Symmetric peripheral gangrene and falciparum malaria.

Sudden onset symmetric peripheral gangrene (SPG) is a relatively uncommon clinical entity manifested by distal ischemic damage at two or more sites in the absence of large vessel obstruction. Here we report a case of complicated falciparum malaria with rapid onset SPG involving all the toes.

[Primary Swanson metacarpophalangeal joint arthroplasty in major hand traumas. A report of two cases at 14 years follow-up].

In major hand injuries, destroyed joints may be treated by joint transfer, arthroplasty or fusion with or without temporary external fixation. Four immediate digital metacarpophalangeal joint Swanson implants in two patients with amputation or devascularizated near-amputation were reviewed with a 14 years follow-up. No infection was noted. Prostheses were stable with a 31 degrees active range of motion and grip strength was 42% of the opposite side. Functional results were "useful" with patients satisfaction. In one patient, heavy manual worker, implants were replaced fourteen years after trau

Distal site testing of sympathetic skin response (big toe) in diabetic polyneuropathy.

The aim of this study was to determine whether the sympathetic skin response (SSR) recorded from the big toe is more sensitive than standard SSR recorded from the sole for the detection of sudomotor fiber dysfunction in diabetic neuropathy. We recorded big toe SSR (SSRBT) and plantar SSR (SSRP) in 17 diabetic patients with non-disabling neuropathy (group A), 13 patients with disabling neuropathy (group B) and 30 age-matched normal controls. With regard to controls, SSRP amplitude was reduced only in group B. In contrast, SSRBT amplitude was reduced in both groups of patients (p<0.0001). In 8 p

A case of systemic sclerosis with abdominal surgery for stenosis of the terminal ileum.

A 71-year-old woman was admitted to our hospital with prominent abdominal distention. The diagnosis of systemic sclerosis (SSc) was made based on the sclerosis on her face, forearms, fingers, and toes as well as pitting ulcers on fingers and toes. After conservative therapy, she presented more severe signs of peritoneal irritation. Abdominal surgery was performed, revealing stenosis at the terminal ileum. Pathologically, this region showed muscularis propria becoming fibrotic. She was discharged from the hospital, returning about once a month as an outpatient.

Differential modulation of withdrawal reflexes by a cannabinoid in the rabbit.

Inhibition of spinal and trigeminal withdrawal reflexes by morphine and by the cannabinoid agonist HU 210 has been studied in anaesthetized and in decerebrated rabbits. In intact, pentobarbitone-anaesthetized animals, the jaw-depressor reflex (JDR) evoked by stimulation of the tongue, and the reflex elicited in the ankle flexor tibialis anterior (TA) by stimulation of the toes were inhibited to the same extent by morphine (1-30 mg kg(-1) i.v. cumulative). In spinalized, anaesthetized rabbits morphine depressed the JDR to the same level as in non-spinal preparations, but the effect of the opioi

Sensory dysfunction in the great toe in hallux valgus.

Injury to the dorsomedial cutaneous nerve in the foot may occur after operations for hallux valgus. Pressure neuropathy before operation is also described but remains largely unexplored. We have investigated the incidence of sensory deficit in the great toe before operating for hallux valgus and examined to what extent any deficit was related to the degree of angulation of the joint. Forty-three patients with a total of 61 great toes with hallux valgus presenting for consideration of surgical correction had their sensation tested in pre-designated zones using a five-filament set of Semmes-Wein

Habenular and interpeduncularis nuclei: shared components in multiple-function networks.

STUDY AIMS: Advance Directives (AD) are central to Advance Care Planning (ACP), allowing patients to document medical preferences in legally binding documents. Despite broad public awareness and support, only one-fifth of the Swiss population has completed an AD. Consequently, the roadmap for ACP implementation by the ACP National Task Force calls for the development of digital applications to promote ACP in Switzerland. This study details the development process for the mobile application "iDecide" for AD creation based on the Swiss Medical Association (FMH) AD template. The primary aim is to

Klinefelter's syndrome presenting with leg ulcers.

A 54-year-old man of Persian origin presented to our department with a 1-year history of ulcers on the right leg that had been unresponsive to numerous topical treatments, accompanied by lymphedema of the right leg. Medical history included hypergonadotropic hypogonadism, which had not been further investigated. He was treated for 20 years with testosterone IM once monthly, which he stopped a year before the current hospitalization for unclear reasons. The patient reported no congenital lymphedema. Physical examination revealed two deep skin ulcers (Figure 1) on the right leg measuring 10 cm i

The force-driven harmonic oscillator model for energy-efficient locomotion in individuals with transtibial amputation.

The force-driven harmonic oscillator (FDHO) model states that the driving force is minimum at the resonant period of an oscillator. By manipulating prosthetic mass, this study explored the compromise of resonant periods between the two legs in persons with unilateral traumatic transtibial amputation (TTA) at self-selected walking velocity (SSWV), with an aim to better understand the energy minimization mechanisms of walking. It was hypothesized that (1) SSWV was the most energy-efficient walking velocity (MEWV), (2) the stride period at SSWV (Ts) is a compromise between the resonant periods of

Bilateral lower leg replantation versus prosthetic replacement: long-term outcome of amputation after an occupational railroad accident.

BACKGROUND: This retrospective study investigated three very similar cases of bilateral lower leg amputation. The aim was to determine which of two therapeutical procedures is associated with better long-term outcome: replantation or primary treatment of the stumps and subsequent prosthetic replacement. METHODS: Evaluation included clinical examination, gait analysis, and a workup of the psychosocial background. Health problems were documented using the Nottingham Health Profile. Follow-up assessments were performed 6, 7, and 18 years after the trauma. RESULTS: One patient underwent successf

Traumatic injuries of the popliteal and tibial arteries: limb ischemia and problems of surgical management.

This paper analyzes the results of examination of 106 patients with traumatic injuries of the popliteal (n=36) and leg arteries (n=70) including gunshot (n=48), punctured and incized (n=32), contused-crushed (n=17) wounds and blunt (n=9) traumas. Sixty-two patients were admitted within 8 hours of injury and 20 within 8 to 24 hours, 24 patients within 1 to 15 days. Sixty-five patients had been rendered first aid at other medical institutions. It is to be noted that the prehospital diagnostic and tactic errors had been made in 67.7% of cases. In the patient group with injuries of the leg arterie

Vascularized nerve grafts for the treatment of large nerve gap after severe trauma to an upper extremity.

We reviewed the clinical outcomes of vascularized nerve grafts for the repair of large nerve gaps (longer than 20 cm) after severe trauma to an upper extremity. Six patients who underwent vascularized sural nerve grafting (five to the median nerve and one to the ulnar nerve) with a monitoring skin flap were evaluated. The length of the vascularized sural nerve grafts ranged from 20 to 30 cm, with a mean length of 23.3 cm. All but one of the monitoring skin flap grafts was successful. In those patients for whom the monitoring skin flap graft was successful, the mean static-2PD at the correspond

Free fillet flap of the hand for elbow preservation in nonreplantable forearm amputation.

The use of non-replantable amputated parts for reconstruction of the stump is a well-established technique. The use of a free fillet flap of the hand and forearm for elbow preservation in massive trauma of the upper extremity is reported in two cases. These free flaps allowed for covering and preserving a functional elbow and a more useful stump.

Single centre experience of the upper limb replantation and revascularisation.

Replantation is defined as reattachment of the amputated limb using the neurovascular and musculoskeletal structures in order to obtain the recovery of the limb. Fortunately, injuries causing limb amputation are rare. Adequate treatment within the optimal time scale can provide successful rehabilitation of the shape and function of the replanted part. We report the experience of our Clinical Centre (regional replantation centre) in the replantation of five forearms/hands and revascularisation of six hands between 1997 and 2001. The most frequent site of injury was the distal part of the forear

Sixty-five clinical cases of free tissue transfer using long arteriovenous fistulas or vein grafts.

Traumatic limb injuries requiring free tissue transfer for coverage, often lack healthy recipient vessels adjacent to the defect. In these patients, vein grafts are required to bridge the gap of either the artery, vein or both. For the latter situation, a temporary arteriovenous fistula (AVF) can be created and allowed to mature and then divided and used as recipient artery and veins for the free flap. These cases are challenging and several variables including vein graft length, vein graft diameter, and arterial inflow affect the patency of the vessels and the final outcome of the reconstruct
