of talar neck, body, and posterior dome fractures while minimizing the soft-tissue stripping and vascular injury associated with the standard anterolateral approach. DESCRIPTION: Following initial closed fracture reduction and any associated procedures, we re

A Rare Case of Spinal Cord Injury Following Thoracic Radiofrequency Ablation.

Medial branch radiofrequency ablation (RFA) has become a common treatment for facet-related back pain. While this procedure is often performed in the lumbar and cervical spinal segments, it can also be applied to the thoracic spine. Complications of spinal RFA at any level are scarce in the literature and are often mild. The patient is a 37-year-old male with a family history of multiple sclerosis in his siblings who underwent thoracic RFA at the T2-T5 nerve root levels. Within one week of the procedure, the patient experienced paresthesias below the nipple line and progressive lower-extremity

Ankle fusion percutaneous home run screw fixation: Technical aspects and soft tissue structures at risk.

INTRODUCTION: The objective of this cadaveric study was to identify the number of attempts necessary for a perfect positioning of the ankle fusion home run screw and the neurovascular and tendinous structures at risk. METHODS: Eleven cadaveric limbs were used. Guidewires were percutaneously placed into the distal posterolateral aspect of the leg, under fluoroscopic guidance, with the ankle held in neutral position. Malpositioned guidewires were not removed and served as guidance for the following wires. The number of guidewires needed to achieve an acceptable positioning of the implant was no

Multiple locations of nerve compression: an unusual cause of persistent lower limb paresthesia.

A paucity of appreciation exists that the "double crush" phenomenon can account for persistent leg symptoms even after spinal neural decompression surgery. We present an unusual case of multiple locations of nerve compression causing persistent lower limb paresthesia in a 40-year old male patient. The patient's lower limb paresthesia was persistent after an initial spinal surgery to treat spinal lateral recess stenosis thought to be responsible for the symptoms. It was later discovered that he had peroneal muscle herniations that had caused superficial peroneal nerve entrapments at 2 separate

Minimally invasive reconstruction of lateral ligaments of the ankle using semitendinosus autograft.

BACKGROUND: Multiple techniques have been described for reconstruction of the lateral ligaments of the ankle. Most require extensive exposure and dissection, which may lead to potential problems with wound healing, higher risk of nerve injury, fibrosis, and stiffness. This study reports on the results of a minimally invasive method to reconstruct the ligaments using a semitendinosus tendon autograft and achieve a stable ankle while avoiding these problems. MATERIALS AND METHODS: From September 2006 to May 2010, 25 patients (14 males, 11 females) with chronic ankle instability underwent latera

Retrospective analysis of neurapraxia and axonotmesis injuries of select peripheral nerves of the foot and ankle and their conservative and surgical treatment (external neurolysis and neurectomy).

There is a paucity of scientific literature that has reviewed the conservative and surgical treatment efficacy for the management of injuries causing neurapraxia and axonotmesis. This retrospective study evaluates the clinical outcomes of certain treatments for these injuries. Twenty-seven patients fulfilled the inclusion/exclusion criteria for the study, and represented both genders and a wide variety of ages, weights, levels of education, and backgrounds. Surgical intervention resulted in a slightly better clinical outcome when compared to conservative therapies. Patients undergoing surgery

The sural nerve in the foot and ankle: an anatomic study with clinical and surgical implications.

Operative procedures and traumatic injuries of the lateral foot and ankle place the sural nerve and its branches at risk. Anatomic studies quantifying the course of this nerve are sparse. In this study, 17 cadaver specimens were dissected to clarify its course and branching patterns. Despite multiple variations of the nerve and its branches, description of the course of a "typical" nerve trunk was possible. The nerve was found to lie in close proximity to the Achilles tendon at a level of 7 cm above the tip of the lateral malleolus. In the hindfoot, the nerve coursed 14 mm posterior and 14 mm
