A classification for congenital limb malformations.

The classification for congenital limb malformations adopted by the American Society for Surgery of the Hand, the International Federation of Societies for Surgery of the Hand, and the International Society of Prosthetics and Orthotics is presented. This method groups similar patterns of deficiencies according to the parts that have been primarily affected by certain embryological failures, whether the insult involves a total part (skeletal and soft tissue) or only the dermomyofascial structures. The main categories of this classification are (I) failure of formation of parts. (II) failure of

Evaluation of forequarter amputation in malignant diseases.

Twenty-one forequarter amputations were performed at our institute during a 15 year period for soft tissue sarcoma, osteosarcoma, recurrent malignant melanoma and epidermoid carcinoma. Two patients in this group who have had recurrent carcinoma of the breast with a painful, edematous and ulcerated upper extremity were palliated and had significant relief. The over-all five year survival rate following this procedure for curative intent was 25 per cent. There were no five year survivors of unclassified sarcoma, myosarcoma and recurrent malignant melanoma. Thus, we do not recommend this procedur

[Pathological anatomy of Edwards' syndrome (trisomy 18)].

Data of the literature (200 section observations) and those of 10 authors' own observations on the syndrome of trisomy with relation to chromosome 18 are presented. The pathologoanatomic diagnosis of Edwards' syndrome can be established without an investigation of chromosomes on the basis of a complex of congenital defects present in the newborn. The main of them are: prenatal hypoplasia, dolichocephaly, microgenia, concha auriculae drawn in the horizontal plane, flexor position of bones, shortness and thickness of the great toe and protruding heel, thickness and distortion of convolution of t

[Experimental skeletal teratogenesis: a disturbance of relative osteo-neural growth].

The previously suggested concept of the closest growth relations existing between the bony and the nervous tissue at the organ level of the spinal cord and the peripheral (including the facial) nervous trunks is experimentally buttressed. It is shown that the normal gross-morphological features of the vertebrae as well as of the tubular bones (viz., their length, physiological curvatures and terminal expansions) result from the adaptation of the bone growth to the slower proceeding and vulnerable neural extensive growth, viz., from a physiological osteo-neural growth disproportion. The more or

Repair of the amputated great toe.

A new method has been introduced to form a functional great toe following amputation. This method has several advantages, especially its simple one-stage operation, which uses a local plantar flap. Results are good from the esthetic and functional point of view. It may be used for traumatic and congenital defects of the great toe. Three cases are presented with illustrations.

Microvascular procedures in reconstructive surgery.

The application of microvascular technics to problems in reconstructive surgery has enabled the surgeon to revascularize ischemic digits, replant amputated digits, and to transfer directly composite tissue from one area of the body to another for reconstructive purposes. The success of these procedures is directly dependent upon the ability to perform and maintain patent microvascular anastomoses. Representative cases are reported including the revascularization of an incompletely amputated digit, the replantation of an amputated thumb, the direct transfer of a toe to replace a missing thumb,

Marlex mesh in gas gangrene.

Clostridial gas gangrene is a well recognized complication of traumatic and surgical wounds, and is associated with an overall mortality rate of 25% (5, 22). Gas gangrene of a limb results in a mortality rate approximately half that of gas gangrene of the trunk (4, 7, 8, 9, 11, 12, 13, 15, 16, 19, 24). Radical debridement and antibiotic therapy or high amputation of involved limbs are accepted traditional approaches to the problem. The role and value of hyperbaric oxygenation (OHP) remains controversial despite intense study over the past few decades. Patients with gas gangrene involving all l

Purpura, pigmentation and yellow nails of the lower extremities in diabetics.

This article describes purpura and pigmentations of the lower extremities as well as yellow nails mainly in elderly diabetics but also in persons not known to have diabetes. When the latter were compared to controls, it appeared that their glucose tolerance was altered in a diabetic direction. Precipitating factors could generally be established for these lesions, predominantly cardiac decompensation with edema of the legs, and were more common in patients not known to have open diabetes than in patients with open diabetes. Petechiae were transformed into small, pigmented, non-atrophic spots.

[The thenar flap].

In covering amputations of fingertips several methods, including free grafts and local flaps have proven useful. When the amputation is at a more proximal level near the cuticle closure of the wound is often accomplished by further shortening of the distal phalanx and eradication of the nail matrix in order to avoid a claw-like deformity of the nail. This results in a unpleasant stump of poor prehensile quality. Therefore attention is called to the distal thenar flap which offers all the advantages of the palmar flap e. g. adequate subcutaneous tissue for reconstruction of a fingertip, good te

Rheographic estimation of peripheral diabetic angiopathy.

Longitudinal rheography was used for investigation of 294 diabetics and 176 healthy controls aged 3-82 years. It was found that the vascular lesions in the lower extremities in diabetics are bilaterally similar, and that they are 3-14 times more frequent in comparison with nondiabetic controls. Whereas the diabetics are certainly threatened by severe vascular obliteration with advanced age, the same is not necessary for the nondiabetics. Lighter vascular lesions were found even in diabetic children under 5 years of age. A conclusion is reached that rheography is a reliable method for investiga

[Acroangiodermatitis and acral hyperstomia (author's transl)].

Acroangiodermatitis is a special form of the 'angiodermite' of Favre and Chaix in patients presenting with chronic venous insufficiency localized on the dorsal areas of the feet and toes. Different stages including exulceration may occur. 60% of our male and 38% of our female patients with postthrombotic syndrome showed signs of acroangiodermatitis. Developmental arteriovenous malformations in the feet (acral hyperstomia, Malan's syndrome; 1 patient) which may present similar clinical and histological pictures are rare and should be differentiated from acroangiodermatitis because their prognos

Effect of mechanical means on the restoration of the limb regenerative ability in metamorphic stages of Bufo regularis Reuss.

Limbs of Bufo regularis REUSS metamorphic stages No. 57, 63 and 66 were transected at the thigh level. Removal of the apical skin cover on the third postoperative day, followed by traumatization after further two days in the limb stumps, was more effective in the enhancement of the regenerative ability than when each one of these procedures was performed independently. Better results were achieved in cases of the preclimactic stage (No. 57) where about 30% of the cases (Series III) developed heteromorphic limb outgrowths having more distal structures as the shank ending with or without toes. [The foot in leprosy].

The more frequently occurring bone lesions of leprosy of the foot are described with illustrations from patients with leprosy seen some years previously in Sardinia. These cases are classified into groups depending on the presence of osteoporosis, osteitis, lesions with predominantly inflammatory manifestations, those with a spindle-shaped appearance and lastly those with pathological fractures. The anatomical relationship between cutaneous and bone lesions is outlined.

The syndrome of ectrodactyly, ectodermal dysplasia, and clefting (EEC).

Early recognition of ectrodactyly, ectodermal dysplasia, and clefting of the lip and palate as a syndrome could result in more beneficial treatment for the patient. Patients with the EEC syndrome often have ocular and auricular deficiencies that progressively become more severe. These patients are often seen first by cleft palate teams who make the diagnosis. The patient's dental status requires frequent evaluation after corrective procedures for cleft lip and cleft palate.

Surgical treatment for absent single-hand grip and elbow extension in quadriplegia. Principles and preliminary experience.

A classification of quadriplegic patients based on available sensory receptors (ocular only or both ocular and hand sensibility) and motor function in each upper extremity greatly facilitates the planning and evaluation of surgical treatment. In forty hands of thirty-three patients with post-traumatic quadriplegia and cord lesions higher than those usually thought to be benefited by reconstructive surgery, three or more procedures were performed at one or more sittings to create an active wrist extensor and a thumb flexor grip, a function easier to provide and much more useful to these patient

Results of minor foot amputations for ischemia of the lower extremity in diabetics and nondiabetics.

Results of 208 minor amputations were analyzed in 179 patients who had no food pulses. Wound healing was assessed at 3 months in relation to diabetes and previous vascular surgery. The results suggest that simple removal of the toe or toes is not advisable unless the blood supply to the foot can be improved by vascular reconstruction or sympathectomy, or both. Transmetatarsal amputation should be considered more often as a conservative amputation for gangrene of the toes. The absence of a palpable posterior tibial pulse is a contraindication to the Syme's amputation.

Hammertoe surgery: waist resection of the proximal phalanx, a more simplified procedure.

A technic for correcting hammertoe and other toe deformities by waist resection of the proximal phalanges of the toes effectively corrected these conditions in 95 patients and 466 toes. This procedure can be done quickly and easily with low morbidity. Through a very short incision over the waist of the proximal phalanx, a subperiosteal resection is carried out which permits settling of the resected bone ends together within the periosteal tube, resulting in bony union. The proximal phalanx is effectively shortened and the imbalance of the tendon length to the bony arch, which originally produc

The surgical management of ingrowing toenail.

A retrospective review has been carried out on 200 randomly selected patients with ingrowing toenail in order to assess the surgical management and its results. There was a predominance of males between the ages of 11 and 30 with an ingrowing toenail. Despite the high recurrence rate following simple avulsion of the toenail, its place in the initial management of the condition is justified. As regards more definitive surgery, total proximal nail bed ablation was found to give the best results. Sepsis at the time of operation neither increased the recurrence rate nor caused severe postoperative
