The aim of this retrospective study was to evaluate factors potentially influencing short- and long-term mortality in patients who had a non-traumatic lower limb amputation in a university hospital. A consecutive series of 93 amputations (16% toe/foot, 33% trans-tibial, 9% through knee and 42% trans-femoral) were studied. Their mean age was 75.8 years; 21 (23%) were admitted from a nursing home and 87 (92%) were amputated due to a vascular disease and/or diabetes. Thirty days and 1-year mortality were 30 and 54%, respectively. Cox regression analysis demonstrated that the 30-day mortality was

BACKGROUND: Thumb amputations proximal to the metacarpophalangeal joint inevitably result in destruction of the thenar musculature and secondary loss of opposition. Opposable tripod pinch is one of the essential goals in toe-to-thumb reconstruction. Pronation osteosynthesis is the traditional method of restoring opposition, but a simultaneous opponensplasty may turn this static process into a dynamic one.
METHODS: From 1992 to 2010, 19 toe-to-thumb transfers at and proximal to the metacarpophalangeal level with concomitant thenar muscle damage were examined. All underwent either pedicled groi

Purpura fulminans (PF) is a rapidly progressing, potentially life-threatening condition characterised by disseminated intravascular coagulation (DIC) and haemorrhagic infarction of the skin. Plastic surgical treatment of PF has never been reported in Scandinavia. The aim of this report was to review plastic surgical treatment of PF and the long-term results of two patients treated in our department. Both patients presented to a community hospital with skin lesions looking like simple traumatic skin bleeds a period after a Varicella infection. They were initially treated at the community hospit

A traumatic amputation of a digit as a result of canine mastication and ingestion occurred in a 48-year-old woman with type 2 diabetes and peripheral neuropathy. The injury occurred during sleep and was not felt by the patient. The dangers of sleeping with one's canine for those with neuropathic wounds are presented, and the literature is reviewed.

Amputation of all or most of the fingers severely disturbs the gripping function of the hand. The purpose of this study was to evaluate the long-term functional results of finger-amputation patients rehabilitated with microvascular toe transfers. Fifteen such patients (10 males, median age at injury 26 years (range 5-49 years)) were examined after a median follow-up of 18 years. Eight patients had no fingers spared by the initial trauma and the rest had at least two fingers amputated. The function of the hand was accessed subjectively (questionnaires) and objectively (tests). Further, physical

Results of grip reconstruction in two patients after severe crush-avulsion injuries of the hand were presented. Total destruction affected the first or the first and the second ray of the hand which was accompanied by soft tissue loss, finger amputations and complex bone fractures with vessels and nerves damage. Soft tissue reconstructions were performed using various methods. Pedicled groin flap followed by nerves reconstruction and tenolysis were done in one patient. In the second case soft tissues were rebuilt with distally pedicled adipo-cutaneous forearm flap supplied by distal radial art

The purpose of this study was to evaluate the long-term functional results of microvascular toe-to-thumb reconstruction after trauma. Forty-one patients meeting the inclusion criteria were available for a clinical follow-up study. The function of the hand was assessed with questionnaires as well as with modified Tamai and Sollerman hand function tests. According to the questionnaires, most activities were considered easy or quite easy and the majority of the patients (36/41) managed with no or minor complaints. Clinical tests showed good recovery of function. Patient satisfaction was high. The

BACKGROUND: Second toe-to-thumb transfer is a good alternative to using the great toe for reconstruction of the thumb. It achieves excellent function and reduces morbidity to the donor foot. However, cosmesis is often poor. The second toe has three unattractive features, a narrow "neck," a bulbous tip, and a short nail.
METHODS: The authors describe a modified second toe transfer that addresses cosmesis in six patients. These include (1) harvesting a flap from the adjacent side of the great toe and insetting it into the volar aspect of the second toe to give more bulk, (2) making skin excisio

BACKGROUND: Reports in the media suggest that escalator-related foot injuries are on the rise. Trendy, bright-colored rubber clogs have been implicated in a significant number of these incidents involving children. We review the children who sustained severe foot injuries on escalators, were wearing rubber clogs at the time of injury, and who were admitted to hospital for emergency surgery.
METHODS: A list of children who sustained foot injuries on escalators was generated from the hospital database and included for study. From clinical chart review, demographic data, footwear type, and injur

The purpose of this study is to describe the harvesting technique, anatomic variations, and clinical applications of a compound flap from the great toe and vascularized joint from the second toe used for thumb reconstruction. Five fresh cadaver dissections were studied, focusing attention on the dorsal or plantar vascular dominance, position of the communicating branch between the dorsal and plantar system, the Gilbert classification, and the size of the first dorsal metatarsal artery (FDMA) and first plantar metatarsal artery (FPMA) to the great toe and second toe. Five compound flaps were pe

BACKGROUND: When replantation of an avulsed/amputated thumb is not feasible, toe-to-hand transfer may be considered as a reconstructive option in appropriately chosen patients. Although selection criteria are purposefully restrictive, immediate one-stage transfer, as opposed to a delayed procedure, provides many advantages. Primary reconstruction reduces hospitalization and operative and recovery time. It also may expedite return of function and allow patients to return to work sooner. The ability of the patient to undergo extensive microvascular reconstruction at the time of injury, the psych

The objective of this study is to review a single institution's 10-year experience of simultaneous double second toe transplantations for reconstruction of traumatic hand injuries. Eleven cases of traumatic hand injuries treated with simultaneous double second toe transplantation for digital reconstruction were retrospectively reviewed. All patients sustained traumatic injury resulting in multiple digit loss not amenable to replantation. A simultaneous three-team approach was performed in all cases. The average operating time was 9 hours (range 7 to 15 hours). The mean time to reconstruction w

In this case of a 3-year-old patient who had a left big-toe amputation through the proximal phalange, re-plantation was performed successfully. Big-toe amputations are not frequent entities, but it should always be re-planted to avoid unsatisfactory aesthetic and functional outcomes. There are just a few reports in literature and their follow-up is very brief compared with our report.

The aim of this retrospective study was to evaluate the functional results of grip reconstruction after metacarpal amputation with microvascular toe transfer or transfers. The Sollerman hand function test and modified Tamai score were determined. Additionally, secondary objective outcomes were measured. As subjective outcomes, the patients were asked about pain, satisfaction, sensibility and activities of daily living (ADL). Complications and secondary operations were also recorded. An average of 12 years 5 months (range 11-270 months) follow-up of 8 patients (11 transfers) is presented. Solle

One-stage osteoplastic reconstruction of the thumb is based on a reverse-flow forearm osteocutaneous flap. It is indicated in traumatic loss of the thumb at or around metacarpophalangeal joint level in situations where replacement with a toe is not feasible or patient desires against such a transfer. It is relatively an easier method of thumb reconstruction and does not require microsurgical expertise. Donor-site morbidity is small. Complications include delayed wound healing at donor site, radius fracture, and venous congestion. This technique is a useful reconstruction option for traumatic l

Deafferentation such as the amputation of a body part causes cortical reorganization in the primary motor cortex (M1). We investigated whether this reorganization is reversible after reconstruction of the lost body part. We tested two patients who had long-standing thumb amputations followed by thumb reconstruction with toe-to-thumb transfer 9 to 10 mo later and one patient who underwent thumb replantation immediately following traumatic amputation. Using transcranial magnetic stimulation, we measured the motor evoked potential (MEP) threshold, latency, short-interval intracortical inhibition

Between 2002 and 2008 author treated 6 patients aged 15-40 (mean 24 years), after severe hand injuries reconstructing soft tissues of the first ray with various flaps based on perforators of distal part of radial artery. All patients requested secondary thumb reconstruction due to its amputation and disfigurement of the hand (particularly tissues of the first ray). Following three different forearm flaps based on radial artery perforators were used: adipo-cutaneous, fascio-cutaneous and island fascio-cutaneous. Primary closure of the donor site was possible in 4 cases. In remaining two patient

BACKGROUND: Mutilating hand injuries result in bone and soft-tissue defects. Osteocutaneous flaps can be used for osteoplastic thumb ray reconstruction as a definitive procedure or followed by toe transfer.
METHODS: Twenty-four hands with thumbs amputated proximal to the metacarpophalangeal joint underwent osteoplastic surgery for first ray bone and soft-tissue reconstruction. The Kapandji classification was used for the evaluation of basic hand function (opposable ulnar and radial components) after osteoplastic surgery and after toe transfer.
RESULTS: Thirteen of 24 osteoplastic neothumbs (

We present two cases of degloving injuries of the thumb with amputation of the distal phalanx. The first one was treated with an osteocutaneous flap from the first toe, giving effective function to the thumb, and the second by a fasciocutaneous interosseous flap around the segment of the exposed thumb, with a satisfactory result.

BACKGROUND: Thumb replantation following thumb amputation is the standard of care. When replantation is impossible, microneurovascular great toe transplantation is a well-established option.
METHODS: A retrospective review was conducted to evaluate functional outcome following isolated thumb replantation or great toe transplantation for thumb reconstruction. From 1974 to 1993, 384 thumb amputations were treated and 110 great toe-to-thumb transplantations were performed.
RESULTS: Ninety-one patients with isolated thumb amputation had an 85% survival rate. Failed replants usually resulted from
