BACKGROUND: The Lisfranc joint comprises the articulations between the cuneiforms and bases of the metatarsals and is susceptible to a variety of complex injuries which are frequently missed or diagnosed late. This can result in morbidity with poor outcomes, and renders it challenging to standardise management and allow prognostication. Existing classification systems, including the Myerson-Hardcastle (MHC) and Nunley & Vertullo classifications, do not consider the propagation of force through the midfoot, which is key to identifying structures which may require stabilization. They also demons

Orthopaedic trauma remains a major public health problem, particularly in rural India, where preventive strategies and trauma-care resources are often limited. This single‑centre descriptive observational study aimed to describe the epidemiological profile of orthopaedic trauma patients admitted to a major trauma centre in rural North India, with emphasis on age, sex, mechanism of injury, and pattern of skeletal involvement. This observational study included 2,340 consecutive patients of all ages admitted under orthopaedics with acute traumatic injuries after initial presentation to the emerge

BACKGROUND: Severe Gustilo-III foot injuries that combine segmental bone loss with extensive soft tissue damage remain challenging. One-stage reconstruction with a free fibular vascularized bone flap (FVBF) or free iliac vascularized bone flap (IVBF) offers the advantages of immediate skeletal stability, soft tissue resurfacing, and early mobilization, but contemporary outcome data are limited.
METHODS: From December of 2000 to November 2021, 6 patients who sustained from severe traumatic metatarsal, tarsal bone, and soft tissue defects were treated with a FVBF or IVBF incorporating soft tiss

Distal leg and ankle defects with exposed bone are difficult to reconstruct, particularly after contaminated open injuries. Free tissue transfer is unavailable in many resource-limited hospitals, making reliable pedicled flaps essential. We report three adult men with Gustilo-Anderson type IIIB distal leg or ankle injuries treated with a distally based peroneus brevis muscle flap at a Sudanese teaching hospital. In the first patient, a 10×5 cm distal tibial defect after motorcycle trauma was covered immediately after debridement and external fixation, followed by skin grafting on postoperative

BACKGROUND: In distal lower-limb reconstruction, reverse-flow pedicled flaps remain vulnerable to venous congestion and partial flap loss, particularly after crush injury or major trauma, in elderly or comorbid patients and when longer flaps are required. We evaluated a standardized distally-based pedicled posterior tibial artery perforator (PTAP)-plus saphenous flap with planned supercharging and a temporary cutaneous bridge to reinforce venous outflow in that high-risk subset.
METHODS: A retrospective study of 26 consecutive reconstructions (2010-2025) performed under a uniform protocol by

This case report presents a 25-year-old man who sustained a crush injury to the right lower limb at the workplace due to compression by a car wheel hub. Initial examination revealed marked swelling of the lower leg and midfoot with preserved sensation and peripheral perfusion. Radiographs demonstrated a bimalleolar ankle fracture and multiple foot fractures. Approximately 5 hours after admission, the patient developed rapidly increasing pain, pallor and cooling of the foot, absence of the dorsalis pedis pulse, decreased toe sensation, and oxygen saturation of 80%. CT angiography showed absent

INTRODUCTION: The incidence of Lisfranc injury in pediatric-adolescent children is lower compared with adults. There is limited research on adolescent patients in this age group, with only a few case reports of pediatric patients. Furthermore, studies with adequate follow-up are lacking in the literature. This study aims to provide insights into the causes and types of fractures and evaluate mid-term clinical and radiologic outcomes.
METHODS: A retrospective analysis was conducted on pediatric-adolescent patients diagnosed with a Lisfranc injury who underwent surgical treatment between June 2

BACKGROUND: Open fractures account for approximately 2.6% of all fractures but continue to pose a significant clinical challenge due to their associated complications and potential impact on limb function. Among these complications, infection remains particularly prevalent. Although numerous recent studies have addressed this issue, many are limited by restrictive inclusion criteria, especially regarding the anatomical segments of the limbs evaluated HYPOTHESIS: We hypothesized that recent advances in open fracture management may have impacted infection rates across the entire appendicular and

BACKGROUND: High-energy lower extremity crush injuries with concurrent infection and segmental bone defects present significant therapeutic challenges, often complicated by recalcitrant osteomyelitis and compromised bone healing. Traditional approaches frequently fail to achieve simultaneous infection eradication and structural reconstruction. This case demonstrates how a staged surgical protocol combining bone transport and antibiotic cement spacer technology addresses these dual challenges.
CASE REPORT: A 45-year-old male sustained an open tibiofibular fracture of the left lower extremity w
