Two patients had toe-to-hand transfers based on the medial plantar artery system. The dorsalis pedis and first dorsal metatarsal arteries were absent in both cases. Although the procedure is slightly more complicated than currently described techniques for toe-to-hand transfers based on the dorsal system, the procedure was performed in both cases without complication.

On the basis of seven transfers of the second toe and one "en bloc" transfer of the second and third toe, technical features are discussed. The use of a plate for osteosynthesis is proposed. It has been found very useful, particularly when a MP-arthrodesis is performed between the first metacarpal and the second toe. In the absence of grasp, either traumatic or congenital, free transfer of the second toe to the hand is one of the reconstructive possibilities. An essential prerequisite for the acceptance of the transferred toe is a guaranteed blood supply through the vascular pedicle. Based on a homogeneous series of 71 toe-to-hand transfers performed between February 1975 and December 1982 the same surgeon, the authors have tried to define their present day indications. A composite graft of skin and attached vein was taken from the dorsal aspect of the foot and used in the replantation of 6 amputated digits in 5 patients which were complicated by loss of skin and blood vessels and with exposed bone and tendon. The 5 replanted digits survived in 4 patients. Complete survival of the skin graft was achieved in two digits and partial necrosis occurred in three digits. The technique is put forward as a possible method of vascular reconstruction in which skin loss is complicated by exposure of the deeper structures i.e., bone, tendon and/or joint.

Free transplantation of subaxillary lateral thoracodorsal flap is recommended. The thoraco-dorsal artery and its accompanying veins may nourish and drain a large musculo-cutaneous flap. Transplantation of the second toe to replace a finger has been suggested more recently than has transplantation of the great toe to replace the thumb. The technique is described in detail. There is no plantar skin incision in the foot. The arterial suture is made at the level of the digital artery. Two toes can be transplanted when several fingers are missing. 11 cases have been operated on and the results analysed. In all cases, pinch grip between the thumb and the transplanted finger was made possible.

A 19-year-old man sustained a mutilating crush injury in August, 1978, in an 80-ton punch press. The injury resulted in transmetacarpal amputations of the index through little fingers.
