OBJECTIVE: To describe a bailout technique for retrieving an entrapped, fractured endovascular device fragment in severely calcified below-the-knee arteries when conventional snare retrieval is not feasible.
KEY STEPS: 1) Immediately stop manipulation and maintain the position of the antegrade guidewire holding the fragment (the "Golden Rule"); 2) obtain retrograde access; 3) advance a snare from the retrograde sheath to capture and externalize the existing antegrade guidewire, creating a pull-through rail without wire exchange; 4) advance a Wingman catheter antegradely over the rail; and 5)

Vascular complications following operative fixation of Lisfranc injuries are rare and may present in a delayed manner. This report describes a case of a woman in her early 60s who sustained a Lisfranc fracture-dislocation following a fall from height and was treated with open reduction and internal fixation. Initial recovery was uncomplicated. The patient re-presented two months later with recurrent pulsatile bleeding from the dorsal midfoot wound and symptomatic anaemia. CT angiography revealed a 14 × 8 mm pseudoaneurysm arising from the dorsalis pedis artery adjacent to the fixation hardware

Background: The superficial plantar-medial release (S-PMR) refers to a group of surgical procedures involving the release of the plantar aponeurosis and adjacent medial plantar soft tissues that are used in selected cases of plantar fasciitis and cavovarus foot deformity. The procedure aims to address pain and contracture of the plantar aponeurosis and intrinsic foot muscles, which may contribute to pathological foot alignment and gait instability. Due to the close proximity of highly variable neurovascular structures in the plantar region, precise anatomical knowledge and a patient-specific,

BACKGROUND: To evaluate the clinical outcomes of a personalized surgical strategy for small-to-medium finger defects, utilizing either free dorsal toe or plantar proper digital artery flaps.
METHODS: We retrospectively analyzed 35 patients with finger defects (March 2021-January 2025). Patients were assigned by defect characteristics to Group A (pulp defects, n = 21; plantar proper digital artery flap) or Group B (dorsal/lateral defects, n = 14; modified dorsal toe artery flap preserving the plantar proper neurovascular bundle). We evaluated operative time, flap survival, Disabilities of the

BACKGROUND: Cerebral small vessel disease (CSVD) usually develops under the combined influence of vascular risk factors, chronic perfusion insufficiency, and age-related brain vulnerability; however, animal models driven by a single insult do not fully reproduce this multifactorial clinical context.
METHODS: We established a composite CSVD rat model by combining spontaneous hypertension, bilateral common carotid artery stenosis (BCAS)-induced chronic cerebral hypoperfusion, and D-galactose (D-gal)-induced aging-related stress. Fourteen-week-old male Wistar-Kyoto (WKY) rats and spontaneously h

This case report presents a 25-year-old man who sustained a crush injury to the right lower limb at the workplace due to compression by a car wheel hub. Initial examination revealed marked swelling of the lower leg and midfoot with preserved sensation and peripheral perfusion. Radiographs demonstrated a bimalleolar ankle fracture and multiple foot fractures. Approximately 5 hours after admission, the patient developed rapidly increasing pain, pallor and cooling of the foot, absence of the dorsalis pedis pulse, decreased toe sensation, and oxygen saturation of 80%. CT angiography showed absent

PURPOSE: The surgical approach for midfoot injuries classically requires dual dorsal incision and identification of the neurovascular structures that are susceptible to injury during the surgery. The aim of this study was to map the topographic anatomy of the dorsum of the foot along with tarsal joints for the dorsal approach of midfoot surgery that would facilitate the surgery and minimize the risk of neurovascular injuries for surgeons who specially focus on foot and ankle injuries.
METHODS: The dorsum of the foot was evaluated in 12 feet injected with latex containing a red colorant to vis

PURPOSE: For patients with small-area emergency finger injuries and short venous defects, using the traditional second toe tibial free flap with dorsal toe veins would increase donor site damage and compromise the esthetic outcomes of both hands and feet. This study presents an innovative harvesting technique for the second toe tibial free flap, specifically designed for fingertip or volar pulp defect reconstruction.
METHODS: From March 2020 to June 2023, our institution managed 15 cases of finger pulp or fingertip defects using second toe tibial flaps with plantar vein grafts, involving 16 d

INTRODUCTION: Morton's neuroma is predominantly attributed to chronic nerve entrapment within third space adjacent metatarsals, the deep transverse metatarsal ligament (DTML), and the plantar skin. While conservative treatments are of election, failures require alternative interventions such as ultrasound-guided injections and various surgical procedures, including minimally invasive neurectomy and DTML release. This study aimed to anatomically assess the risks associated with the endoscopic dorsal surgical decompression of Morton's neuroma.
