Symphalangism in children. Case report and review of the literature.

Symphalangism is an uncommon syndrome characterized by fusion of the interphalangeal joints of the hands and feet. The fusion can involve the proximal or the distal joints; however, involvement of the proximal interphalangeal joints is more common. Symphalangism often is associated with several other skeletal and nonskeletal abnormalities. Analysis of the pedigrees of affected families reveals this trait to be autosomal dominant. The authors present the case of a 9-year-old boy with bilateral symphalangism of the proximal interphalangeal joints in the fingers and toes. His father is affected s

Proximal phalanx osteotomy for the surgical treatment of hallux rigidus.

This study reviewed 17 patients (24 great toes) treated by a single surgeon for hallux rigidus with a dorsal-closing wedge osteotomy of the proximal phalanx in conjunction with a moderate cheilectomy. The average age of patients was 47 years (range, 20-69 years). The minimum follow-up was 1 year (range, 1-16 years; median follow-up, 30 months), and all but five patients were studied for 2 years or more. Ninety-six percent of patients affirmed their decision to have surgery if they had to make the choice again. Subjectively, all patients improved. Preoperatively, 67% of the cases had severe pai

[Stewart-Treves syndrome arising in post-traumatic lymphedema].

The authors report a case-history of lymphangiosarcoma or Stewart-Treves syndrome which occurred in a patient with posttraumatic lymphedema of the arm. He presented two recurrences after surgery which led to the decision of amputation. He is now disease free nine years after this treatment. Initially defined as "lymphangiosarcoma arising in chronic lymphedematous extremities after mastectomy for breast cancer", Stewart-Treves syndrome complicates rarely traumatic, postsurgical, postradiation, idiopathic, congenital or filarial lymphedema. Clinical diagnosis is based on nodular, purple and freq

Distal thumb reconstruction with a great toe partial-nail preserving transfer technique.

The great toe partial-nail preserving transfer technique is another modification for distal thumb reconstruction in composite defects at or below the interphalangeal joint of the thumb. Noting the size difference of the nail width between the great toe and thumb, the authors dissected only a thumb nail width and skin flap from the great toe, leaving the remainder of the medial skin flap and nail of the great toe at the donor site. A total of 25 cases between 1993 and 1996 were performed using this technique, and the mean follow-up period was more than 12 months. The reconstructed thumb had a b

Favorable prognosis for children with Pfeiffer syndrome types 2 and 3: implications for classification.

Pfeiffer syndrome (PS) is an autosomal dominant condition comprising bilateral coronal craniosynostosis, midface hypoplasia with a beaked nasal tip, and broad and medially deviated thumbs and great toes. It is a clinically variable disorder and has been divided into three subtypes [Cohen, 1993: Am J Med Genet 45:300-307]. Type 1 represents the less severe cases, while types 2 and 3 are the more severe cases. These latter types tend to have a higher risk for neurodevelopmental problems and a reduced life expectancy. Here we review the clinical course of seven children with PS type 3. All of the

Characterization of vasodilatory adenosine receptors in equine digital veins.

Isolated equine digital veins (EDVs) which had been denuded of their endothelium were used to study adenosine receptors causing vasodilation. When the blood vessel wall tension was raised with the thromboxanemimetic, U44069 (30 nM), the order of vasodilator potency of adenosine receptor agonists was: 5'-N-ethylcarboxamidoadenosine (NECA) > 2-p-(2-carboxyethyl)phenyl amino-5'-N-ethylcarboxamido-adenosine (CGS 21680) > 5'-N-methylcarboxamido-adenosine (MECA) > N6-cyclohexyladenosine (CHA) > N6-cyclopentyladenosine (CPA) > N6-2-(4-Aminophenyl)ethyladenosine (APNEA) > adenosine. Extent of nociceptive dermatomes in adult rats is not primarily maintained by axonal competition.

Nociceptive innervation territories of individual peripheral and spinal nerves in the skin of the rat hind paw were investigated. In addition, the hypothesis that competitive interactions among the axons from adjacent dorsal root ganglia (DRG) play an important role in maintenance of dermatomal extent in adult animals was tested. The area of innervation territories of individual spinal and peripheral nerves was determined by nociceptive pinch test of the skin after extirpation of adjacent DRGs or transection of adjacent peripheral nerves, respectively. Positions of nociceptive dermatomes and i

Cold intolerance is not more common or disabling after digital replantation than after other treatment of compound digital injuries.

Cold intolerance is a common reason for disability after hand injury. In this study of posttraumatic cold intolerance, 20 patients with a history of digital replantation were matched with 20 control subjects who had not undergone replantation. The incidence and intensity of cold-related symptoms among patients in the two groups was investigated through the use of individual interviews and a grading scale for self-assessment of symptoms. The analysis of data indicates that although the pattern of symptoms may vary, the condition is neither more common nor more disabling among those who have und

The feet in Pfeiffer's syndrome.

Broad toes are the classic clinical finding occurring in the feet in Pfeiffer's syndrome patients, but few cases undergo formal radiological assessment. However, the feet in other craniosynostosis syndromes resulting from mutations of the fibroblast growth factor receptor 2 gene have anomalies at many other sites within the feet, which raised the possibility that there may also be a wider range of anomalies other than broad big toes in the feet of those with Pfeiffer's syndrome. The object of this study was to assess prospectively the incidence and pattern of clinical and radiographic anomalie

An immunohistochemical study of palmar and plantar digital nerves.

We present a qualitative and semi-quantitative description of palmar and plantar digital nerves obtained from cadavers prepared for routine class dissection. The fingertips of the three most functionally important and sensitive digits, namely thumb, index and middle fingers, received more myelinated axons than those of the ring and little fingers. In both palmar and plantar nerves the numbers of myelin basic protein (MBP)-positive myelinated axons decreased proximodistally along each digit and also from thumb to little finger and hallux to little toe respectively. Our results suggest that ther

[Clinical evaluation of cholesterol embolization syndrome after cardiac catheterization].

The clinical features and outcomes of patients with cholesterol embolization syndrome after cardiac catheterization were evaluated. Among 4,920 patients undergoing cardiac catheterization during 1991 to 1996, the symptoms, signs, laboratory data, treatment and prognosis of eight (6 males and 2 females, mean age 69 years old) were reviewed who were pathologically or clinically diagnosed as having cholesterol embolization syndrome. All patients had more than two coronary risk factors. Mobile plaque of the aortic arch was detected in all five patients who underwent transesophageal echocardiograph

Idiopathic toe walking: a comparison of treatment methods.

Eighty children with idiopathic toe walking (ITW) were examined 2-8 years (average, 34 months) after initial presentation to compare outcomes [passive ankle dorsiflexion (DF) and parental satisfaction] after three treatment modalities: observation, cast/brace treatment, or surgical triceps surae lengthening. Overall, 32% had a family history of ITW, 28% were born prematurely, and 16% had psychomotor delay. Forty-eight untreated patients showed little change in passive ankle DF at final follow-up, and only 25% of parents were satisfied with the child's gait. Compared with untreated children, ca

[Treatment of fingertip defect injuries with a semi-occlusive dressing].

In the treatment of substance loss of fingertips it is crucial to maintain functional length and to restore adequate sensibility. By treating those injuries with a semiocclusive dressing according to Mennen and Wiese (1993), we were able to achieve both goals with excellent results, avoiding the necessity of local or regional flaps as well as shortening of bone to achieve primary closure. 82 patients with 85 injured digits were treated either conventionally (primary closure with or without shortening of bone, vaseline gauze dressings: 31 digits) or with semiocclusive dressings (54 digits). 42

"Extensor toe sign" by various methods in spastic children with cerebral palsy.

Upper motor neuron lesion in adults is usually associated with spasticity and "extensor toe sign" on plantar stimulation (extensor plantar response). There are various methods of eliciting this sign including the classic method by Babinski. Other methods produce this response when the area of reflexogenic zone is increased due to upper motor neuron lesion. There are varying reports of Babinski positivity in spastic cerebral palsy. This study was undertaken to assess the sensitivity of different methods of eliciting "extensor toe sign." An attempt has also been made to correlate the severity of

Diving speeds and angles of a gyrfalcon (Falco rusticolus).

An optical tracking device recorded the three-dimensional paths of 11 dives by a 1.02 kg gyrfalcon, trained to dive to a falconer. The dives started at altitudes up to 500 m above the ground and were inclined at angles of 17-62 degrees from the horizontal. The falcon controlled its speed during the dives, rather than simply falling from the sky, and the dives had three phases. During the first (acceleration) phase, the falcon accelerated to speed limits between 52 and 58 m s-1 in the seven fastest dives, evidently with minimum drag, because its accelerations were close to those predicted from

Homologous plasma-stored tendon grafting in equids--gross and histomorphological observations.

The efficacy of homologous plasma-stored tendon grafting in the superficial digital flexor tendon severed in the mid-metacarpal region was evaluated in 10 donkeys. Experimental tenectomies were performed, followed by grafting using plasma-stored tendon and human amniotic membrane encasement in group I (n=8) but no grafting or encasement in group II (control) (n=2). Gross observations in group I showed no peritendinous adhesions, except in one animal, while control animals showed thick fibrous adhesions and thickening at both the cut ends. Histologically, in the grafted tendon there was normal

Clinical outcomes of four patients with microdeletion in the long arm of chromosome 2.

We present clinical outcome, through several years of follow-up, of 4 mentally retarded patients, each with a small interstitial deletion in the long arm of chromosome 2, within a region on which clinical reports are infrequent. Our patient 1 was found to have del(2)(q22.3q23.3); patients 2 and 3, del(2)(q23.3q24.2); and patient 4, del(2) (q24.2q31). By comparison of our cases with each other and with those previously published with comparable interstitial deletion, we attempted to identify characteristic clinical findings. Short neck with excessive cervical skin was seen with monosomy of chro

Analysis of rapid stopping during human walking.

The mechanisms involved in rapidly terminating human gait were studied. Subjects were asked to walk at a comfortable speed and to stop walking as soon as they felt an electrical stimulus to the superficial peroneal nerve. This simulated hitting an obstacle with the top of the foot. Stimuli were presented repeatedly at random during a 20-min period of walking. Electromyograms and joint angular movements of the right leg and forces under both feet were recorded. The step cycle was divided into 16 parts, and the responses to stimuli in each part were analyzed separately. Subjects generally stoppe

Small-fiber sensory neuropathies: clinical course and neuropathology of idiopathic cases.

We describe the clinical features, natural history, and neuropathology of 32 patients presenting with "burning feet," for whom no specific cause was identified. All had neuropathic pain in the feet and morphological abnormalities of cutaneous innervation in skin obtained using punch biopsy. Most (29) had an abnormal sensory examination. All had normal strength, proprioception, tendon reflexes, and nerve conductions. Two clinical patterns were apparent, based on natural history and spatial distribution of cutaneous denervation. Most (28) patients presented with neuropathic pain initially restri
