A detailed examination was undertaken of hospitalized car occupants who sustained a lower limb injury in a frontal crash. The assessment included an analysis of the type, severity and causes of these injuries and mechanisms involved in lower limb fractures. The findings showed that fractures and dislocations occurred in 88% of lower limb injury cases, that more than half were from crashes < 48 km hour-1 and that the number of fractures was directly proportional with delta V. Ankle dislocations and foot fractures from the floor and toe pand were the most common injury-source combination overall

The authors describe an approach to the calcaneus: the incision follows the Achilles tendon and curves anteriorly between the lateral and plantar skin of the foot. This review is designed to address the problem of severe fractures of the lower extremities that often accompany the patient who is polytraumatized. Although open fractures occupy a predominant place in the subsequent discussion, closed fractures also may be associated with a significant crush component, a vascular lesion, a degloving injury, or a compartment syndrome. It is of paramount importance that the treating physician understand the implications of the potential or actual soft tissue damage to maximize the outcome after these devastating injuries.

We report the case of a 36-year-old man who developed a compartment syndrome after a minor ankle fracture was casted. The treatment was inadequately monitored and he subsequently developed the manifestations of a crush syndrome. This case demonstrates that relatively minor injuries can indeed be fatal if appropriate vigilance is not maintained.

Two different methods of treatment for open dislocation of the extruded talus without soft tissue attachments (missing talus) were examined. In case 1, a 20-year-old man sustained an open total dislocation of the talus due to a motorcycle accident. Authors describe the operative treatment of a tangential tissue defect with severe crush in the region of the external malleolus. Because of the injury with bone defect and opening of the joint an arthrodesis on the talocrural joint was performed. The opened joints and the free bone surface were covered with primary a. dorsalis pedis island flap plasty. Healing without complication was reached.

Split-thickness skin excision (STSE) was used as an adjunctive modality in the treatment of eight crush injuries of the foot. Compartment syndromes were present in four feet and were treated with fasciotomy. Wound debridement, internal fixation of fractures, and STSE followed. This technique accurately determined the viability of the skin flap, simultaneously providing skin for local wound coverage. Severe crush injuries with compartment syndrome were treated in five patients by an immediate one-stage procedure. We report the case of a Maisonneuve fracture of the fibula in which the diagnosis was initially missed. The patient presented with a chief complaint of medial ankle pain. One of the most easily misdiagnosed injuries, which can result in severe consequences in young patients, is a fracture involving the epiphyseal growth plate. The Salter and Harris type V fracture is a relatively uncommon injury resulting from a severe crushing force applied through the epiphysis to an area of the growth plate. It is most likely to occur in the region of the knee or ankle. Seventy-two patients with 76 fracture-dislocations of the Lisfranc tarsometatarsal joint complex were evaluated. Fifty-eight (81%) were polytrauma patients and the remainder suffered isolated injuries. Sixty of the original 72 patients were available for long-term study. Eight of these had an amputation at or shortly after the original admission, leaving 52 patients with 55 Lisfranc injuries for analysis. Crush injuries and abnormal pronation and supination are the most common reasons for typical ligament tears in this region. Most of them should be treated conservatively by plaster of Paris and early weight-bearing after exact clinical and X-ray diagnosis. At upper ankle joint, suture of ruptured fibular collateral ligaments should be favoured. For dislocations and fracture dislocations of the Lisfranc type operative reposition and K-wiring is recommended. Our treatment results with fibular collateral ruptures and fracture dislocations of the Lisfranc type are presented.
