The Journal of foot and ankle surgery : official publication of the American College of Foot and Ankle Surgeons2026eng
BACKGROUND: Total ankle arthroplasty (TAA) and tibiotalar arthrodesis (TTA) are primary surgical treatments for end-stage tibiotalar arthritis, but comparative short-term outcomes remain incompletely characterized.
PURPOSE: To compare complications, readmission, discharge disposition, and hospital length of stay (LOS) after TAA versus TTA.
STUDY DESIGN: Level III retrospective cohort study.
METHODS: Adults with primary or post-traumatic tibiotalar osteoarthritis who underwent TAA or TTA from 2018 through 2024 were identified in the HCA Enterprise Database. Outcomes included surgical site in

BACKGROUND: Diabetic foot ulcers are a leading cause of non-traumatic lower-limb amputation, but early identification of inpatients at high risk of major amputation remains challenging.
METHODS: We retrospectively reviewed consecutive admissions for diabetic foot ulcers at a single center, developing models in a 2019-2020 cohort and temporally validating them in a later 2024 cohort. The outcome was in-hospital major lower-extremity amputation above the ankle. Candidate predictors were routinely available admission variables within 24 hours, including comorbidities, bedside limb/ulcer assessme

Diabetes mellitus (DM) is the leading cause of non-traumatic lower extremity amputations in the USA. After these amputations, patients exhibit reduced mobility and increased energy demands of walking. The best surgical practice is to preserve as much of a functional limb as possible, in part due to the fact that proximal amputations result in a greater increase in energy expenditure compared to more distal amputations. While differences in transfemoral, transtibial and partial foot amputation levels have been previously documented, no studies have directly compared transtibial and transmetatar

BACKGROUND: The Lower Extremity Assessment Project (LEAP) and Military Extremity Trauma Amputation/Limb Salvage Study (METALS) reported conflicting results with respect to severe lower extremity injuries treated with limb salvage versus amputation. The LEAP study reported no difference between amputation and limb salvage groups, while the METALS study reported improved outcomes with amputation. The purpose of this study was to re-evaluate the METALS data to determine whether the ankle/hindfoot injuries were the main driver of the results of improved outcome with amputation.
METHODS: This is a

BACKGROUND: From traumatic injury to diabetic foot infections, minor amputations of the foot and ankle rely on viable and adequate soft-tissue coverage. When employing a plantar foot flap, coverage often determines amputation level. This study examined the relative plantar foot flap length needed to cover various levels of minor foot amputations.
METHODS: Demographic data and plantar flap length of ten cadaver specimens were recorded. The minimum plantar flap length needed to cover transmetatarsal, Lisfranc, and Chopart amputations was measured following each amputation. Absolute and percenta

In cases of traumatic major extremity amputations, particularly in the lower extremity, every stage-ranging from decision-making to implementation and outcomes-remains a topic of debate. Managing major amputation cases with high mortality rates is extremely challenging both during the injury and treatment processes. We present a rare and severe case of a 30-year-old patient who sustained injuries from a concrete machine, resulting in bilateral lower extremity amputations and a unilateral proximal arm amputation. We successfully performed a replantation at the proximal ankle level on one side.

IntroductionOpen pilon fractures represent a challenging injury for both patients and physicians. Patients must contend with prolonged non-weightbearing, wound complications, and the possibility of post-traumatic arthritis. Surgeons also deal with a fracture which is difficult to treat and that is prone to poor outcomes. To better understand these challenging injuries and identify details that may portend better patient outcomes, we performed a retrospective review of open pilon fractures treated at a level 1 trauma county hospital by orthopedic trauma-trained surgeons.Materials and MethodsA s

INTRODUCTION: Amputation leads to a permanent disability and brings a dramatic change in the life and function of the individual, more so in individuals with lower limb amputation. A lower limb amputation reduces mobility and can make persons dependent on assistive devices like crutches or a wheelchair. Restoring mobility and optimal physical functioning of an individual with lower limb amputation is the most important rehabilitation goal. There are very few studies that have quantified mobility deficits with valid outcome measures, especially in the Indian population. Our study aims to quanti

BACKGROUND: Knowledge about factors associated with long-term outcomes, after severe traumatic injury to the lower extremity, can aid with the difficult decision whether to salvage or amputate the leg and improve outcome. We therefore studied factors independently associated with capability at a minimum of 1 year after amputation or free flap limb salvage.
METHODS: We included 135 subjects with a free flap lower extremity reconstruction and 41 subjects with amputation, between 1991 and 2021 at two urban-level 1 trauma centers with a mean follow-up of 11 ± 7 years. Long-term physical functioni

BACKGROUND: Tibiotalocalcaneal arthrodesis (TTCA) is a salvage procedure to fuse the ankle and subtalar joints to treat severe ankle and hindfoot disease. Patients often have multiple operations before a TTCA. A below knee amputation (BKA) has major physical and psychological impacts. Our aim is to investigate the outcomes of these treatments in salvage situations, with a focus on objective functional measurements and patient reported outcome measures (PROMs).
METHODS: 52 patients underwent TTCA with a retrograde intramedullary nail and contacted for clinical and functional assessments and co

New technologies in additive manufacturing and patient-specific CT-based custom implant designs make it possible for previously unimaginable salvage and limb-sparing operations a practical reality. This study presents the design and fabrication of a lattice-structured implant for talus replacement surgery. Our primary case involved a young adult patient who had sustained severe damage to the talus, resulting in avascular necrosis and subsequent bone collapse. This condition caused persistent and debilitating pain, leading the medical team to consider amputation of the left foot at the ankle le

BACKGROUND: Peripheral artery disease (PAD) and diabetic peripheral neuropathy (DPN) are two of the leading causes of non-traumatic amputation worldwide with tremendous negative effects on the quality of life, psychosocial well-being of persons with diabetes mellitus; and a great burden on health care expenditure. It is therefore imperative, to identify the common and contrast determinants of PAD and DPN in order to ease adoption of common and specific strategies for their early prevention.
METHODS: This was a multi-center cross-sectional study which involved the consecutive enrolment of one

BACKGROUND: More than 86,000 Americans with type 2 diabetes mellitus (T2DM) undergo nontraumatic lower-extremity amputations annually. The opioid-prescribing practice of podiatric surgeons remains understudied. We hypothesized that patients with T2DM who undergo any forefoot amputation while using antidepressant medication will have reduced odds of using opioids beyond 7 days.
METHODS: We completed a retrospective cohort study examining patients with T2DM who underwent forefoot amputation (toe, ray, transmetatarsal). Data were restricted to patients with a hemoglobin A1c level less than 8.0%

Whether secondary to deformity, traumatic injury, infection, neoplasm, or ischemic disease, the transmetatarsal amputation provides a functional means of limb preservation prior to major proximal amputation. With similar readmission rates following inpatient and outpatient surgery, prevention of an unnecessary admission among vulnerable patients, specifically geriatrics, proves beneficial. This investigation examines differences among geriatric patients admitted and not requiring readmission following outpatient transmetatarsal amputation. An ACS NSQIP database analysis following filtering for

BACKGROUND: Although chronic kidney disease (CKD) has been associated with poor outcomes following traumatic fractures, there is a scarcity of literature describing the effect on outcomes of ankle fractures. We will examine the impact of CKD on clinical outcomes following torsional ankle fracture, including complications and unplanned surgical procedures.
METHODS: A retrospective review of 1981 adult patients with torsional ankle fractures treated at a level 1 trauma center was performed to identify patients with CKD based on glomerular filtration rate. Demographic, injury, and treatment-rela

BACKGROUND: Severe traction injury to the brachial plexus (BP) during childbirth can induce the formation of intraneural scar tissue or neuroma in continuity. This can lead to nerve entrapment and demyelination, which inhibit axonal transport to the target muscles. Secondary complications include muscle weakness, contractures, joint deformity, and altered limb growth with prolonged and persistent disability. These patients find difficulty performing activities of daily living. BP neurolysis is one of the multistage reconstructive procedures in patients with obstetric brachial plexus injury (OB

BACKGROUND: There is limited available information to guide early discussions involving limb salvage for patients with non-traumatic foot ulcers. We hypothesized patient, wound and treatment factors identifiable at initial operative treatment would be associated with failure of attempted limb salvage.
METHODS: We retrospectively assessed United States military veterans treated operatively for non-traumatic foot ulcers at a Veteran's Administration (VA) hospital from 2008 to 2018. Cox proportional hazard analysis assessed for independent associations with eventual above ankle amputation.

AIM: Prescribing optimal prosthetic feet to ensure successful rehabilitation is difficult since there are no generally established clinical guidelines based on objective data. The aim of the study was to compare functional capacity, pain intensity, satisfaction level and quality of life (QoL) of high activity patients with unilateral transtibial amputation using non-articulated carbon foot (non-articulating ankle, NAA) with those of using carbon foot with hydraulic ankle (articulating hydraulic ankle, AHA).
METHODS: Forty-two patients (21 with NAA and 21 with AHA) with unilateral transtibial

INTRODUCTION: Hindfoot deformities in the pediatric population can be painful and result in severe limitations. Although arthrodesis is known to relieve pain, there are concerns over its use because of the risk that adjacent joint degenerative disease could result, leading to a new source of pain, dysfunction, and additional surgical procedures later in life.
METHODS: A literature review of hindfoot fusions in children focused on articles with the highest levels of evidence and those of particular historical significance. Case examples were obtained by querying the billing records of the loca

Reconstruction of large bone defects of the metatarsals, whether resulting from trauma, infection, or a neoplastic process, can be especially challenging when attempting to maintain an anatomical parabola and basic biomechanical stability of the forefoot. We present the case of a 42-year-old man with no significant medical history who presented to the emergency department following a severe lawnmower injury to the left forefoot resulting in a large degloving type injury along the medial aspect of the left first ray extending to the level of the medial malleolus. The patient underwent emergent
