Simultaneous multiple toe transfers in hand reconstruction.

Our experience with simultaneous transfer of two or more toe units to the same hand where multiple digits were missing is presented. Forty-six toes from 38 feet were transferred to reconstruct 19 hands in 19 patients. The transfers consisted of 7 combined second and third toe units and 32 single toes. Three patients had a primary and 16 patients had a secondary reconstruction. There was one complete and one partial failure. The two-point discrimination ranged from 6 mm to protective sensation. Total active movement averaged 57 degrees in the thumb and 127, 93, 71, and 68 degrees, respectively,

The wraparound procedure for thumb and finger reconstruction.

Nineteen thumb and finger reconstructions were performed using the wraparound procedure; there were five early arterial failures. Thirteen of the 14 successful digital reconstructions were reviewed at an average of 26 months after surgery. There were complications related to the upper limb in seven patients, several of whom had more than one problem; these included radial skin breakdown (2), malrotation (2), and infection (2). One patient underwent late deletion for chronic infection following corrective osteotomy. Time until toe donor site healing averaged 2 months. We considered that cosmesi

Preoperative and postoperative gait analyses of patients undergoing great toe-to-thumb transfer.

Microvascular transfer of the great toe is a reliable method of thumb reconstruction for many patients, but the precise deficit incurred by elective amputation of the great toe has not been fully defined. Twelve patients who had this procedure were evaluated with preoperative and postoperative gait analyses and clinical examinations. The mean age was 29.3 years, with a mean clinical follow-up of 30.3 months, and a comparative follow-up gait study at a mean of 21.1 months postoperatively. Only one patient exhibited a significant motion change postoperatively, which was a reduction in ankle plan

[Toe transfer to a mid-hand stump covered with abdominal skin].

Following amputation of all four fingers from the hand, the cover of the amputation site with an abdominal flap gives a hand which is functionally inferior. This paper reports our experiences of three toe transfers to such a hand in two patients, one a 22-year-old man, the other, a 50-year-old woman. In our opinion, the toe transfer will be successful if the vessels, nerves and tendons are harvested to the level of the extensor retinaculum, giving a length in excess of the skeleton of some 12-15 cm. The routing of these structures beneath the abdominal flap is done in such a way as to avoid an

Second toe to thumb transfer with extensor digitorum brevis opponensplasty.

A second toe to hand transfer is described for thumb reconstruction which includes the extensor digitorum brevis as a simultaneous opponensplasty. This procedure was effective in a young man injured in a bomb explosion that completely destroyed the first ray. The second toe was dissected in continuity with the extensor digitorum brevis muscle, its nerve was anastomosed to the motor branch of the median nerve. Thus a one-stage operation allowed a complete reconstruction of the first ray proximally to the trapezium. Recovery at 2 years is good, and the muscle transfer can be demonstrated to work

[Toe transfer].

Today toe-to-hand transfer by micro-anastomoses is one of the most important methods in reconstructing severely injured hands. An experienced surgeon will obtain a survival rate of the transplant in about 94 to 96% of cases. All measures required can be performed by only one operation. The free toe-to-hand transfer guarantees the best functional and aesthetic results so far known.

Thumb reconstruction pollicisation or toe-to-hand transfers. A comparative study of functional results.

A statistical study of thumb reconstruction after traumatic amputation, based on the records of 33 pollicisations and 21 microsurgical toe-to-hand transfers was made. A precise method of evaluation of results is presented. The types of lesions are divided into 4 groups; 1: amputation of the thumb without injuries of the other fingers; 2: amputation of the thumb associated with amputations or mutilation of other fingers; 3: metacarpal hand; 4: amputation of the thumb distal to the metacarpophalangeal joint. The aim of this study was to determine the respective indications of the two methods. It

[Free transplantation of the 2d toe. Technical features, problems and solutions].

On the basis of seven transfers of the second toe and one "en bloc" transfer of the second and third toe, technical features are discussed. The use of a plate for osteosynthesis is proposed. It has been found very useful, particularly when a MP-arthrodesis is performed between the first metacarpal and the second toe. It gives stability and therefore secondary displacement and/or rotation of the toe are avoided. It enables more accurate tension to be placed on the repaired tendons. For the flexor tendons it is suggested to repair the communis longus only, whereas both extensors, longus and brev

[Anatomical variations in blood vessels and their effect on the technic in toe transfer].

In the absence of grasp, either traumatic or congenital, free transfer of the second toe to the hand is one of the reconstructive possibilities. An essential prerequisite for the acceptance of the transferred toe is a guaranteed blood supply through the vascular pedicle. The blood supply of the second toe can be either by dorsal or plantar arteries, a fact which is due to anatomic variations. Through preoperative angiographic examination the arterial blood supply of the second toe can be defined. The location and length of the vascular pedicle have to be considered in the design of the anastom

[Free toe transfer to the forearm stump following wrist amputation--a current alternative to the Krukenberg operation].

A new and simplified technique for reconstruction of the gripping ability after total loss of the hand is introduced. Previous techniques have not gained popularity because of the rather complicated procedures trying to extend the stump with two additional toes. Poor skin in the distal end of the stump has been a limiting factor and technically transplantation of two separate toes has been a difficult procedure. The stability in such a grip may be compromised. In the author's modification only one toe is used with an island flap from the big toe. The opposing post in this grip is the stable re

The choice of procedure following thumb amputation.

The attributes that make the thumb unique are position, stability, strength, length, motion, sensibility, and appearance. Of these qualities, the first four must be present to an acceptable extent for function to approach normality, while the latter three are very desirable but not essential. Reconstructive alternatives following amputation can be considered in four broad groups: where the length is acceptable but the covering is poor; subtotal amputation, where length is equivocal; total amputation with the basal joint preserved; and total amputation with the basal joint destroyed. In the fir

Bilateral hand reconstruction: report of three cases.

Although it has been established that a single lost hand can be reconstructed by autogenous bilateral toe transplantation, the problem of how to reconstruct bilateral hand loss still remains. The authors present a novel approach to solve this intricate problem. A big toe free skin-nail flap, along with the second digital ray or second and third digital rays, with a common vascular pedicle, is taken from the donor foot and transferred to the forearm stump by microsurgical technique, thereby creating a hand with two or three digits. Either a piece of the iliac bone or an ulna block cut during th

Transplantation of severed digits to forearm stump for restoration of partial hand function.

Three cases of mutilating injury of the distal end of the forearm, wrist, and palm treated by transplantation of severed digits to the forearm stump are reported. Follow-up examinations made at 1 year and 4 months to 31/2 years postoperatively revealed fair sensory and motor functions. The functional result is better than that obtained after Krukenberg's operation or prosthesis fitting, and is comparable to that of "hand" reconstruction by autotransplantation of toes. Since this procedure can fulfill the basic requirements of hand function by reconstruction, namely, good sensibility; basic mot

[Growth behavior of microvein and artery grafts].

In an experimental study with young rats the authors were able to prove that arterial and venous grafts show almost normal growth. With respect to patency rate, growth and integrity of the vessel wall the results with arterial grafts were more satisfactory than those with venous grafts. Examination of thirty-four patients who had undergone replantation or transplantation in infancy or childhood, showed that after digital replantation the extremities grew almost normally and that there was no indication of lag in vessel growth. Three times a slight change in growth after major limb replantation

Time-dependent changes in the functional organization of somatosensory cerebral cortex following digit amputation in adult raccoons.

Surgical removal of the third forepaw digit in raccoons causes both long-term and short-term changes in functional organization within the digit 3 primary somatosensory (SmI) cortex. Previous studies have shown that 36-52 weeks following amputation in infant raccoons, neurons within the digit 3 cortical territory had become responsive to cutaneous stimulation of "new" forepaw regions adjoining the digit stump (Carson et al., 1981; Kelahan et al., 1980, 1981); the "novel" receptive fields (RFs) were often larger than normal and revealed no orderly somatotopic organization. In the present study,

[Reconstruction of grip function in cases of a fingerless or missing hand].

In two cases a grip reconstruction by double toe-to-hand transfer is demonstrated. In the first patient all fingers of the left hand were amputated at the metacarpo-phalangeal joint level. The second and the third finger were reconstructed by transplantation of the second toe from the left foot and the third toe from the right foot. The distal and middle phalanx of the transplanted third toe had to be re-amputated because of mummification two weeks after the transplantation and was covered with a groin flap. Good function of the newly formed grip could be demonstrated. The second patient was a

Free nail bed graft for treatment of nail bed injuries of the hand.

Free full-thickness grafts of nail bed of the lesser toes or an amputated fingertip were successfully performed on 11 fingers of 10 patients since 1979. In nine patients in whom the nail beds had been severely crushed or lost, but the nail matrix was intact, the end results of this technique were excellent. In one patient in whom both the nail bed and matrix had been lost, free grafting of the toenail bed and matrix was performed, with a good result. The procedure can be used when restoring the length of the tip in fingertip amputation if used in combination with local skin flaps such as V-Y a

A free total toe phalanx transplant to a finger: a case report.

A six-year-old boy sustained traumatic loss of the index finger middle phalanx of his right hand and one year after the accident the proximal phalanx of the fourth toe was transplanted to the index finger as a free graft. Four years after the operation x-ray films demonstrate acceptance of the graft and satisfactory function of the finger.

[Continuous plexus anesthesia to improve circulation in peripheral microvascular interventions].

Five patients were demonstrated: one finger replantation, three toe-to-finger transfers and one finger-finger transfer. An axillary plexus catheter was inserted for postoperative sympathetic blockade. This brought about spasmolysis, an increase in the circulation and an increase in the acral systolic blood pressure. Simultaneously an adequate analgesic effect was achieved through the administration of local anaesthesic. The muscle relaxation assisted the immobilisation of the extremity. The improved circulation, the analgesic effect and the suppression of the muscular activity through continuo

The article reports on x-ray findings in the skeleton of 20 finger replants, one successful arm replant and two successful replants of toes to the hand in 23 children aged one to fourteen years. The quota of successful healing together or setting was 80% in finger replants. Setting of the fracture was delayed by an average period of seven days. Osteonecrosis and osteomyelitis were equally frequent in two children each (10%) and resulted in problems of differential diagnosis. In three children the bone fracture set in false position, without, however, causing any complaints. All epiphyses not d
