Although rarely used, the abductor hallucis muscle has its indications in coverage of small defects at the medial aspect of the hindfoot as a proximally based muscle flap. The authors describe a 69-year-old female patient in whom the abductor hallucis muscle was used as a distally based flap to reconstruct a defect in the forefoot. An anatomic study was undertaken on two cadaveric feet to explore the practicality of the distally based abductor hallucis muscle flap before it was applied clinically. The distally based abductor muscle flap receives its blood supply from minor and major pedicles i

The fractures of the shaft of the tibia were analyzed in 120 cases. The level of the osteoporosis in the radiological examination was assessed in comparison to the healthy limb. Osteoporosis was recorded in 60.6% of the cases. The clinical symptoms of the Sudeck syndrome was recorded in 11.25% of the cases. The Laser-Doppler examination was carried out in 120 cases. A standard 3 points measurement was employed: the apex of the toe, the flexion-dorsal part of the foot, and the medial part of the mid tibia length. The examination was done comparatively on both limbs. The pressure used in the occ

A study of the arterial system in the first intermetatarsal space was performed during 53 toe-to-hand or vascularized tissue of the toe transfers with long arterial pedicles. The first dorsal metatarsal artery was used as a single pedicle in 25 transfers and the first plantar metatarsal artery in 21: both arteries were taken simultaneously in seven transfers. In 11 transfers, the dorsalis pedis artery and the perforating branch were absent, although this did not exclude the presence of a first dorsal metatarsal artery which arose from the plantar system by means of an ascending vascular branch

BACKGROUND: The combination of dorsal dislocation of the navicular from the talus and an associated comminuted fracture of the calcaneus (transcalcaneal talonavicular dislocation) is an unusual and severe injury. Six cases have been described previously. The purposes of this study were to report the prevalence of this injury and the variations in injury pattern and to characterize methods of treatment and patient outcomes.
METHODS: Eight patients with nine cases of transcalcaneal talonavicular dislocation were treated by the senior author between January 1, 1990, and February 28, 1998. The co

Basing on anatomical studies (20 dissected lower extremities) and clinical experiences (4 cases), the paper presents local transposition possibilities of a distally pedicled tibial flap (pedicled on the posterior tibial artery). The flap allowed for soft-tissue coverage of 2/3 of the plantar side of the foot and 1/4 of the dorsal aspect of the foot.

A 63-year old woman underwent a resection of a pseudoaneurysm in the dorsalis pedis artery. The aneurysm was suggested to have formed because of a bruise on the dorsal aspect of the foot 5 years previously. This is a rare case of a traumatic aneurysm in the peripheral artery.

BACKGROUND: Skin damage in chronic venous disease (CVD) may be partially attributable to free radical injury including that of nitric oxide (NO). The aim of this study was to measure total plasma NO in patients with CVD compared to control subjects.
METHODS: Forty-four patients with CVD and 13 control subjects with no arterial or venous disease were included in this study. The patients underwent duplex ultrasonography to confirm the extent of the venous disease and were assigned to the appropriate CEAP clinical stage. Exhaustive exclusion criteria were applied to prevent the influence of diet

The authors report 35 cases of use of the supramalleolar flap described by Masquelet et al. in 1988. In 27 cases, the arterial blood supply was in a mixed (anterograde and retrograde) fashion since the perforating branch of the peroneal artery was spared. In eight cases the arterial blood supply was in a retrograde fashion due to the location of the skin loss. As described by Valenti et al. In 1991, the authors recommend the use of a distal subcutaneous pedicled to avoid skin grafting over the tendons at the distal part of the lag. In main cases of anterograde blood supply the superficial pero

Twelve patients with posttraumatic defects of the thumb were treated in the years 1990-1999 by the trimmed great toe method. Eight of them had additional injuries or loss of long fingers (two patients lost all fingers). Reconstruction methods were chosen basing on the amputation level. A fragment of the trimmed great toe of an appropriate length was used distally to the distal part of the first metacarpal according to the technique described by Wei (9 patients). In cases of a more proximal amputation only a distal part of the great toe was used. Pollicisation of other elements of the index wit

The authors report a case of an Y anastomosis of a free flap on the peroneal artery. A 30-year-old patient, the victim of a multi-traumatism after a motorcycle accident, presented open tibia and peroneal fractures and skin defect at the dorsal aspect of the foot uncovering several fractures and luxations of the foot. When general state of health allowed the achievement of a free flap, the authors noted that the two tibial arteries were destroyed with an extensive thrombosis on each side of the injury which precluded the reconstruction of a reliable artery axis. The peroneal artery insured alon

A 36-year-old woman sustained an amputation of her right leg at the thigh level and a degloving injury of her left foot and ankle region in an accident during a suicide attempt. Primarily, her left foot was covered with a split skin graft, resulting in a soft-tissue defect at the medial malleolus and at the calcaneus bone. Reconstruction was planned with a free latissimus dorsi muscle flap. Preoperative examinations revealed an arteria peronea magna with a hyperplastic peroneal artery solely providing arterial blood supply to the foot. The arteria peronea magna divided into two branches proxim

To devise a new classification of vascular variations in second toe transfer we analyzed the anatomic details of the first dorsal metatarsal artery in 304 cases of second toe transplantation and the venous structure in 200 cases. According to location, the arterial vascularity was classified as superficial (18% of cases), intramuscular (54%), inframuscular (23%), and absent (5%). According to diameter, it was classified into large (16%), medium (64%), and small (20%). Based on branching pattern, the artery was classified into ramifying type (88%), main trunk (4%), and fine branch type (8%). Th

Difficulty of soft tissue defects of the lower leg demands the development of new methods to treat such defects. The aim of this study is the examination of perforators and the various ways of blood supply to the skin in the lower leg. Provided with certain regularity, we would be able to cure soft-tissue defects also in the difficult zone of the distal segment and on the dorsum of the foot not harming vessels and not affecting mobility of muscles. Subcutaneous island-flaps supplied by perforating vessels could replace free flaps. By saving the crural fascia of 10 lower legs we flayed layers o

We evaluated the short- to mid-term results of an unconstrained total ankle prosthesis (S. T. A. R.) with uncemented fixation. Fifty consecutive ankle replacements were performed in 48 patients between 1996 and 1999. The initial diagnosis was posttraumatic osteoarthrosis in 31 cases (62 %), primary osteoarthrosis in 8 cases (16 %), and systemic joint affection in 11 cases (22 %), e. g. rheumatoid arthritis (6 cases), hemochromatosis (2 cases), psoriasis arthritis (1 case), lupus erythematodes (1 case), and sclerodermia (1 case). There were two perioperative complications: one superficial wound

OBJECTIVE: This is to report the design and application of the medialis pedis flap.
METHODS: Based on the medial fasciocutaneous branches of the dorsal pedal artery, a medialis pedis flap can be raised. The flap had been used in 18 patients with soft tissue defects of the dorsum pedis, ankle region, lower leg or hand.
RESULTS: The anatomic observation of the blood supply to the medial region of the foot showed that a constant distribution of fasciocutaneous branches issued from the dorsal pedal artery was an important blood supply to this area. All flaps used in the 18 patients survived comp

OBJECTIVE: In order to investigate the causes of necrosis of the retained flap after transplantation of the great nail flap, according to the principle of plastic surgery, the following operation was designed.
METHODS: A tongue-shaped great toe flap was made with its base on the tibial side of the great toe and the width of the base was 1 cm to 1.5 cm. The flap contained the supporting vessels and nerves and the periosteum was also preserved.
RESULTS: From June 1982 to November 1997, the flap was used to repair 267 case, 277 fingers defects. After operation, the retained flap on the tibial s

Ten arterialized venous toenail flaps with two venous pedicles, one of which was anastomosed to a digital artery and the other to a dorsal vein of the finger, were used in nine patients to reconstruct nail loss due to trauma. Four flaps were taken from the lateral part of the big toe and six flaps from the second toe. Four toenail flaps with pulp and three flaps with the distal half of distal phalanx were used. Nine flaps survived completely and one had partial necrosis. All showed excellent aesthetic and functional results except for one case with minimal deformity in growth of the nail. The

The dorsal thoracic fascia is the anatomic layer that contains the blood supply to the scapular and parascapular fasciocutaneous flaps. The cutaneous vascular territory of the circumflex scapular artery and its parascapular branches is well known. During the past 8 years, the authors have employed the dorsal thoracic fascia free flap for extremity reconstruction in 17 patients (upper extremity, n = 9; lower extremity, n = 8). The indications for using this free flap included primary coverage after trauma (n = 10), resurfacing of "unstable" scar (n = 3), coverage of plantar foot ulceration (n =

Two original operative techniques of raising the extensor digitorum brevis muscle flap are presented. These methods allow for covering distal foot defects that are difficult to cover by other reconstructive means. In the first technique, the flap is based on an extended distal pedicle supplied by the dorsal interosseous artery of the first intermetatarsal space. In the second technique, the flap receives its vascular supply from the medial tarsal artery; this procedure may be valuable when the vascular supply of the dorsalis pedis pedicle has been disrupted. To confirm the availability of thes

The aim of this study is to stress the possibility of harvesting a more sizable adipofascial turn-over flap based on the perforators of the dorsalis pedis artery than previously described. This flap can resurface the most distal dorsal defects of the forefoot, and the posttraumatic distal amputation of the forefoot, as a new indication. A detailed anatomic dissection of the flap was carried out on 12 fresh lower extremities together with injection study to determine flap pedicle location, flap dimensions, and its area of reach in the distal forefoot. The anatomic findings encouraged the author
