Reconstruction of plantar degloving injuries using a prefabricated anterolateral thigh plantar glabrous skin flap.

ObjectivesTo evaluate the clinical efficacy of reconstruction of plantar degloving injuries using a prefabricated anterolateral thigh plantar glabrous skin flap (PALT-PGSF).MethodsTwelve patients, aged 22-56 years, with plantar degloving injuries were treated between July 2016 and December 2022. These defects are typically caused by traffic or machinery trauma. The avulsed plantar skin surfaces are intact, but revascularization are deemed unsuitable. The free tissue flaps transposed to reconstruct the defects were PALT-PGSFs. The flap size varied in width (6-13) cm and length (8-26 cm). The ti

Defining Potential Neurovascular Risk Zones in Superficial Plantar-Medial Release: An Anatomical Study.

Background: The superficial plantar-medial release (S-PMR) refers to a group of surgical procedures involving the release of the plantar aponeurosis and adjacent medial plantar soft tissues that are used in selected cases of plantar fasciitis and cavovarus foot deformity. The procedure aims to address pain and contracture of the plantar aponeurosis and intrinsic foot muscles, which may contribute to pathological foot alignment and gait instability. Due to the close proximity of highly variable neurovascular structures in the plantar region, precise anatomical knowledge and a patient-specific,

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Acute digital ischemia in a child following a suspected arthropod bite: A case report.

RATIONALE: Acute limb ischemia is exceptionally rare in pediatric patients and poses a significant risk of permanent disability or limb loss. While arthropod bites are typically benign and self-limited, this case illustrates an unusual presentation of digital ischemia in a previously healthy toddler, suspected to be associated with an arthropod exposure. This report contributes to the limited literature highlighting vascular complications secondary to insect-related injuries in children. PATIENT CONCERNS: A 2-year and 5-month-old male presented with a 3-day history of black discoloration in t

Reconstruction of Fingertip or Pulp Defects Using the Free Second Toe Tibial Flap With Plantar Vein.

PURPOSE: For patients with small-area emergency finger injuries and short venous defects, using the traditional second toe tibial free flap with dorsal toe veins would increase donor site damage and compromise the esthetic outcomes of both hands and feet. This study presents an innovative harvesting technique for the second toe tibial free flap, specifically designed for fingertip or volar pulp defect reconstruction. METHODS: From March 2020 to June 2023, our institution managed 15 cases of finger pulp or fingertip defects using second toe tibial flaps with plantar vein grafts, involving 16 d

Safety of One-Stage Reconstruction of Achilles Tendon and Soft Tissue Defects Resulting from Infection After Achilles Tendon Repair using Flexor Hallucis Longus Tendon Transfer and Reverse Sural Artery Flap.

Infection after Achilles tendon (AT) repair can lead to defects in both the tendon and the surrounding soft tissue. In such cases, a two-stage surgical approach is generally adopted, with soft tissue reconstruction performed initially, followed by secondary tendon reconstruction. A reverse sural artery flap (RSAF) after AT repair is not commonly performed due to potential damage to the flap pedicle. This study aims to evaluate the clinical outcomes of simultaneous flexor hallucis longus tendon (FHLT) transfer and RSAF to treat soft tissue and tendon defects resulting from infection after AT re

Tunneling the Pedicle: A Novel Technique for Free Flap Reconstruction in Plantar Forefoot Wounds.

Free flap reconstruction has become the standard of care for managing critical foot defects, particularly in diabetic forefoot plantar wounds. Key considerations in achieving optimal outcomes include minimizing flap failure risk, restoring foot shape without excessive bulk, and reducing donor site morbidity. However, reconstructing plantar forefoot ulcers presents a unique challenge due to the distance between the defect and recipient vessels, such as the posterior tibial artery (PTA) or anterior tibial artery (ATA). Conventionally, large and bulky flaps are required to bridge this gap, often

Is endoscopic decompression for Morton's neuroma a safe technique?

INTRODUCTION: Morton's neuroma is predominantly attributed to chronic nerve entrapment within third space adjacent metatarsals, the deep transverse metatarsal ligament (DTML), and the plantar skin. While conservative treatments are of election, failures require alternative interventions such as ultrasound-guided injections and various surgical procedures, including minimally invasive neurectomy and DTML release. This study aimed to anatomically assess the risks associated with the endoscopic dorsal surgical decompression of Morton's neuroma. MATERIALS AND METHODS: Twenty feet from ten fresh-f

Percutaneous Flexor Tenotomy Plus Plantar Capsule Release for Rigid Hammertoe: A Cadaveric Study.

BACKGROUND: Rigid hammertoe deformities in diabetic patients present a therapeutic challenge: percutaneous tenotomy often fails to address capsular contractures, whereas arthrodesis carries an elevated risk of complications. We hypothesized that combining tenotomy with plantar capsule release would provide effective correction while preserving neurovascular structures. This study evaluates the feasibility correction magnitude and vascular safety of this technique through a cadaveric model. METHODS: We perfused 10 clinical-grade lower-limb cadaveric specimens, possessing 14 rigid hammertoes (N

Transverse Distraction of Great Toe to Enlarge the Donor Site before Finger Reconstruction.

Partial great toe transfer is widely used in finger reconstruction. Although satisfactory results have been reported at the recipient's hand, the donor foot still presents with many problems due to the large amount of tissues harvested. In this study, the Ilizarov technique was utilized to enlarge the great toe in order to minimize the amount of tissue sacrificed of the donor foot. In this retrospective study, 23 patients (30 toes) underwent transverse distraction of the great toe for finger reconstruction from September 2020 to December 2022. The width of the contralateral normal finger was s
