The authors present a new type of the classical medial plantar flap which derives its main blood supply from the medial plantar artery, a terminal branch of the posterior tibial artery. Our approach concerning flap dissection has not been modified, but the vascular supply of the flap has been shifted and a reverse blood flow coming from the lateral plantar artery is created after dividing the posterior tibial artery just proximal to its distal bifurcation. The reverse flow medial plantar artery island flap survival is ensured by rich anastomoses that usually occur between the dorsalis pedis an

Eighteen free second toe were used for pollicization other than the conventional method. From the first and second plantar metatarsal arteries junction of the both digital arteries of the second toe were dissected retrogradely and anastomosed to the deep of superficial palmar arch or terminal branch of the radial artery. The venous return was established between the dorsal venous arch and cephalic vein. These vessels were found to have much bigger diameters and less anatomical variation than those used by conventional methods. Seventeen transplantation were considered successful both functiona

The instep neurovascular flap, based on the medial plantar artery and innervated by the medial plantar nerve, is in our opinion the treatment of choice for resurfacing heel skin defects. The flap is usually raised as an island flap in the non weight-bearing surface of the foot. It provides heel cover with thick skin combined with a perfectly adapted texture. Conservation of sensibility offers a protective advantage. Four patients, treated with the flap, are discussed.

Absence of dorsalis pedis artery in 6 feet of 6 patients was found in 160 feet of 148 cases operated upon for toe-to-hand transfer, rating 3.8%. In such a transfer, the first dorsal metatarsal artery, or the first plantar metatarsal artery, if the former was also absent, which was always larger than usual, was dissected out and cut off at its origin. Being the nutrient vessel for the transfer, it was anastomosed to the distal segment of the radial artery at or beyond the anatomic snuffbox. Otherwise, vein grafting was done to bridge the gap in between, or direct anastomosis of the common digit

An anatomic study (40 fresh dissected specimens) and clinical experience (14 patients) have shown the reliability of a skin flap designed on the lower third of the lateral aspect of the leg. It is supplied by a cutaneous branch from the perforating branch of the peroneal artery. This perforating branch continues distally deep to the fascia along the anterior ankle and into the foot. This can be used as a reversed pedicle, giving the flap an arc of rotation that allows coverage of the dorsal, lateral, and plantar aspects of the foot, the posterior heel, and the lower medial portion of the leg.

The external supramalleolar flap is a mixed cutaneoaponeurotic flap removed from the external surface of lower part of leg and with, as its principal vascular pedicle, the anterior perforating branch of the peroneal artery. The flap can be used in two ways: as a rotation flap with distal cutaneous hinge allowing lover third of internal surface of leg and instep to be covered, or as a retrograde flux island flap by anastomosis of perforating artery to arteries of foot. The latter procedure is of interest in that it permits covering of loss of distal substance of foot in the dorsal or plantar re

This report correlates the results of the lateral angiogram of the foot with the operative dissection and eventual outcome in 29 patients in whom a great toe-to-hand transfer was performed to treat a traumatic loss of the thumb. Our angiographic findings were confirmed by surgical exploration and indicated that (1) in 20 (70%) of 29 patients the first dorsal metatarsal artery (FDMA) originated from the dorsalis pedis artery, dorsal to the midlongitudinal axis of the first metatarsal bone; (2) in 6 (20%) of 29 patients the FDMA originated from the dorsalis pedis artery, plantar to the midlongit

The chinese forearm flap is vascularized by the radial artery. It can be used as an island transplant or as a free flap. The medial plantar island flap has also been called chinese flap. Advantages and disadvantages of these flaps are discussed in an attempt to summarize their indications and contraindications.

Foot skin can be used in traumatic nonreplantable leg amputations as an immediate neurovascular free flap for proximal stump reconstruction. The thick, glabrous skin on the sole of the foot provides durable, sensate coverage. The flap is large, avoids a secondary donor site, and can survive on either the dorsalis pedis or posterior tibial artery if one preserves the connecting deep plantar branch of the dorsalis pedis artery. Two cases of immediate stump reconstruction using the "fillet of sole" flap are presented.

Safe planes exist for plantar incisions that minimize the possibility of subcutaneous nerve injury and are therefore useful in flap design. Nerve branch orientation in the plantar subcutaneous tissue is specific and guides dissection so as to avoid producing anesthesia in weight-bearing areas. An extensive proximal plantar subcutaneous plexus exists that permits elevation of plantar flaps in a superficial plane. This is due to the major contribution that the dorsal circulation makes to the skin of the plantar surface. The blood supply to the non-weight-bearing midsole area is not from the medi

Based on the experiences of sixteen toe-to-hand transfers (nine for thumb reconstruction, seven for finger replacement), some technical problems of the operation are discussed. On the foot the incision should be planned in such a way that the wound can be closed directly to achieve a better aesthetic result The excision of the distal third of the second metatarsal bone marrows the foot, which improves the later appearance. On the hand deeper skin defects with exposed bone are easily closed by a forearm-island flap based on the distal radial artery. The rotation of the head of the transferred m

Two patients had toe-to-hand transfers based on the medial plantar artery system. The dorsalis pedis and first dorsal metatarsal arteries were absent in both cases. Although the procedure is slightly more complicated than currently described techniques for toe-to-hand transfers based on the dorsal system, the procedure was performed in both cases without complication.

On the basis of seven transfers of the second toe and one "en bloc" transfer of the second and third toe, technical features are discussed. The use of a plate for osteosynthesis is proposed. It has been found very useful, particularly when a MP-arthrodesis is performed between the first metacarpal and the second toe. It gives stability and therefore secondary displacement and/or rotation of the toe are avoided. It enables more accurate tension to be placed on the repaired tendons. For the flexor tendons it is suggested to repair the communis longus only, whereas both extensors, longus and brev

In the absence of grasp, either traumatic or congenital, free transfer of the second toe to the hand is one of the reconstructive possibilities. An essential prerequisite for the acceptance of the transferred toe is a guaranteed blood supply through the vascular pedicle. The blood supply of the second toe can be either by dorsal or plantar arteries, a fact which is due to anatomic variations. Through preoperative angiographic examination the arterial blood supply of the second toe can be defined. The location and length of the vascular pedicle have to be considered in the design of the anastom

Based on a homogeneous series of 71 toe-to-hand transfers performed between February 1975 and December 1982 the same surgeon, the authors have tried to define their present day indications. Technical modifications have been described for the second toe transfer : isolated dorsal approach of the toe with early-stage resection of the second metatarsal bone, utilisation of the plantar metatarsal artery of the second interosseous space, utilisation of the volar vessels for the anastomosis at the recipient site, dorsal subcutaneous tunnel for the venous anastomosis of the hand. Aside from the usual

Defects of the heel represent a difficult reconstructive problem. Previously described methods have not always been ideal especially for the posterior heel. The weight-bearing functional requirements of the heel tissue over the calcaneus are a sensitive, well padded, durable cover. The technique of choice should provide local similar tissue plus involve a single reliable operative procedure. Skin grafts placed on the calcaneus or on a muscle transposition flap, such as the flexor digitorum brevis, abductor hallucis, or abductor digiti minimi muscle, provide a thin, insensitive, and dissimilar

Transplantation of the second toe to replace a finger has been suggested more recently than has transplantation of the great toe to replace the thumb. The technique is described in detail. There is no plantar skin incision in the foot. The arterial suture is made at the level of the digital artery. Two toes can be transplanted when several fingers are missing. 11 cases have been operated on and the results analysed. In all cases, pinch grip between the thumb and the transplanted finger was made possible.

A 19-year-old man sustained a mutilating crush injury in August, 1978, in an 80-ton punch press. The injury resulted in transmetacarpal amputations of the index through little fingers. The thumb was revascularized by microvascular anastomoses. The replanted index to the little finger position failed. A free transfer of the patient's second toe to the fifth metacarpal was done to provide a post to oppose the thumb. The first web space of the toe was used in expanding the contracted single cleft of the hand. The innervated dorsal foot flap provided coverage for the dorsal surface of the hand. An
