Relationship between dermato-physiological changes and hormonal status in pre-, peri-, and postmenopausal women.

We previously reported that hormonal changes in perimenopausal women are associated with dermatologic changes. In the present study, we evaluated such dermatologic changes by means of dermato-physiological testing methods in perimenopausal women with various types of hormonal conditions. The study group consisted of 46 consecutive women 41-70 years of age (mean, 54.0 years), attending a climacteric outpatient clinic for a healthy check-up is whom no abnormalities were recognized. The women were divided into four groups according to menstrual history and sex-related steroid hormone values: a pr

CD40-CD40Ligand interactions and their role in cytotoxic T lymphocyte priming and anti-tumor immunity.

Tumor-specific immunity relies on interactions with the antigen receptors as well as costimulatory molecules, such as those of the CD28/B7 pathway and relatives of the TNFR gene family. Cytotoxic T lymphocytes specific for cellular antigens are in general primed by professional antigen-presenting cells that indirectly present antigens derived from cells in the periphery. This cross-priming of CD8(+) T cells requires signals provided by CD4(+) T helper cells. Although this dependency on [help' for efficient cytotoxic T lymphocyte priming has been well documented, it was only recently that more

[The diabetic foot].

Problems of the diabetic foot are frequent. The magnitude of the clinical picture and morbidity mirrors the severity and complexity of the underlying pathobiology. The three pathogenetic mechanism involved are ischemia, neuropathy and infection. Seldom do these mechanisms work in isolation, rather most foot problems result from a complex interplay among all three. The clinical picture of the diabetic foot reaches from the neuropathic deformity with diminished or absent sensation of pain to limited gangrene or superficial ulcer. The polymicrobial infection leads to extensive tissue destruction

Lower extremity preference during gross and fine motor skills performed in sitting and standing postures.

The evaluation of lower limb preference in physical therapy practice is critical in order for the clinician to assist patients with functional retraining tasks. No studies in the physical therapy literature present a systematic approach to determine the criteria needed to identify the preferred limb. This research was designed to present a series of tests for effectiveness in determining limb preference. The purpose of this study was to determine whether lower limb preference existed in a group of recreationally athletic women when performing either stability or dynamic skills with the lower e

The role of cell death in limb development of rats manifesting Lx allele on different genetic backgrounds.

Control of vertebrate digital pattern is a phylogenetically old mechanism. Animal strains with abnormal digital counts are a useful model system to study tissue, cell and molecular factors involved in limb patterning. The aim of this study was to investigate rat limb morphogenesis on gestation days 13 to 16 in normodactylous, polydactylous and oligodactylous fetuses where the deviation from the normal pentadactylous phenotype is caused by interaction of mutant Lx allele with different genetic backgrounds. General development was assessed by measurements of crown-rump length, and limb morphogen

Rat tail replantation as a training model for microvascular procedures of digit replantation.

In this study we sought to evaluate the potential of rat tail replantation as a tool for very-small-vessel microvascular anastomoses. We used 10 adult Sprague-Dawley rats. The tail was completely amputated 2.0-cm distal to the base of the tail. Then the tail was replanted with anastomoses of two superficial dorsal veins from both sides and one artery. All 10 replanted tails were pink, viable, and normal-appearing at all daily inspections performed from the first to the fourteenth postoperative days. This model can provide a training tool for the acquisition of superior microvascular surgical t

[Hand injuries in the users of hydraulic wood-cutters].

This study has been carried out in order to suggest possible prophylactic recommendations. We investigated the type of injuries, the type of logsplitter used, the injury circumstances and the safety measures. Fifty-two patients were included. There were a total of 21 crushing injuries with amputations of fingers, hand, on forearm. Nine replantations, two revascularisations and seven amputations were carried out. Eighty-eight percent of the patients had their hand in the splitting area. Sixty-seven percent had not received any instructions on how to operate the machinery. In 58% of the cases tw

Foot asymmetry in healthy adults: elliptic fourier analysis of standardized footprints.

The size and shape of paired structures differ in the left and right sides of the body. Shape characteristics should be analyzed separately from size to supply information about the normal variations of human organs. In the present study, the within-subject normal symmetry of footprint shape and size was analyzed from a mathematical standpoint. On the standardized left and right footprints of 46 healthy adults (23 women and 23 men; 19-26 years old), the outline of each foot excluding the toes was identified and its shape was quantified independently from size by the elliptic Fourier analysis.

Upper-extremity self-amputation and replantation: 2 case reports and a review of the literature.

BACKGROUND: Patients who deliberately amputate 1 or more of their own extremities present a unique set of challenges for the entire treatment team. Decisions regarding replantation of the amputated extremity must be made quickly, and the psychiatrist is involved early in the care of these difficult patients. Surgical staff may feel that replantation surgery is inappropriate for such patients, although there is limited literature addressing this issue. Therefore, the psychiatrist must also address the strong feelings that such patients generate in nonpsychiatric caregivers. METHOD: Two cases o

In vitro attenuation of impact shock in equine digits.

This study was designed to test the impact characteristics of the equine digit in vitro with the objective of providing a better understanding of the role of the digital structures in the attenuation of impact shock. Uni-axial accelerometers were mounted on cadaver digits on the distolateral hoof wall, the proximolateral hoof wall, the dorsal surface of the second phalanx, and the mid-lateral first phalanx. The hoof-mounted accelerometers were aligned with the hoof tubules while the bone-mounted accelerometers were oriented along the longitudinal axis of the bone. Each digit was mounted in a t

Influences of walking speed change on the lumbosacral joint force distribution.

To more understand the influence of the walking speed on the spinal joint force distribution, a three-dimensional biomechanical model was used to estimate the spine loads during human gait with three different walking speeds. This previously developed and validated model included a dynamic external model and an internal model with forces of disc, 8 major muscles, 2 ligaments and 2 facet joints at L5/S1 level. A linear optimization method was used to solve the internal model to estimate the L5/S1 spinal joint force distribution. The results of five young male subjects showed that the mean peak

Genetic heterogeneity of Saethre-Chotzen syndrome, due to TWIST and FGFR mutations.

Thirty-two unrelated patients with features of Saethre-Chotzen syndrome, a common autosomal dominant condition of craniosynostosis and limb anomalies, were screened for mutations in TWIST, FGFR2, and FGFR3. Nine novel and three recurrent TWIST mutations were found in 12 families. Seven families were found to have the FGFR3 P250R mutation, and one individual was found to have an FGFR2 VV269-270 deletion. To date, our detection rate for TWIST or FGFR mutations is 68% in our Saethre-Chotzen syndrome patients, including our five patients elsewhere reported with TWIST mutations. More than 35 differ

[Acute myocardial infarction due to traumatic hemorrhagic shock with open fracture: a case report of staged amputation and CABG].

A 55-years-old man was admitted to an emergent hospital with hypovolemic pre-shock and open fracture of the right lower extremity. CPR was needed for myocardial infarction induced by hemorrhagic shock. Wound infection and bacteremia (Enterococcus SP) were noted 10 days after admission. Severe three vessel disease was demonstrated by preoperative (amputation) evaluation, he was also referred for CABG. For a control of infectious focus before using cardiopulmonary bypass, two-stage-operation (amputation and CABG) were selected. Both perioperative courses were uneventful and the patient recovered

Impact of femoro-distal bypass on major lower limb amputation rate.

BACKGROUND: Limb salvage is one of the cardinal aims of vascular surgery, but the influence of distal bypasses on amputation rates has been difficult to demonstrate. METHODS: The incidence and indications for major limb amputations and infrapopliteal bypass during two consecutive 16-month periods were analysed. RESULTS: There was a decrease in the number of major amputations from 34 during the first period to 13 in the second, while the number of bypasses to the infrapopliteal arteries increased from four in the first period to 21 in the second. CONCLUSIONS: An increase in the incidence of

Effects of alignment changes on stance phase pressures and shear stresses on transtibial amputees: measurements from 13 transducer sites.

Interface pressures and shear stresses were measured at 13 sites on two unilateral below-knee amputee subjects ambulating with lower-limb patellar-tendon-bearing prostheses. Interface stresses at the time of the first peak in the shank axial force-time curve were investigated at different socket-shank alignment settings. Stress magnitudes ranged from 1.2 to 214.7 kPa for pressure and 0.4 to 79.6 kPa for resultant shear stress, and changes in stress due to misalignment ranged from 1.3 to 80.7 kPa for pressure and from 0.2 to 38.0 kPa for resultant shear stress. For both subjects interface stres

Mechanisms of cortical reorganization in lower-limb amputees.

The human motor system undergoes reorganization after amputation, but the site of motor reorganization and the mechanisms involved are unknown. We studied the site and mechanisms of motor reorganization in 16 subjects with traumatic lower-limb amputation. Stimulation at different levels in the CNS was used to determine the site of reorganization. The mechanisms involved were evaluated by measuring the thresholds for transcranial magnetic stimulation (TMS) and by testing intracortical inhibition and facilitation. With TMS, the threshold for muscle activation on the amputated side was lower than

Ten-year survival of Finnish lower limb amputees.

Data on mortality for the ten years following lower limb amputation were obtained from all the 16 surgical units in Southern Finland and the National Social Insurance Institution. In Southern Finland during the period 1984-1985, amputations of the lower limb were performed on 705 patients, of whom 382 (54%) were women and 323 (46%) men. The majority of the amputations, 47% were performed for vascular diseases and 41% were performed for diabetes mellitus. The overall survival was 62% at one year after amputation, 49% at two years, 27% at five years and 15% at ten years. The median survival afte
