Pedal Pull-Through Rail and Wingman-Assisted Coaxial Push for Entrapped Device Retrieval in Calcified Tibial Arteries.

OBJECTIVE: To describe a bailout technique for retrieving an entrapped, fractured endovascular device fragment in severely calcified below-the-knee arteries when conventional snare retrieval is not feasible. KEY STEPS: 1) Immediately stop manipulation and maintain the position of the antegrade guidewire holding the fragment (the "Golden Rule"); 2) obtain retrograde access; 3) advance a snare from the retrograde sheath to capture and externalize the existing antegrade guidewire, creating a pull-through rail without wire exchange; 4) advance a Wingman catheter antegradely over the rail; and 5)

Reconstruction of plantar degloving injuries using a prefabricated anterolateral thigh plantar glabrous skin flap.

ObjectivesTo evaluate the clinical efficacy of reconstruction of plantar degloving injuries using a prefabricated anterolateral thigh plantar glabrous skin flap (PALT-PGSF).MethodsTwelve patients, aged 22-56 years, with plantar degloving injuries were treated between July 2016 and December 2022. These defects are typically caused by traffic or machinery trauma. The avulsed plantar skin surfaces are intact, but revascularization are deemed unsuitable. The free tissue flaps transposed to reconstruct the defects were PALT-PGSFs. The flap size varied in width (6-13) cm and length (8-26 cm). The ti

Delayed Dorsalis Pedis Artery Pseudoaneurysm Following Open Reduction and Internal Fixation of a Lisfranc Injury: A Case Report.

Vascular complications following operative fixation of Lisfranc injuries are rare and may present in a delayed manner. This report describes a case of a woman in her early 60s who sustained a Lisfranc fracture-dislocation following a fall from height and was treated with open reduction and internal fixation. Initial recovery was uncomplicated. The patient re-presented two months later with recurrent pulsatile bleeding from the dorsal midfoot wound and symptomatic anaemia. CT angiography revealed a 14 × 8 mm pseudoaneurysm arising from the dorsalis pedis artery adjacent to the fixation hardware

Defining Potential Neurovascular Risk Zones in Superficial Plantar-Medial Release: An Anatomical Study.

Background: The superficial plantar-medial release (S-PMR) refers to a group of surgical procedures involving the release of the plantar aponeurosis and adjacent medial plantar soft tissues that are used in selected cases of plantar fasciitis and cavovarus foot deformity. The procedure aims to address pain and contracture of the plantar aponeurosis and intrinsic foot muscles, which may contribute to pathological foot alignment and gait instability. Due to the close proximity of highly variable neurovascular structures in the plantar region, precise anatomical knowledge and a patient-specific,

Clinical application of the 'like-with-like' split toe flap technique for reconstruction of soft tissue defects in multiple fingers.

OBJECTIVE: Conventional methods for reconstructing multiple digital soft tissue defects often result in significant donor-site morbidity and suboptimal functional outcomes. This study aimed to evaluate an innovative application of the "like-with-like" split toe flap (STF) technique for single-stage reconstruction of multiple digits while optimizing functional and esthetic outcomes and preserving donor-site function. METHODS: A prospective case series was conducted in six patients presenting with multiple digital soft tissue defects. Technical innovations included layered debridement, precisio

A male rat model integrating hypertension, chronic cerebral hypoperfusion, and aging recapitulates multimodal cerebral small vessel disease phenotypes.

BACKGROUND: Cerebral small vessel disease (CSVD) usually develops under the combined influence of vascular risk factors, chronic perfusion insufficiency, and age-related brain vulnerability; however, animal models driven by a single insult do not fully reproduce this multifactorial clinical context. METHODS: We established a composite CSVD rat model by combining spontaneous hypertension, bilateral common carotid artery stenosis (BCAS)-induced chronic cerebral hypoperfusion, and D-galactose (D-gal)-induced aging-related stress. Fourteen-week-old male Wistar-Kyoto (WKY) rats and spontaneously h

Acute digital ischemia in a child following a suspected arthropod bite: A case report.

RATIONALE: Acute limb ischemia is exceptionally rare in pediatric patients and poses a significant risk of permanent disability or limb loss. While arthropod bites are typically benign and self-limited, this case illustrates an unusual presentation of digital ischemia in a previously healthy toddler, suspected to be associated with an arthropod exposure. This report contributes to the limited literature highlighting vascular complications secondary to insect-related injuries in children. PATIENT CONCERNS: A 2-year and 5-month-old male presented with a 3-day history of black discoloration in t
