[A preliminary report of two cases of human hand allograft].

OBJECTIVE: To study the feasibility of reconstruction of amputation by hand allograft in human being. METHODS: Two male recipients with traumatic right wrist amputation for 2 years, were matched respectively to two ABO- and Rh-compatible, HLA- half mpatible brain-dead donors. Direct crossmatch was performed to confirm the absence of prior sensitization to alloantigens. After amputation the donor's arm was irrigated with UW organ preservation solution at 4 degrees, and transported in a box with ice. One of the two donor arms was randomly selected and irradiated by 8 gy x-ray before transplanta

Thumb reconstruction with a wrap-around free flap according to the level of amputation.

In 1980, Morrison and O'Brien reported their experiences with the reconstruction of an amputated thumb using a wrap-around neurovascular free flap from the great toe, but its indication has been limited distal to the metacarpophalangeal (MP) joint (Morrison et al., J Hand Surg 5:575-583, 1980). We have performed 37 wrap-around free flaps from the great toe for the reconstruction of thumbs amputated at distal or proximal to the MP joint and investigated their functional results according to the level of amputation. The amputation was distal and proximal to the MP joint in 25 and 12 cases, respe

Microsurgical toe to thumb transplantation for traumatic thumb loss.

Toe-to-thumb transfer was performed on 13 patients with traumatic thumb loss, using microvascular technique. Of those, 9 patients had industrial injury, 3 patients had a sharp cut injury and the remaining one had animal bite injury. The toes used for the transfer were the great toe(5), and the second toe(8). There were twelve successes, and one partial success. Ten patients have been followed-up for more than 12 months, and they are reviewed in detail. Total active motion after great toe and second toe transfer was 45 degrees and 68 degrees respectively. Static two-point discrimination was 10-

[Composite nail transfer by the traditional method].

INTRODUCTION: We have retrospectively reviewed our experience in the treatment of traumatic loss of the whole nail complex. METHOD: Contraindications for our technique are cases suitable for microsurgical transfer and cases with a poorly vascularised recipient bed. Under local anaesthesia, we harvested the nail unit composed of the eponychiurn matrix, nail table, hyponychium and lateral folds. The transfer to the recipient finger was also performed under local anaesthesia. Post operative cryotherapy for 72 hours was combined with a prostaglandin E1 infusion (50 micrograms per day during 3 day

Should the amputations of the great toe be replanted?

Seventeen great toes, amputated at the distal phalangeal to the level of the MTP joint, were replanted between 1990 and 1998, at Izmir Hand and Microsurgery Hospital. Replantation in five out of six complete amputations, and seven out of eleven incomplete amputations were successful, and the overall survival rate was 76.4%. In failed replantations, the base of the proximal phalanx of the great toe was preserved during closing of the stump. Nine of 17 patients were available for review in the follow-up period of mean 3.5 years (range 1-6.5 years). Clinical and biomechanical evaluations of the o

Use of the distally-based radial forearm flap supplied by the dorsal carpal arch, or palmar carpal arch, or both, in mutilating injuries. Two case report.

The distally-based radial forearm flap is safe, easy, and versatile as a regional, one-stage procedure to reconstruct soft tissue defects of the hand. However, there is a general perception that the deep and superficial palmar arches have to be intact to raise a distally-based radial forearm flap. We successfully used two flaps supplied by the dorsal carpal arch, or the palmar carpal arch, or both, despite the fact that the deep and the superficial palmar arches were damaged in mutilating injuries.

Are triphalangeal thumb-polysyndactyly syndrome (TPTPS) and tibial hemimelia-polysyndactyly-triphalangeal thumb syndrome (THPTTS) identical? A father with TPTPS and his daughter with THPTTS in a Thai family.

We report on a Thai man who had triphalangeal thumb-polysyndactyly syndrome (TPTPS, MIM *190605) and his daughter who had tibial hemimelia-polysyndactyly-triphalangeal thumb syndrome (THPTTS, MIM *188770). The father had polysyndactyly of triphalangeal thumbs, syndactyly of fingers, duplicated distal phalanx of the left great toe, brachymesophalangy of toes, and the absence of middle phalanges of some toes. He was diagnosed as having TPTPS. His daughter was more severely affected, having complete syndactyly of five-fingered hands in rosebud fashion (Haas-type syndactyly), hypoplastic tibiae, a

Suppression of polydactyly of the Gli3 mutant (extra toes) by deltaEF1 homozygous mutation.

Digit patterning is established through multiple genetic interactions. Delta-crystallin enhancer/E2-box factor (deltaEF1) is a zinc finger and homeodomain containing repressor protein, and is expressed in the posterior half of the forelimb bud and in the entire hindlimb bud during the early stage of limb development. The 6EF1-deficient mutant mice display various skeletal abnormalities, among which inferior ossification and abnormal patterning of autopodial bones are similar to those observed in Hox and Gli gene mutants. Gli3 mutant mice, extra toes (Xt), exhibit pre-axial polydactyly losing t

[Classification and treatment of symbrachydactyly. A series of 117 cases].

In the present study, a modification has been proposed of the Blauth and Gekeler classification, aimed at a more accurate definition of appropriate surgical treatment. An analysis was made of a series of 120 cases of symbrachydactyly (117 patients); however, surgery was only performed in 86 cases (51 toe transfers in 49 patients; mean age at surgery 12 months). Type I included the separation of short and sometimes stiff fingers; type II, the 'pseudo-cleft', could be subdivided into three groups. Type IIA included those hands with more than two long and frequently hypoplastic digits, regarding

The donor foot in free toe or joint transfers.

Eighty-four cases of free second toe, multiple toe or second metatarsophalangeal joint transfers were studied by case review and follow-up. The function of the donor foot had recovered completely within 6 months in 89% of patients. The wounds on the donor foot healed in 2-3 weeks in 90% of patients. Slight numbness on the dorsal aspect of the donor foot, intolerance to cold, mild reduction in push-off, scar tenderness and pain or swelling occurred in only a few patients and generally were not considered of significance. Multiple toe transfers created more donor problems in terms of healing and

Characterization of an interstitial deletion del(13)(q22q32) using microdissection and sequential FISH and G-banding.

The objective of this study was to delineate a chromosome 13 abnormality and establish its clinical correlation by using molecular cytogenetics procedures. A newborn boy presented with clinical findings, including mild symmetric intrauterine growth retardation (IUGR), small ears with thickened helices, a scalp lesion, short fifth fingers, missing toes, and talipes equinovarus. Routine G-banding of cultured peripheral blood cells revealed that the patient had one abnormal and shortened chromosome 13, but uncertainty remained as to whether the abnormality was the result of an interstitial deleti

[Use of reverse flow pedicled osteo-fascio-cutaneous radial forearm flap in reconstruction of the first ray in the hand].

A reverse flow pedicled osteo-fascio-cutanous radial forearm flap was applied in different clinical situations. Treatment of 1 female patient--age 28 years, and 3 male patient--age 17, 29 and 33 years is presented. Indications for treatment were: congenital malformation of the thenar region and thumb, deficit of the soft tissue, of the IMP joint and a segment of the first metacarpal bone, amputation of the thumb at IMP level and congenital total absence of the first ray of the hand. The author's modification of total thumb reconstruction is presented. Some steps of the procedure, using osteo-f

[Pollicization, remains of the past or current operation].

Pollicization is a long time honored operation but more recent introduction of microsurgical techniques allowing toe to thumb transfers has changed the indications in trauma and congenital malformations. We reviewed two series of patients to assess the long-term result in both traumatized and malformed hands. Twenty-seven pollicized mutilated fingers were reviewed with a mean follow up of 9.5 years; the longest follow up published in the literature. Despite the age quite advanced of the population and multidigital amputations (more than two fingers were amputated in 16 cases), 21 patients had

Laparoscopic radical prostatectomy: preliminary results.

BACKGROUND: Retropubic and perineal radical prostatectomy are used for curative treatment of localized prostate cancer. More complex urological procedures are now being done with laparoscopy. We present our initial results of transperitoneal laparoscopic radical prostatectomy. MATERIALS AND METHODS: Twenty laparoscopic radical prostatectomies were performed between May 1998 and May 1999. The mean age at the time of surgery was 64.2 years. There were 14 stage T1c, 5 stage T2a and 1 stage T2b tumors. The preoperative PSA was 9. 3 ng/ml (normal <4 ng/ml). The Gleason score for positive specimens

Biomechanics of the first ray. Part II: Metatarsus primus varus as a cause of hypermobility. A three-dimensional kinematic analysis in a cadaver model.

Variation in functional stability of the first metatarsocuneiform joint was analyzed between transverse plane deviated (adducted) and corrected first metatarsal positions in a closed kinetic chain model. Six fresh frozen cadaver specimens with intact ankles and feet were fitted with a custom fabricated titanium metatarsal jig, which allowed for manipulation of the first metatarsal in the transverse plane. Specimens were mounted into a custom-made acrylic load frame and axially loaded to 400 N. Radiowave three-dimensional tracking transducers were attached to the following osseous segments: fir

[Early clinical screening in psoriatic arthropathy].

AIM: Because of doubts in early clinical assessment and full documentation of all various cases of psoriatic arthritis outside of rheumatologic centres a clinical study in a dermatologic department of a rehabilitation clinic was performed. METHODS: 148 patients between 1996 and 1999 were interviewed and examined clinically. 27 (18.2%) of them had osseous joint processes proved by X-ray (severe cases). 90 (60.8%) patients had no radiologic changes but a positive history of joint pain (mild cases). 31 (21%) patients had none of the problems (Control group). Blood specimens were taken in order t

The use of osteotomies in the treatment of hallux limitus and hallux rigidus.

Though osteotomies for relief of hallux limitus and rigidus have been around since the earliest surgical corrections, no sound clinical studies have been performed to warrant their use over the standard accepted techniques of cheilectomy and arthrodesis. These operations are surely more technically demanding than such standard procedures, and involve significant increased risk and postoperative immobilization than cheilectomy alone. Sound theories such as metatarsus primus elevatus and excessive metatarsal length contributing to hallux rigidus have never been proven, and no accurate way to dia

Function after correction of a clawed great toe by a modified Robert Jones transfer.

We carried out a cross-sectional study in 51 patients (81 feet) with a clawed hallux in association with a cavus foot after a modified Robert Jones tendon transfer. The mean follow-up was 42 months (9 to 88). In all feet, concomitant procedures had been undertaken, such as extension osteotomy of the first metatarsal and transfer of the tendon of the peroneus longus to peroneus brevis, to correct the underlying foot deformity. All patients were evaluated clinically and radiologically. The overall rate of patient satisfaction was 86%. The deformity of the hallux was corrected in 80 feet. Catchin

Avascular necrosis of the hallux metatarsal head.

Avascular necrosis of the first metatarsal head is rare. Although idiopathic cases have been reported, AVN of the first metatarsal head is usually iatrogenic following surgical correction of hallux valgus using a distal metatarsal osteotomy with or without lateral soft tissue release. A thorough understanding of the delicate vascular anatomy of the first metatarsal head is essential when surgery is considered. Careful operative technique permits a safe combination of distal osteotomy and lateral soft tissue release. Because the intraosseous blood supply is completely disrupted with distal meta
